    Let me start off by mentioning the stop all I can say. Is this place has helped me recuperate and most of the staff here are wonderful? I don't have enough work to express my gratitude towards them. There's a gentleman named yasser. I believe that's how it's spelled I'm not too sure. And a lady named This is a kind of service I mentioned that I wanted coffee will. Do you know he walked to 7011 and bought me a full cup of coffee? I couldn't believe it that's as generosity goes beyond anything. I could expect from a place like this so let me start up by just letting you know. If you have to be anywhere this is the place to go. Even to director Sean has been made it his priority to help me out in any way. You can not too many people will do that I really appreciate. These people from the bottom of my heart and the food is not bad. It's not the greatest but it's not bad thank you.

After…read moreover a year of making calls to try to move her to a closer facility for our family visits, Claudia was finally able to make it happen for us. She and I had been in touch weekly for over a year and she never lost her patience with me or got annoyed when I called the way other places did. Instead she was apologetic when there wasn't an available bed. The admissions staff was very caring, detailed and informative from the beginning till end. We stayed in close touch with Crystal through the care conferences, when my grandmother had to be sent to the hospital and back, along with when she had to go on hospice. Crystal is kind and considerate. The staff took care of her through her occupational and physical therapy sessions with Alicia and Melody. She was able to self feed and sit up right in a wheelchair again. They got her moving more than she did at her previous SNF. Her dieticians Zakirah and Yesica provided a great diet for her that she enjoyed until she couldn't anymore. I let them know she loves her sweets and they helped make it happen. Her last bites before she passed were pudding and Ensure fed to her by her nurse supervisor, Vivian, who took extra special care during the last days. They built a connection because Vivian spoke Vietnamese to her so naturally our grandma took a liking and was more responsive. Vivian even took the time to stay a little later so she could feed breakfast to her. Dan (charge nurse maybe?), was very detailed, gentle, and graceful in his responses when I called to ask questions about her status. And when he had to break the news, I felt the empathy in his voice over the phone. Her other RN's and CNA's (I'm sorry I don't remember everyone's names) including Adona, have all been a lovely presence in her final stages. My grandma always appeared very clean and fresh and lovely at every visit. The janitorial team is fantastic at what they do too, keeping the facility clean and spotless. Protective PPE is always organized and right outside the door for easy use. It doesn't smell like a SNF. Her last SNF didn't smell clean. Highly recommend this facility and their staff! Thank you for all you have done to take care of her with care, respect, and dignity till her last breath.

They are under LOTS of lawsuits & they violate lotsssss of labor laws. Do NOT send your loved ones here if you love them, this facility will not extend their time here with us.

I want to thank NuNu, Dillon, Isaac, Delfina, Grace, Eddie, Pam, for all of their assistance and understanding during my stay. The physical therapy department is excellent. They are all caring, understanding in all of the different ailments from all kinds of different patients. I want to thank Jacalyn, Jim, Claudia, Max and Ranon for all of the therapy and helpful ideas to continue my own therapy at home. The facility is very clean and doesn't have any kind of smell to it. I like the fact that they have ice water available all the time and staff will come around and ask if you would like ice refill and even a water refill. I would recommend this place for injury rehab and for your elderly loved ones.

    My mom had dementia, fractured her knee and had to be taken there because I couldn't care for her…read more24/7 and don't have the means to have a nurse at home. -My mom's clothes got "lost" day 1. I dropped them off, name on each piece, including a small blanket. I visit the next day and she's wearing someone else's clothes. Her blanket no where to be found. When I went to pick up her clothes after her passing, they tried to give me a bunch of stuff that wants hers. And I only got back one shirt and 1 pant. Out of 4 tops and 3 pants I had bought her. -The place stinks -Nurses were good while we visited, other than that they didn't help her. -She got mrsa, within a week of her being there. No one noticed it until they had to call 911 to take her to the hospital, almost a week later. -Lost like 15 pounds in those 2 weeks. She wasn't eating and they didn't tried to get her to eat. -I asked all the questions, they "tried" to communicate, asked me different things she liked to eat to get her to eat, or liked to do so she can have some distractions. But nothing went any further than that. -Half of the time I visited I found her soiled. I visited every other day, as I was taking turns with my brother. All of this because we don't have the means for the good places. If your loved one doesn't have a voice, avoid this place. Because they won't take care of them at all. It was the worst experience for all of us. *The people who can speak up for themselves, were ok. This was mid-January 2026

Gemma has been so supportive through the whole process. She has guided my family through the paperwork and process of the move in and a temporary move out for skilled nursing. Then, Gemma and Lynn helped us with the transfer paperwork and settling back in. Both Gemma and Lynn have patiently and promptly responded to our questions and concerns. Furthermore, the caregivers and medial staff on the 4th floor are kind, patient, and efficient in caring for their residents. My mother is now bed-bound but she is calm and content. Their care and guidance has allowed me to take a small step back. Knowing that she is well cared for allows me to breathe easier and allows me some time to myself. Thank you so much.

He has been living in the San Diego area for more than 50 years and although he has many friends, (who look in on him regularly), he has no relatives nearby. It is for that reason, that both myself and another sibling, we are dependent completely on the care and care givers at Grossmont Gardens. We could not be more pleased. Their communication, kindness and concern has given us a sense of relief. We are all very appreciative of the genuine warmth given to our brother.

Unfortunately we live in San Diego & Cottonwood is in El Cajon, so it was difficult for me to get there every day. The receptionists Arecelli & Ally made sure he received any food I ordered. Everyone is friendly & helpful. Alexandra was a great help making appointments & setting up transportation for my husband's doctors appointments. I definitely recommend Cottonwood. Thank you everyone. My husband just told me they called to check up on how he's doing. I miss this facility.

    Substandard healthcare with equally deplorable living conditions. Electrical sockets not properly…read morecovered, and probably not tamper resistant, despite the patient population being overwhelmingly elderly with severe conditions. There is a serious pest control issue (ants indoors and in patient rooms). There is no dignity neither privacy, and nursing staff takes at minimum 30 minutes to attend a patient's most petty and serious matters. I see elderly patients on wheelchairs just strewn about all over the lobby and at times obstructing the hallways, making you wonder whether they brought themselves out there, or if any of the healthcare workers simply left them there. I was here to visit my eldest brother, who recently is on the mend from a serious and rare necrotic infection. He was sent here from a UCSD hospital, who did not seem to offer him much of a choice at all. While he is lucky to be young enough to put up with a relatively shitty recovery and assistance, being soaked in urine for two hours and not given clean linens and clothing with "meal time" as an excuse for obvious negligence and healthcare abuse for someone who already has a hard time fending for himself is not grounds for recovery. The situation as it is is already hostile to neurodivergent patients, as my brother is one, too. The only time he got faster care was because we had an able-bodied person with us at the time. If this and any other healthcare facilities like this one were your choice, and you were already getting better healthcare where you are, fight for your right to stay, even if it may cost money later on, because this is not worth saving money over. The only positives that exist are the facility is air conditioned and the staff appears to be nice. The food is reported to be mediocre.

    Somerset Post Acute -- Staff Recognition…read more I've been at Somerset for quite some time, and I believe it's important not only to speak up when something needs improvement, but also to recognize employees who genuinely make a difference in residents' lives. Alma, CNA/RNA, deserves special recognition. One thing I've personally noticed is that throughout the hallways, one of the names you constantly hear is, "Alma, can you help me with this?" or "Alma, can you help me with that?" Somehow, Alma seems to be everywhere. Whether it's a resident, CNA, nurse, RNA, wound care, Activities, or someone else needing an extra hand, she's always willing to step in, usually with a smile and positive attitude. What also stands out about Alma is that she understands resident care isn't only about completing a task. Sometimes a resident needs encouragement, motivation, patience, or simply someone willing to listen. She provides that human connection, which can make a tremendous difference to someone living in a facility. Alma's work ethic, compassion, positive attitude, and willingness to help residents and coworkers deserve recognition. In my experience, she represents many of the qualities you hope to find in someone caring for residents. Rebecca handles scheduling along with multiple office responsibilities. Despite having a lot on her plate, she remains approachable, positive, and willing to help residents and staff, always with a caring attitude. Saron, Director of Human Resources, has also made a very positive impression on me. When I've struggled to get certain matters addressed, Saron has stepped in and helped make things happen quickly. She's caring, dependable, responsive, and an important part of the team working behind the scenes to support the facility and its staff. Krystal, CNA/RNA, genuinely cares about her residents, maintains a good attitude, and treats people with kindness and respect. Luisa, CNA, works hard, is willing to help, and makes an effort to provide her residents with good care. Narmin, CNA, is another staff member I appreciate. She's always trying to help wherever she can and maintains a positive attitude. Katrina, in wound care and infection control, is someone I've known for almost three years. From day one, she has treated me with kindness, professionalism, and respect. She has a warm personality, excellent manners, and a sweet way of interacting with residents that makes people feel comfortable and cared for. I also want to recognize our Activities Department. Mary is compassionate and attentive and works hard to make sure residents have activities and opportunities that meet their needs. She stays involved and makes an effort to keep residents engaged. Sarina, who works in Activities on weekends, is charismatic and encouraging. She motivates residents to come to the dining room, socialize, participate in activities, and spend time outside their rooms. Her positive energy is greatly appreciated. Finally, I want to welcome our new Administrator, Jose. My early impression is that he's thoughtful and capable and can bring fresh ideas and positive changes to Somerset Post Acute. I believe the facility can move in a positive direction under his leadership, together with our new DON, Terry, our DSD, and the rest of the leadership team. No healthcare facility is perfect, and this is difficult work. Residents see more than people sometimes realize. We notice employees who take an extra moment to listen, answer when someone needs help, encourage residents when they're struggling, and continue treating people with dignity. To Alma, Rebecca, Saron, Krystal, Luisa, Narmin, Katrina, Mary, Sarina, Jose, Terry, and the other employees who work hard every day: your efforts are noticed and appreciated.
