I have had to hand carry documents, hand deliver them, track down documents and request they process them to no avail. Every time I request an update, I get misinformation back. I finally went to my prospective employer and requested they step in as I cannot seem to get Peak Form to make any progress. Do not use the Boulder Colorado facility. The office staff is clueless.

I'm a local bus driver for Boulder and I have to have a DOT physical for my required CDL. I've had to go to Peak Form clinic in Boulder for as long as I've been driving. Every time I go for my DOT physical there's always some sort of problem, it could be lack of paperwork or medical information they need from my primary doctor. I had a DOT physical last October of 2021. I recently received a letter from the Department of Revenue saying my DOT certification could not be processed because the box on the for was not checked that I wear glasses. The doctor did not check the required box. The assistant who conducted the vision test told me to take my glasses off and tested me as well as testing me with my glasses on. So because I passed the vision test without my glasses they decided not check the required MEC box, I spoke to Dr Parsons of the Boulder Peak Form Clinic and I was told I had to take the DOT physical again to get a new Certification and I was told I would have to pay for the physical out of my own pocket. I highly recommend if your job requires a CDL and you have to take a Department Of Transportation physical DO NOT GO TO PEAK FORM CLINIC. No one there knows what they're doing !

I myself am a healthcare provider so I have taken care of many patients and I always say what terrible patients that we as providers make. I do know that the best decision I have ever made is choosing Concentra. Being new to this state and new to how workmans comp is handled has been difficult. I felt lost in a system that was stacked against me. I was truly blessed when they assigned me to Dr. Nystrom. From the moment he walked in the exam room I knew that he was knowledgeable, compassionate, caring and that his diagnostic abilities were impeccable. I knew my foot was broken from the fall that occurred at work and the whole process had been mishandled. The day I walked into Concentra was after I had left the Ortho. What a nightmare! The Ortho walked in the room and remember he had only seen me once. He was cheerful and began to talk about sending me to have Physical Therapy. This was without taking an x-ray or MRI. He had the nurse take the airboot off and put a little black lace up ankle support to replace the boot. I left there and went to Concentra. The bottom line is Dr. Nystrom is a God-send. This doctor knew from his evaluation and examination of my foot what the problem was. He sent me for stat MRI and confirmed that the foot was broken in 2 places and one tendon hanging on by a thread. Dr. Nystrom never rushes, he comes in and sits down and explains what he has found and answers any and all questions. He is a rare find in medicine today. I highly recommend you to seek any medical attention at Concentra and I can tell you that you will be greeted at the front desk with a huge and comforting smile. From check in to check out you are made to feel special and you will receive superior care. Thank you Lorie for getting the MRI scheduled ASAP and for making each patient feel special and like they are the most important patient of the day. I highly recommend Concentra to anyone needing good and thorough medical care.

    This facility is in better condition and the nurses are kinder then the Thorton , CO Concentra…read more Here are my issues with this Concentra. 1. I went in after work and was told I just had a sprain. I was shown how to use the crutches, which was incorrect. Had my crutches set up in correctly and was given a boot for a sprain. I complained about the pain and feeling unable to walk on my foot several times. The way they showed me to use crutches was to put partial pressure on my foot and walk that way. Because I complained about the pain they said they could get me in for a CT scan. 2. I went for the CT scan and 2 hours after the scan was called for an emergency Dr apt. I was told I had broken my foot. I was told to continue using the crutches and continue using the boot for a sprain. The way I had been taught to use crutches had me putting partial pressure on my foot to walk. 3. I was supposed to see the Orthopedist on Friday but because my employer failed to approve the claim they had to cancel that and the rest of my appointments. 4. I work things out with my employer over the weekend and then get my orthopedist apt on the following Tuesday, this is 6 days after my injury mind you. They did an xray to make sure the bone had not moved and it did move. I was told at that apt that I was wearing a generic boot mostly for a sprain and they immediately got me into a boot for a broken foot. The Boulder clinic is better than the Thorton clinic but I will never be going to a CONCENTRA AGAIN! Peak is way better, more friendly, cleaner and nicer facility and seem very interested is helping the patient to the best of their ability. I think if I had gone to Peak in the first place none of this would have happened.

Here many of my injury…read moreneeds are met (they also offer massage and PT, acupuncture is outsourced to another local office) and in a location that is convenient for me. The waiting room always has several fresh copies of the Daily Camera newspapers and there are also a variety of magazines to enjoy in both English and Spanish. The candies at the check in desk are also welcoming. Dr. Long is by far my favorite doctor to see for my injury needs. I feel that she not only really listens to my concerns but also genuinely cares about my progress and overall health. It is truly great to feel that you have someone fair and compassionate in your corner. Caryn the physical therapist has done a great job coming up with a home program for me and helping me understand the progress I have seen thus far. Kyle the massage therapist has also been terrific. He always takes the time to hear how my injury has felt in the past week and often tells great stories of Boulder wildlife encounters when working on some of the tough muscle knots. The location also includes a room of gym equipment and I am happy that they let me perform some of my foam roller stretches/exercises following my visits with their practitioners. Something I would love to see is a bike rack. Currently the closest ones are located at King Soopers and down by Ripple. I definitely recommend this location for your injury needs.
