I have visited my mom on several occasions at this point and, despite my 2 interactions with Sheena, the staff has been quite pleasant, helpful, and they really are trying their best to go above and beyond with answering questions and tending to her needs. It's nearly impossible to reach her by calling the main number, and I need to call 3-4 times before someone answers at the nurse's station, otherwise the rating would be higher. The place would actually be pretty great if management wasn't mentally and emotionally abusive towards me last week. It's already an incredibly stressful situation for my mom and for me and my family, and what the social worker did was just horrible. I shouldn't need to break out into stress-induced hives because of manipulation, coercion, and her intentionally causing fear. Not having the ability or bedroom to take my mom home immediately, and then having POA leveraged and conditional upon doing so, and using fear tactics that she would be sent to a shelter was beyond wrong and psychologically abusive. read more

I would not recommend Community Care Center to anyone who values timely and compassionate care. The facility is severely understaffed, leading to long wait times and frustration for patients. The staff that is present seems more interested in chatting among themselves or texting on their phones rather than attending to patients' needs. The receptionist could not even be bothered to greet/assist because she was watching YouTube on her phone. It's clear that patient well-being is not a priority here. The lack of urgency, poor communication, and general disregard for those seeking care make this one of the worst medical experiences I've had. I can guarantee you this is ran as a money grab and not an establishment that is meant to nurse people back to health. If you have other options, I highly recommend going elsewhere. The only exception to this experience was the RN Amor. She was so sweet, kind, patient, and helpful. Such a gem! read more

Community Care Center Review My 105 year old grandmother came to CCC for PT/ OT a week ago. The staff here is nothing less than remarkable. They are very attentive , kind, polite, and respectful in every way. Every single staff member we have encountered has been extremely helpful. The place is kept very clean and the residents are all well looked after. There is not an hour of the day that goes by that we are not asked if we need anything for our grandma.The staff is always checking on her to make sure she is kept clean, warm, and fed. I have seen them redirect wandering patients with compassion and kindness. We as a family are so thankful for the quality of care our grandma is receiving here. Do NOT send your loved one here. All we wanted for our loved one was a place where she would be comfortable and cared for; she received neither. There was little to no interaction by staff with her during the day, zero communication with us unless it was initiated by us, no doctor oversight of her care. The conditions at the center are horrendous: uneaten meal trays left all day, floors were consistently dirty, bed and clothing left soiled, an unpleasant odor upon entry, and on multiple occasions medication was found on the floor. But the biggest concern is the lack of care. My loved one was brought to an outside doctor appointment UNATTENDED despite us being told on two different occasions that she would be accompanied by a CNA. She had documented cognitive impairments that would require her to have someone with her. She wandered from the doctor's office and was found on the ground unattended. She ended up in the hospital and then passing away. PLEASE let this be a warning to a loved one's family and do not bring them here. read more

BEWARE!!! I'm really worried about what happened to my dad at Community Care Center in La Mesa. The button he uses to call for help didn't work well, so it was hard for him to get assistance quickly. When he made a mess on himself, it took a really long time, like an hour and a half, for the staff to clean him up. His roommate had a colostomy bag that spilled, making a big mess in the bathroom. It took the staff four hours to clean it, and the strong smell made my dad uncomfortable. When he had symptoms like a stroke, it took two hours for the staff to call for help. This is a big deal because when someone has a stroke, you need fast help within three hours to prevent permanent damage. On top of all this, my dad had a prescription for pain medicine, and it took the staff a whole week to give it to him, even though it had been ordered for some time. Waiting so long for pain relief is not okay, and it adds to the concerns about the care at this facility. They really need to improve how they handle medications and respond to residents' needs. read more

Overcrowded understaffed, no air conditioning, swamp coolers throughout the hallways, portable ones no telephones in patients room, pass around nurses phone in the station with germs to everyone no seats for sitting visiting lack of medical attention, like to give patients morphine around the clock until they pass without family, knowledge, nor patient. The quality of care is poor. The doctor never comes in to see any of those patients. parking lot too small have to park in busy street. The nurses station most of the time never answers the phone. I would never recommend this place for anyone to go to for therapy or aftercare from the hospital. They need to close this place down residents sit there, dirty stinky smelly food everywhere shit in their pants. I speak this because my uncle was there and that's what happened to him? They figure nobody comes to visit he doesn't have anybody they do whatever they want by the time we got in there was too late, can you lift size and he's crying and said he wanted to come home he was supposed to get therapy to get more therapy the morphine around the clock until he's gone. read more

11/15/2021 If you want to get a hold of your loved one on the phone, forget about it! We are in TX, so that was our only option. Such a frustrating experience to try to get a call through. Looking back, we should have gotten my father in law a burner phone or cell phone, to let him know we loved and cared for him on a regular basis.  We moved him to another facility that had all the same issues, so that was heartbreaking! It is too late now, so we are warning other people.  This facility was recommended by the ER doctor at Grossmont hospital. read more

How can you leave someone without an IV and not eating expecting him to survive while your sedating him? *DO NOT SEND YOUR LOVED ONE HERE* My 85 year old Great Aunt had the misfortune of staying here while recuperating from a stroke and developed bacterial infections & bacterial pneumonia 2 times! She was sent to Sharp hospital on Frost Street the 1st time. I received a phone call from Adult Protective Services due to care concerns observed by medical staff whereby a referral was made to San Diego County Health and Human Services APS this worker stated that the referral concerns were related to numerous, severe bed sores, hygiene and overall deteriorated health conditions. Prior to Community Care Center my Great Aunt had two months of daily progress with small victories each day in her recovery first at Scripps La Jolla and then at Select Speciality Hospital in Hillcrest. She didn't have issues with bedsores, infections nor bacterial pneumonia except her initial stint in the ICU (while intubated she developed pneumonia briefly) to reiterate she had two straight months of steady gains. Unfortunately it was just over a week into her stay at Community Care Center when problems developed...Once treated for the bacterial pneumonia and no longer feverish she was cleared to be released from Sharps and bed availability/insurance sent her back. It was almost identical timelines...just over a week later she developed bacterial pneumonia again! Had our family not had an outside nurse go to visit her, she probably wouldn't be with us. Community Care scrambled when he showed up placed a call to my relative to ask permission to cut a matted dreadlock out of her hair. They completely neglected her hygiene and her overall care was atrocious and heartbreaking. My Great Aunt is still recuperating from her 2nd trip to the hospital (over a week at Sharps Grossmont as I write this) with a plethora of acute bed sores and continuing to recover from the bacterial infections. Although the truth isn't always attractive we don't hide from it and I feel strongly that censorship is Un-American and dangerous (I'll elaborate on why I threw this comment into the post shortly) If this review can help a loved one and their family avoid the trauma and nightmare that my Great Aunt has suffered (by being transparent) then the emotions, time and spirit of this review are integral. My wife who is not (by blood) family with my Great Aunt was trying to help others as well and yet somehow her post was removed as she was "reciting 2nd hand and "not direct experience with this facility and their patient " (our family member and loved one) somebody or entity wanted to control the narrative and prevent the reality and truth of my Great Aunts treatment from damaging their public relations facade along with their profit before people incentive to suppress our voices censorship is never attractive nor democratic I think of totalitarian foreign dictators or misconduct coverups in recent years by politicians, corporations and law enforcement where the entire narrative and version of events does an about face complete 180 once video footage is presented and truth is allowed to prevail without spin doctoring. Sadly I don't have security, bodycam or bystander cell phone footage and as one review has already been removed I can only pray and hope that truth and integrity are allowed to remain and that ethics and morality prevail. I'm the only blood relative she has in San Diego let alone California my testimony is the time I've seen and spent with her (multiple times in this facility) along with my daughter and wife so if this is somehow heresy or 2nd hand information then perhaps the Idea of honesty and integrity is just that...only a concept yet undesired when it's not packaged correctly and bad for business. Perhaps the COVID-19 outbreak that went undisclosed when we first visited my Great Aunt upon her reception at Community Care is a good way to refute the narrative...we were all complaining about what felt like size extra small N 95 masks and the literal indents they left on our noses and cheeks (petty trivial complaints In comparison to what my Great Aunt has endured) to the extent that we all packed our own (plush and comfy masks in contrast) masks for the next visit. while checking in as visitors I noticed they had different masks and commented oh nice, you have new masks! The employee said yeah everybody complains about the N-95 masks we only make our visitors wear them during COVID-19 outbreaks at the center...the latest outbreak ended a few days ago...so good to know this after the fact...to all the families considering care and placement of their loved ones at Community Care I'm doing all I can to let you know before you jeopardize/compromise their well being. read more

Adequate care towards residents though some staff really brighten up the day and go out of their way to help anyone they can. There are a few cnas on station one at night who handle themselves really professionally and are very friendly. I noticed though that at night time some of the cnas were sleep at the station and no nurse says anything, and call lights were going off. Why are they working late but sleeping, isn't it their job to be up? I don't know if they were on a break but if so why would you sleep at the nurses station, that's very concerning. read more

Things start to look promising, then we lose 2 of the best CNA's. They really completed the morning crew. I knew when I didn't have the expertise of Elizabeth and Cristal, I could count on these powerhouses. The talent and hard workers is significantly slashed. They worked extra shifts no matter what and didn't give anyone a hard time. This is going to affect EVERY shift because that's what assets they were. I'm trying so hard to give this place another chance. But they want me to look forward to Coachella without any headliners! We have a couple of nurses here I would much rather trade places with them. Luckily there's still some good people. like Sheena. Perfect for her job. Always a pleasure to work with. Max who helps hold pm down. He doesn't half way do anything. His showers are so complete and caring, some people refuse to have anyone but him. Jazmine just helped my neighbor immensely who has begged every single CNA to help her with her closet for days. She was just passing by, too. Cat the new nurse is perfect. Wouldn't change a thing. We'll all miss Art who left too soon. Great sub acute care for my mom. Staff in every department here go above and beyond to make sure my mom is taken care of, happy, and on the way to recovery. Leaving the hospital after critical condition in the ICU for four months was the scariest experience in the world, but it is comforting to know that my mom is being well taken care of here. We almost lost her and it is a gift to spend time with her here. The RTs, LVNs, RNs, are knowledgeable and ready to help. The CNAs really go out of their way, and treat my mom like family. The activities team are very hands on and get involved in what she likes to do. The social workers and PTs are always responsive when I have questions about my moms healing, safety, and recovery. PT director is always on top of informing me of any changes to the facility. The dietitians and kitchen team look out for her. Thank you Community Care Center!! Highly recommend. Had the worse most infuriating experiences at other San Diego sub acutes and Community Care ensured us with the complete opposite. read more

This facility had soul, excellent customer service experience and patient care! The nursing staff here are very caring, alert, friendly and compassionate. Support staff and the cleaning staff were very well going above and beyond their duties. Nutritious meals and healthy snacks were served during my family member's entire stay. Everyone was so friendly and I felt that my family member was genuinely cared for the during their whole stay. Even after a week time that my family member was discharged, the admissions and marketing team from called my family member at home to check her health conditions and recovery after the rehab from her fall and surgery. We often receive calls from the facility inquiring about my health conditions. I am so impressed and happy with the level of care being taken even after being discharged. One way to assess if a facility has great care quality is when you have people who has been in the facility for long.... that means consistent care for patients. I went to see my Mother and I felt positive vibes walking into the facility. My Family and I were welcomed by the kindest LVN named Erondira Hernandez. I believe it was her 1st week and I was very happy that Erondira was caring for my Mother. She was understanding and explained how my Mom was doing. I also enjoyed the facility. I went out the sliding door and it almost felt like I was in a condominium community. I look forward to the day that my Mother is able to sit in a wheel chair and we can have a picnic with her and take the first steps to put this behind us. read more

If I could give no stars I would. When you lose your father the last thing you want to experience is insensitivity by the people who were the last ones to see him alive. My father passed away on a Friday and I went in the following Monday to collect his personal effect including his wallet with all his banking and identification items. I was greeted immediately by the receptionist and she was very pleasant. That was the only good thing about this day. We were acknowledged ten minutes later by a gentlemen saying they were gathering my fathers things and would be with us shortly. An hour and a half later I had to ask what was going on as we had seen the maintenance man who was supposedly looking for the items walk in and out several times. The entire hour we were sitting in the lobby we noticed a patient in a wheelchair. When we first arrived she claimed she was cold and the nurse left to get her a blanket. By the time we left she was still sitting in the same place cold and looking lonely as can be. Finally someone came up to talk to us, brought the inventory list of my fathers personal effects and stated that his wallet was up in the safe. We were then told that me, his daughter, was not on the list as an emergency contact so I couldn't pick the items up with out my grandmothers permission. So we left under the instructions to obtain a letter from my grandmother would couldn't come down because she was undergoing chemo sessions and a promise from Maribel that she would call me once she confirmed the wallet was in her safe. I waited a full day before calling Maribel and left 3 messages in 3 days with never a call back. We came back three days later and when I walked up to Maribel who had seen me but was trying to rush off as if she didn't she instructed me to go ask the receptionist. She acted as if she couldn't be bothered to deal with us. Remember I had just lost my father earlier this week but its obvious there is no compassion to be found in this place. We produce the letter, and another hour later the lady who showed me the inventory list stated that there were no personal items in the hospital and that the letter I brought was unacceptable. My grandmother would have to come down. I complained to the receptionist and unfortunately she tried to make an excuse for the way we were treated. They were being inspected by the state. Honestly, I can understand why they were so freaked out their service is inadequate to be kind. So an entire week goes by and my grandmother was able to make it down to this place to give it a try. Things went very bad from here. They stated they didn't have the wallet and had no record of it. Remember I had been shown the inventory report! So my grandmother had to threaten to call the police before they would try to track down the wallet and promised to mail it out. A week later they did call back but said they had found the wallet with the mortuary and had mailed it out. I am sad to report that they are dishonest and despicable people who cannot simply admit they had problems locating it. When I picked up my fathers ashes from the said mortuary who supposedly had the wallet I tried to thank them for locating it and I was sad to learn that they never had it and didn't understand why it was said so. Happily we did receive the wallet in the mail. Now imagine leaving a love on in the care of these people. They cannot admit when they make a mistake. They treated the grieving family with little compassion and neglected to acknowledge us for more than an hour. What do you think they will do to your loved ones who need them to look after them, provide their medications and be expected to treat them with compassion and like human beings? We all hear about the terrible things people do to the elderly and I am afraid to know what would have happened to my fathers identity or banking information if we never retrieved the wallet. Its a sad thing to think of in another human being but if the actions deem it then so be it. Please note my father was brought here after leaving the hospital before any of the family could be notified otherwise we would have looked into the facility. Please look into any place your family member is taken to or considering going to because the last thing you would want is to have to deal with this kind of stuff. read more

The nurses , the CNA's, the people that are in the physical therapy department, nurse practitioner, and case managers, and Joellen all have been just unbelievable. I am planning on having surgery in October and i will definitely be coming back for rehab. read more

Checked out this facility and was highly impressed. They just finished a remodel, and the place is stunning! One of the first things I check when going to a facility is if there is a smell. This one has none, which says something about the care provided. They have a state of the art subacute unit, and are super close Grossmont hospital. Horrible experience. I was kicked out of grossmont hospital a only 24 hours after a total hip replacement. I had to go to this place and I started off with all kinds of problems. The worst was no meds for several hours the first night , waiting for the pharmacy to deliver my meds. Finally at 9:30 at night . I was promised I would get my meds at 1:30 am . I woke up in terrible pain at 4 am no meds. They tell me after 30 minutes that they have to get the meds from the pharmacy. I told them over and over that the meds were deliver last night they argued that they were not after a half hour of arguing I finally got only some of my meds. I want to leave this place. It has been a total nightmare. I am right outside the nurses station and I have to hear them all night long talking loudly and everyone buzzer going off. I can not sleep or relax. read more

I don't know where to start. So I will just put it all out there. My father got COVID that effected him neurilogicly. The only facility was this one or one on Anza. This one is closer to my Mom so this was it. After the 9 day quarantine we FINALLY got to visit him. He had someone else's clothes on and the facility said they could not locate his. Other than 2 times to ride the PT bike his only PT was walking the hallways. The TV was so loud he couldn't sleep. In his 15 day stay he NEVER got a bath. The director tried to keep him for the 20 days Medicare would pay @100%. I had him discharged as soon as he could walk on his own (15 days). When I went to pick him up one of the nurses was vomiting in the bathroom and when I said something she said "what can't someone have a day when they don't feel good". I was glad to get my dad out of there. If you are able to stay on top of your loved ones care DON'T put them here. read more

I am impressed with Community Care Center because of the outreach the staff does to maintain great follow up with families, communities and hospitals to provide the best experience. I recommend having your loved one here as I recommend to anyone in need of a skilled nursing rehab facility to go to Community Care. One of the residents from our senior living community needed a short term rehab stay and from day one Dina was in constant communication with me and our nursing director to make sure that we were kept in the loop and that she would come back to our community quickly. This place is a most horrible place ever please do not put your parents in this nasty nursing home. MY mom was in there for 3 months and she got plenty of infections she was always left in soil and left dehydrated nobody there cares and the nurses ain't no good.The CNAS is stealing money from the patients. I don't know who run that place they need to shut them down help me report this place. The staff are always short and very mean to the old people in a lot of people can't report them because they don't know no better. Oh God please save your loved ones or yourself! I swear I was greeted with disdain from the beginning. There was few choices to go to rehab because of flu outbreak so out of the blue I chose this place. Very bad leaving it up to chance. When I arrived via ambulance they acted as if they didn't know I was coming. They put me in a room with a bed that sagged to the floor. Highly not advisable for a patient with hip precautions. I somehow managed to sleep the night. The next morning I woke up in very much pain. I asked for pain medication and the nurse just stared at me like I wasn't there. I was able to get out of bed and chased her down asking for pain medication. Once I reached her at the nurse station she looked at me and said, oh so you need medication? The other nurse's sat there and laughed. I was also in need of a raised toilet seat. I waited 3 hours for toilet that was high enough for me to sit on. This was only the beginning of this awful stay. I had many questions for social services and all they said was I'll get back to you. She didn't get back until a day and a half later! Every single person except maybe Dorien, lie straight to your face or indulge in passing on to the next person. I am not kidding! When you ask someone something don't let them say to you I'll be back. You end up having to chase them down to get answers. I was asked if I wanted CPR performed on me should I stop breathing and I told them, (hell, no!) When asked why my answer was if I died and had to come back to life I would not want to come back to life to be in this place again. There are a few good nurses but few and far between. Look up their sister facility called lifeouse and you will definitely see you don't want either of these places. I am discharged tomorrow so I may still have plenty more comments to make. She was very nice and even though she seemed very busy she took rusting to genuinely answer all my questions. The employees here don't seem to know the simplest basics of patient care, such as turning a patient every two hours to prevent bedsores. A friend of ours who is comatose was placed in this facility. He was there for a few weeks. He then developed a fever and had to be transferred to a hospital. According to a nurse at that hospital, he now most likely has pneumonia. He also has one of the hugest bedsores she has ever seen. They are calling in a wound specialist to look at it. When we visited our friend once at Community Convalescent, he was lying flat on his back with his head slightly below his body. This can cause choking and aspiration pneumonia and shows extremely poor skills on the part of the staff. We had to call someone to raise his head. We were also informed that he had a bedsore. When we called Community Convalescent to report the poor care our friend received, we got no cooperation at all. The employee with whom we spoke asked only if we were family members and then proceeded to shout on the telephone. These are the facts. There is no way that anyone should be placed in this facility. A couple of bumps in the road but overall it was a great place to stay. One of the top places I would recommend to someone if needed a place to recover. The call light needs some improvement. There was times when the response should of been a lot quicker. I get only so many cna's on the floor but unlike myself some residents can't do anything on their own, what if it was an emergency? Sanitation would be a B. housekeeping does what they got to do. Food is decent, needs to be served faster. Certain foods depending on what you are serving are only good when they are hot straight from the kitchen. Didn't really go to activities much, stayed on my patio. Relaxed and recovered, focused on myself so that I could have a speedy recovery. Physical therapy is a fantastic team. Great people put together and very knowledgeable. They made sure I would walk out the facility and learn how to stand on my own. honorary mentions go to Shelly and Dustin, Two important roles in my recovery. We bonded so smoothly and knew I could always depend on them to get the job done. Alyssa my nurse another honorary mention. She went above and beyond when it came to my care, better yet my entire stay at this facility. Always made sure I was taken care of and my needs were met. any questions I had, she had an answer. Never once she didn't know anything. Most of the cna's I had were good but two if them stuck out and i'll always remember their hard work and care. Seth was exceptional, never missed a light and always had my back. Catherine i think that's her name was great as well, very helpful. Better yet everyone from the lady at the front desk down to administration was good to me. Thank you community care. It is very difficult to call my mother there. They use a portable phone and they only have one so usually it takes up to an hour to get a call thru. If you need to be able to call the patient, try someplace else. read more

Worst experience ever! Nurses are sweet but the administrative staff is a complete joke. No professionalism whatsoever. Lost almost $300 worth of personal items. Took 3 months to get them to do anything, and that was only because we finally kept showing up and making a scene until we talked to the "administrator". They have the worst communication ever. After a month my grandmother was not any better at all! After getting her home, the in home physical therapist who came for an hour two days a week did so much more for her than the place she stayed 24-7 for a month. They are all about money. I had to badger someone to tell me when we'd have to pay out of pocket. They finally told me two days before we'd start getting billed $148/day! Had I not asked, I would've gotten an enormous bill that I didn't have the monetary resources to pay. They lost medication. Her glasses. I can't even explain in words enough about how awful this experience was I'm so many ways. And not once did anyone contact us to let us know they were on top of things I'm trying to find the missing items, that's after me writing a list 6 times and providing to 6 different staff members. They never apologized. They were irritated and rude and completely unhelpful. After all was said and done, we had to wait nearly 4 hours to leave because their either didn't let administration know we requested discharge do we wouldn't pay out of pocket. This is not the place to take your loved one! They are grossley under staffed they do not stock enough patient supplies or enough bed linens or towles for patients to bath. The director of nursing over steps boundries making medical decisions for patients before consulting a doctor putting the patients heath at risk even over riding nursing staff assesment and recommundations to send patients to the E.R for proper treatment.
