He got an infection due to neglect, an open pressure sore, his clothes were only changed 3 times in a total of 4 weeks, and he had two showers in a month. I was very clear with them about me doing his laundry to ensure nothing was lost. Even still, they took his laundry and lost his clothes. I visited him every 2-3 days and spent most of my time at the nurse's station harping on them about lack of care and why he had become wheelchair bound after several weeks there when before he got there he was walking. Myself and other family members would find him sleeping bedside on his wheelchair on a regular basis. He couldn't reach his call light from the side of the bed, and he shared they never checked on him. He got an open sore on his tailbone from sitting in his wheelchair 24/7. The photos they posted are fluff; the place is a totally apocalyptic like. Dirty, old and broken, wreaks, and residents all over struggling to get around on their own. Getting parking in the bottom lot or the garage was usually a challenge. The outdoor areas were also filthy and gross. No real place to visit with residents. And sadly, the employees do not care (aside from Bong and Alex). Social services - useless.. she was aware of these issues and agreed that there was a lack of proper care at a level she wouldn't allow for her own self or family member and nothing changed. Even after a meeting with the ombudsman, the neglect continued until I discharged him because I was terrified he would die there. He went to the hospital for SOB and was sent to post-acute to gain cardiovascular strength and improve his ability to move around (in and out of bed, up and down stairs, walking longer distances). He was admitted to this facility relying on only a cane to walk short distances, he left unable to walk AT ALL, even with a walker.

They are all caring, understanding in all of the different ailments from all kinds of different patients. I want to thank Jacalyn, Jim, Claudia, Max and Ranon for all of the therapy and helpful ideas to continue my own therapy at home. The facility is very clean and doesn't have any kind of smell to it. I like the fact that they have ice water available all the time and staff will come around and ask if you would like ice refill and even a water refill. I would recommend this place for injury rehab and for your elderly loved ones.

The commendable staff, shout out to Rick (RT) and Nora (Nurse), for being great! My family member said that these two were especially informed and very very professional. Not only were they phenomenal but the facilities were similarly great. I don't understand all the negativity in some of the reviews, but from the grounds, treatment, staff, management to service, everything was top notch. I'm just saying, if I had to come down sick, Avocado Post Acute would definitely be my place to go.

I called Bradley Court looking for placement for my mother …read more I was transferred to Becky ( social worker) ,I asked if she had any available beds for my mother who needs skilled nursing she said no.. I felt defeated , once again.She was probably the 10th social worker I had talk too. But she stayed on the phone with me and asked me questions about my mothers needs which really surprised me ,but was nice because I felt she truly cared .She took the time to talk me through all of the steps I needed to take to find my mom"s placement .She doesn't realize what a great relief she gave me and helped me understand this process so much better. Because of Becky I was able to find my mom placement and I just want Becky to know how grateful my siblings and I are to her for taking the extra time to share her knowledge with me. She will never be forgotten by my Family Becky is an amazing social worker and her Company should be proud and grateful to have her at Bradley Court

My mom had worked there for about 35 years and they welcomed her back as a resident with open arms. Ana always was there to answer questions quick to respond on emails and calls. Working with Ana was a fantastic experience; she was incredibly knowledgeable and efficient". Ana always displayed high-level professionalism and made the entire process stress-free. I highly recommend talking with Ana when considering this property for your loved ones. Ana went above and beyond to solve our issues or concerns I had. I did look at other properties in San Diego and they looked and felt like dorms or prison not to match the price. The Gardens had the lowest price for the best clean rooms and staff, the caregivers are very nice always there when needed. The Gardens felt like Family Ana made us feel like family. My mom is happy and safe. My 2 concerns: 1: with the wait staff and servers most of then were nice and friendly but a handful were always grumpy and just not professional.. They are not quick to see if someone comes in to eat and take there order or even clear the tables so other people can come in and sit. February 15th 2025 he was sent to Cottonwood & was released a few days before Christmas to a different facility closer to our home. I have to say Cottonwood is a wonderful SNF. Everyone was great. The place is clean. The nurses, CNAs, & therapist took such great care of him. If there's an issue they address it immediately. My husband is a very picky eater. Unfortunately we live in San Diego & Cottonwood is in El Cajon, so it was difficult for me to get there every day. The receptionists Arecelli & Ally made sure he received any food I ordered. Everyone is friendly & helpful. Alexandra was a great help making appointments & setting up transportation for my husband's doctors appointments. I definitely recommend Cottonwood. Thank you everyone. My husband just told me they called to check up on how he's doing. I miss this facility.

Behind the parties and smiling photos, many residents suffer in silence…read more In my experience, residents who could speak clearly, defend themselves, or had respected backgrounds received more attention. The weakest residents -- especially those with memory problems -- were often neglected. My husband was sometimes left hungry unless we brought food or fed him ourselves. He loved showers, but later he cried whenever they tried to take him for one. After baths, soap was often left on his scalp, in his ears, and on his feet. His feet became covered with thick white buildup. I was first told it was dry skin, then fungus. But when I personally washed and scrubbed his feet carefully, layers of old soap came off and the white buildup disappeared. I often had to return after his showers to dry under his arms and between his toes myself because residents who could not care for themselves properly were not fully cleaned or dried. My husband was also dropped on the floor. If we had not arrived unexpectedly, nobody would have known what happened. Many times we visited and found him crying, begging us, "Please get me out of this prison. This is torture." Some CNAs were compassionate angels, but others treated residents as burdens instead of human beings. Families should visit often and unexpectedly. What the public sees is not always the reality behind closed doors.
