Hello, I'd like to preface this by saying that I don't normally write reviews - only when an establishment goes above and beyond will I take the time and effort to let the world know about them. On the flip side to that, if the establishment is very poor, I will also steer people away. This here review, is for the former. I applaud everyone at Avocado Post Acute for a wonderful job well done. So here is what happened. One of my close people came down with COVID-19 and was sent to Avocado Post Acute for treatment. The commendable staff, shout out to Rick (RT) and Nora (Nurse), for being great! My family member said that these two were especially informed and very very professional. Not only were they phenomenal but the facilities were similarly great. I don't understand all the negativity in some of the reviews, but from the grounds, treatment, staff, management to service, everything was top notch. I'm just saying, if I had to come down sick, Avocado Post Acute would definitely be my place to go. read more

Worst place ever. They are now called Stillwater and still horrible. My sister had to be rushed to the hospital 911 after suffering for 3 days. The hospital turned them into the state for neglect. Her ostemy was so backed up she went into Septic shock and passed away after a week of suffering. They would leave her to sit in a wet diaper for at least a hour. Her bedsore became so infected it got gangrene. The outside wound care specialist called and said to get her out the day before she went into septic shock. They should not be in business. If your loved one is in need of special care do NOT put them here!!! read more

Let me start off by mentioning the stop all I can say. Is this place has helped me recuperate and most of the staff here are wonderful? I don't have enough work to express my gratitude towards them. There's a gentleman named yasser. I believe that's how it's spelled I'm not too sure. And a lady named This is a kind of service I mentioned that I wanted coffee will. Do you know he walked to 7011 and bought me a full cup of coffee? I couldn't believe it that's as generosity goes beyond anything. I could expect from a place like this so let me start up by just letting you know. If you have to be anywhere this is the place to go. Even to director Sean has been made it his priority to help me out in any way. You can not too many people will do that I really appreciate. Avocado was fantastic. The nurses were speedy and I enjoyed speaking with them everyday. The physical therapy was very helpful. They've turned my recovery into a very rapid pace. I went from barely walking to using a walker and/or wheel chair. I would highly recommend sending someone that you love to avocado. Thank you so much! read more

My 84-year-old Alzheimer's suffering Mom spent almost three weeks at Avocado Post Acute, and I was completely happy with the empathic and skilled medical care she received. Even more than that, I was pleased with the way I was kept informed of Mom's progress. The team helped my mother get through a serious health scare, and I am pleased that we were able to discharge Mom from Avocado Post Acute to move her to her new memory-care home. One final comment: I felt a very strong team-spirit in the facility. Whenever I was visiting Mom and needed something, I would just find someone and ask for help. Uniformly, that person would take ownership and help me, or find the right person who could help me. The nursing team at Station 1 NEVER left me hanging! read more

First of all i was very leary to bring my mother here given all the negative reviews this place is beyond horrible to say the least. My mother had almost a two month stay at this place was sent her to rehab and her condition got worse after arriving at this place. My mother had a colosotmy bag and it would burst on her daily they would take literrally hours to come and change it daily she would sit there for hours in poop which lead to her obtaining sepsis and ultimatly almost claimed her life ahe lay in the hospital bed almost a day and a half until someone noticed she was unreponsive she was rushed to the hosptal put on life support for almost 2 days thank god shes still here please dont bring your loved ones here they will recieve serious neglect warning " stay away" from this place unless you want your loved ones to die. read more

salina onevathana Respiratory Therapist is the best. She's willing to go the extra for each of her patients and is consistent in her daily work. She takes time to listen to your concerns and deals with problems if they occurred. she makes me feel like a patient and not a number. She is straight to the point, a good trainer because she explains thoroughly the process of getting better and going home. This place is disgusting, not only do they have one of the highest rates of Covid deaths they don't take care of the clients. I work in the health care field and we get some patients from there. They often come in their pajamas or with out socks and shoes on and they come filthy. I don't understand how they can be Medicare approved. Obviously the state and local agencies aren't doing due diligence. If I could give it less than 1 star I would. I beg you don't take your loved ones here. read more

My brother spent the last 2 years of his life at Avocado Post Acute. He suffered from multiple health issues for the entirety of this time. There was an adjustment period of several moths when he first arrived, however, over time, he came to know and love the staff. They did their best to care for him and provide him with the care he needed. Things were not perfect, but the staff here does their best with the resources they have and appear to genuinely care about the patients. As far as visitations I have no complaint. Me and my sweetheart have had 4 window visits and 3 face to face visits. All my visits have been without any problems and am very happy about that. Rachel, the caring person at this facility has been very professional, courteous and caring about our visit's. She had asked us is we had concerns about my sweetheart's stay and we said yes and voiced our opinion. She had a form that she completed and told us that it would go to a person in management. Some of the matters have slightly improved. Shawana, another staff member has also taken care of my sweetheart by fixing her hair and with her grooming. Very much appreciate that. read more

I am a very happy Resident / Patient at Avocado Post Acute Care Center (SNF). Although the treatment is extremely good what really impacts is the way they treat everyone. Everyone goes out of their way to be especially kind to everyone: patients, other staff and visitors: everyone is treated with excellence and genuine kindness all the time. Even late at night people are polite. In the midst of chaos there is healing calm. In my career before I became disabled, I spent time in a lot of my competitors' resorts (Hyatt, Marriot, RCI, Wynn, Wyndam, Etc.) evaluating service. I will gladly tell you that Avocado beats them all. In every department everyone tries to prevent, then resolve any concern or connect to someone who can. Reminds of the military where they will reinforce and enforce the guiding principle that you don't have to meet the service standards, however if you want to serve with us, you will either step up to this service standard or step away from us. It is that simple. If you are not totally onboard, then you are offboard. Strictly by choice, so people are glad to be here, and it shows in team (Together Everyone Achieves More) attitude & aptitude. Adapt, improvise & overcome. No excuses, no exceptions; just be glad and get it done: right the 1st time. Although the official rating system is 1 up to 5 stars Avocado earns the 6th star for spectacular service. Thanks! This place is terrible. I worked here before short staffed all the time specially Night Shift they don't care about the employees here. Specially the good ones specially, when there's a concern they just brush you away I had a sick uncle that stayed in station 1 terrible service on the p.m. shift they took forever to answer his call light wrong someone needs to go and check station1 it's terrible that place is all favoritism. They don't focus on the good Cna's no credit at all for the good ones. They only give credit for the bad Cna's who, gossip all the time and don't do their patient care good thing my uncle is no longer there stay away from Avocado post Acute. This is the worst care facility I have ever had anything to do with. When I came here the room was filthy the toilet was stopped up and stayed that way for a day and a half. There have been spiders crawling around on the floor I did not have any clean sheets for the first two days I was here. There was poop on the wall of the bathroom and the sheet on my bed had poop on it when I came in the room. Whenever I ask for a nurse to come in my room by ringing a bell it would take 30 minutes sometimes longer to have anyone come in my room. When I woke up at 7:00 the morning I told them my oxygen tank was empty. I have been on oxygen in the hospital for the last 10 days. I told them I needed oxygen and they did not give me any oxygen until 1 o'clock today. If they had a reading system of one through 10 I would be giving them a -10 read more

WARNING: If there is any other facility you as a potential resident or the loved one of someone who would be staying here, DO NOT take them/go here. My grandmother has been a resident since January 5th 2023 and she has gotten worse. many times her oxygen tank fluid was not refilled, her room had gnats or some sort of small group of flying bugs swarming around her over a week and no one did anything about it. her blankets, dialysis pillow, wheelchair, cane all STOLEN (wheelchair belongs to the facility was in BAD shape and she ended up tipping backwards and got a knot on the back of her head. she was not taken to an ER and no one called me, I found out from her. There was some conflicting stories about how it happened and who was helping her-the Nurse Supervisor didn't speak with the staff on shift when it happened and told me there were cameras that could reveal the answer.I called the social worker and right away and was told the cameras were BROKEN. I was lied to many times about my grandmothers health. most of the staff don't speak clear English so my grandmother was unable to understand them much of the time. On shower days the water was either ice cold or scolding hot and she had shampoo and body wash squirted on her and told to wash herself. There are so many more horrible happenings and I would help her get out of there but I live on the East coast. VERY sad and devastating. The social workers and the rarity of a good nurse and a few nurse aids have brought me and my grandmother a small amount of being okay. read more

I hope no one gets stuck having to deal with this facility. They are not here to rehabilitate or aid your family member. They are here for a paycheck and an insurance payout. They gave my father the wrong medication, left him without a few doses of medicine because they're were "short staffed", and he had to wait weeks to see a doctor. My dad was in a coma for four weeks due to COVID 19 complications and after he (thankfully) woke up, he was transferred here for rehabilitation. It was a daily nightmare. Everything from missing medical records to them hanging up my calls happened during the month long stay. Social workers, directors and RECEPTION does not care to help you. I really hope you don't get stuck having to deal with this team. read more

Rude social worker by the name of Jessica, lies about taking notes and never calls back unless they want to kick your loved one out. My mother was there for about a month, but she rarely saw a psychiatrist or doctor for her condition. She eventually contracted what looks like a severe rash. They only gave her vitamin d creme which did nothing but made the rash on her feet worse. Now that she is out of that sh*thole, she's going straight to the ER to treat this skin condition she contracted there. Like someone else said, I wouldn't send my worst enemy there. Place has a bad stench of sewage/feces, place is dirty, and though there were some staff that were nice, the few bad reps killed it for me. Especially the person named above. read more

I have my uncle who previously recovered from Covid. The doctor said his case was pretty severe. He stayed in the Sub Acute unit. After 7 long months, he walked out of Avocado. It was thanks to the nursing staff, respiratory therapists, and rehab team. Keep it up everyone and stay awesome. read more

I would be very wary of having any loved ones in Avocado Post Acute. Unfortunately, I have had to deal with multiple skilled nursing and post acute facilities because of ailing parents, and Avocado is by far the worst facility I have ever dealt with, bordering on criminal. They do have some very kind and caring staff at the facility, but the administrative and organizational procedures and communications are atrocious. My father was there recovering from a toe surgery. He was not always a model patient or the picture of health upon entering, but he declined so rapidly during his few months there. Because of the covid pandemic he had to remain at Avocado past his discharge date. I discovered the facility had positive Covid patients in his unit, and they never informed his medical team or any family. When I found out I called that day to try to get him moved. As per usual, I had a difficult time getting through to anyone. My calls to the nurses station usually went unanswered and disconnected about 75% of the time. I had to have his social worker call Avocado to get through to anyone, and then they lied to her saying he had been transferred to a different unit when he had not. He ended up passing away in Avocado that night. I was never informed he was ill or not doing well in my many calls that day or in the days prior. My aunt called to check in on him that night (since his cell phone was stolen, for the second time) and was told he was non responsive and they thought he wouldn't last through the night. They then asked her if she wanted to uphold his DNR....As his medical power of attorney, I should have been called and informed of his decline and asked that question. I was never even called/notified when he died! To top it all off they ended up sending his body to a completely different funeral home than what we had arranged. I had to spend half the day after my father died trying to find his body. Unbelievable! When I got his medical records, there were no notes about his being sick or being tested for covid, but he died of "an unknown organism or pneumonia". I don't think that happens in a matter of hours. He was obviously sick well before his death, and nothing was done. I would think standard protocol when you have an outbreak would be to test everyone in the unit/facility and monitor those with symptoms. I realize that these are unprecedented times, and a lot of facilities have been overburdened and under supplied, but that is no excuse for the lack of communication and LYING. This was only the most egregious of many issues my family and I had with Avocado Post Acute. I truly hope this doesn't happen to more patients at Avocado, especially those who don't have family or advocates looking out for them. read more

When first admitted IN SEPTEMBER 2021 I lost 30 pounds. I was NOT getting enough food. For a man 6'2" & weighing 200 lbs. to get 2 small sausages & 1 slice of toast for Breakfast was a joke. Lunch & Dinner were not that much better & no one seemed to care no matter what I said (even that I 've already lost 30 lbs.) After being stonewalled from seeing the dietician multiple times I finally met with her. Her name is Jackie. She is a WONDERFUL human being. She SAVED MY LIFE! Jackie revised my diet with much more Nutritious Food so I started to gain back the weight I had lost. Once that started to happen I started to regain my strength so I was able to do Physical Therapy and finally get out of this place. The kitchen was Very Uncooperative. Even though Specific Food items were Ordered & Specifically Written on my Food Ticket the Kitchen omitted Giving Them To Me Once or Twice a day EVERY DAY. I had to Yell at the Staff to go to the Kitchen Immediately to get my Omittrd Food (otherwise the kitchen would close & I couldn't get the missing food.) As you can imagine this made the whole process of eating (which should be a pleasant experience) Very Unpleasant & Stressful to me. To be fair there are some Wonderful People working there who have Kind & Loving Hearts & Really Care for their Patients. The Nurses : Lisa, Kim , Sidney & Craig are all like that. The people Supervising everything called CNAs (for being Certified Nurses Assistants) : Conrad, Rosie & ,Miranda are All GREAT CNAs. There is a Social Worker named George who is really WONDERFUL He also really Cares about people. George helped me a Great Deal & even went Beyond the Call of Duty. I will always be Grateful to George for his Assistance and Who He Is. I also experienced a Lot of Incompetence & Irresponsibility with Avocado. I've been on Oxygen 24/7 for over 2 Months. When it was time for my Discharge there was NO Oxygen ordered or delivered to go to my house.(Even though it Was supposed to be Ordered). Adrianna (a Social Worker) really Dropped the Ball on NOT handling that and saying that she would stop in & see me after 1:30 P.M. Thursday to fill me in about my Discharge the next day (Friday). Not only did Adrianna never show up to see me on Thursday she never even had the courtesy to notify me that she would NOT be coming to see me. For a person in a Supervisory position to do something like this is VERY INAPPROPRIATE & definitely sends the WRONG MESSAGE, There is a woman named Christine who is a Unit Manager who is supposed to handle recurring problems.When I contacted her she Never got back to me. I finally had George contact Christine . She told George that she would come see me that same day. Not only did Christine NOT see me that day SHE NEVER EVER CONTACTED ME. This DEFINITELY SENDS THE WRONG MESSAGE ESPECIALLY FOR SOMEONE IN A SUPERVISORY POSITION. I stilled needed additional Physical Therapy & the Home Health Agency Avocado ordered for me does NOT service my Area. Karin the person in charge at Avocado really Dropped the Ball on This. This has delayed me getting the Additional Care I Need for around 10 Days. When it was time to get a ride home from Avocado DO NOT USE PRESTIGE (OR PREMIER) SERVICE.) The Driver & Owner Ricardo is NASTY & MEAN SPIRITED. When he dropped me off at my place he REFUSED TO HELP ME CARRY 2 BOXES (ONE HEAVY) UPSTAIRS. He was more interested in picking up his next passenger THAN IN HELPING ME. Ricardo also Texts while he drives which is also Dangerous & Illegal. In summary I hope that this Review is helpful. If you choose Avocado hopefully you'll be able to Prevent & Avoid what happened to me. I'd also do a Very Thorough Review on Similar Places. When i walked in into the building and I noticed that the staff is very nice and friendly. Specially the admissions ladies and Marketers. The patient is very clean and looked happy. And that's when I decided to put my grandmother at avocado post acute. And I made a right decision. My grandma was very happy and she loved the Physical therapy department and the nurses. I would not recommend this place. It's essentially a Medicare dumping ground, but when every other place us full, its a tough choice. My father was a patient here. Way too many patients for the staff to handle. They always came up with an excuse of "well it's the weekend, and we're short staffed". Upon entry my father did not get any food or drink for 12 hours. He arrived after the dinner service, but got no fluids until the next morning. The rooms were dirty. Nurses were slow to answer call buttons. One nurse abused my father, yelled at him, threw the call button device at his chest. Overall unprofessional environment. Had to pull him out of there after 10 days to take his to a better care facility. read more

Dear Avocado Post Acute Care, I am writing was sincere gratitude and appreciation for the tremendous care that was given to my husband while staying at your facility. He recently had Covid and as you can imagine it was a very scary time for our family. We are so happy to say that he is recovered and is now home doing well. The staff there was very informative and responsive to all of our questions and concerns. We requested FaceTime visits each day and they ensured this happened consistently. This was a huge benefit for us. While all of the staff was kind and wonderful, a very special thank you goes to your administrator and assistant administrator Ronda Williams. They both provided their cell phone numbers to reach them during off-hours. This was another sign of their professionalism and care for my husband. While we hope to never face a situation like this again, we take comfort in knowing your facility is operated with the utmost care, concern and commitment to our loved one.
