On the surface, the facility presented well with a clean appearance, beautiful rehab gym, pool, carpeted hallways, chandeliers, and a private room option. There was, however, a slight odor in the air during the tour that we tried not to focus on. We ultimately chose the facility because we were told my mother would receive three hours of therapy a day. Unfortunately, after getting her settled in, things quickly became concerning. To be fair, some members of the daytime staff were wonderful. FIVE STARS for nurse Shannon, occupational therapist Mary, physical therapist johnathan, the social worker, and several daytime aides (Jeanette D, AnnJohn, and Maxine) were kind, attentive, and professional. I truly appreciate them. The nighttime environment, however, was a completely different experience. Patients could frequently be heard screaming for help. Outside of the staff members I mentioned, many employees did not appear engaged or fully competent. Leadership was also not transparent, and when concerns were raised directly, communication became evasive instead of solution oriented. Our family was present from approximately 8:00 a.m. to 8:00 p.m. daily to make sure my mother's needs were being met, which turned out to be necessary. At one point, my mother had an outside medical appointment and required paperwork from the facility. We were given documents belonging to an entirely different patient, including that patient's photograph. The explanation felt careless and deeply concerning, especially given the seriousness of patient privacy and identification. There were also several instances where my mother was nearly given the wrong medication, and long periods where she waited extensively for assistance. The odor in the facility became significantly worse at times, particularly later in the day. Additionally, the therapy schedule did not match what was promised. Instead of receiving three hours of therapy daily, my mother was receiving roughly 40 minutes combined between services. This experience taught our family that appearances can be misleading. While some staff members genuinely cared and worked hard, there were too many operational, communication, and patient care concerns for us to feel comfortable recommending this facility without caution. I'm grateful we made it through the experience and thankful for the few staff members who truly supported my mother during her stay.

During a recent visit, a nurse informed me that she waits for patients to notify her if something is wrong. When I asked whether she routinely assesses her patients during rounds, she did not respond. This exchange was troubling and inconsistent with the level of proactive care that should be expected in a healthcare setting. Additionally, my mother's legs have been noticeably swollen for several days, yet no staff member took appropriate action or notified me of her condition. This represents a clear lapse in monitoring and follow-up care. Families entrust their loved ones to this facility with the expectation of compassionate, professional, and proactive care. Unfortunately, my experience thus far has fallen far short of those expectations. I will be contacting the facility director and corporate office to ensure these concerns are addressed and corrective action is taken. The evening and weekend staff are nothing short of a liability!!

This was from years ago to now, years ago I visited this place for a loved one and decided 100 no way I could bring my loved one here or anyone not even someone I did not know but needed a place. The rooms some of them are 4 to a room that's a bit much for sick ones, they need space and comfortability not to be crowed to bring in money. Unfortunately, not under my care another family member was there, went to visit and the same crowded situation with the 4 in one room in some situations which was my loved one situation. You can tell nursing staff did not wash this family member hands and when asked for my other family member with me to clean them, I asked for hand wipes nurse staff they said we do not have wipes. Really in a rehab, nursing facility what? When speaking with 4 nursing staff all 4 of them were very and I extend very nonchalant, face expression less, did not ask who are you with a smile, who are you here to see, who's your loved one absolutely heart breaking. No kindness, communication, no joy or happiness in the atmosphere just blank no warmness. When asked a few questions like about dinner time, what's the bedtime protocol, and other protocol facility rehab interest it was as if communication had a Baria in language, facial, no eye contact. It was the worst I've ever seen literally and physically not that cleanliness it should have. Definitely no fresh aroma as most do not have sadly so and why? I can never understand this about rehabs, nursing facilities why? Two of the other residents whether temp or perm I do not know ask my assistance to help them with something I had to tell/get a nurse to assist this was not comfortable for me. For the elderly women or myself again heart breaking. How could this be, but it was, it is. I hope this place is shut down sometime soon and very soon and these individual person's all of them will have somewhere caring kind, safe, and secure to be taken and cared for. "They are human beings- for God's sake and need help.

Oh boy I hate this but somebody gotta tell the truth around here first of all you can tell the difference between the clean nice facilities for those w/ a certain level of income versus this facility... I'm sorry but I'm an officer and been to the worst places in the city but I have a family member her at this "Whitehouse" and I'm trying to find the quickest way to get her out of here because she's only going to get worse. The moment we walked in the first this you smell is feces and piss just because there are people with ailments or certain conditions there is no reason why this place smells like this you can clean with WATER ONLY ! get some fabuloso or Mr. Clean or some of the other medically sufficient cleaning agents, this is the worst. It it wasn't just walking in while visiting my family member I walked down the hall and the floors were dirty and the sinks the bathroom in her room reeked of piss and my family member is bed bound so WHY, WHY does it smell like this in here !! So now I'm forced to go to the bathroom in the common area and.... Wait for it ‍ (see photos) the garbage can look like it hasn't been emptied in days take a look at the floor and this bathroom didn't smell any different from the bathroom in my family member room... This facility is predominantly African American people both the staff and the patients... and once again I'm not trying to make this about race but the facility is now owned by Jews and not one of them were doctors... They were walking in there like they was just there to see how much money was collected and from where for the day ( b/c you know that is all that they do actually care about) #1. They don't give a "F" about none of us I don't care what color you are if you ain't one of them #2. If you want your family member to stick around a little while longer DO NOT bring them here . And lastly on my way out of there, there was club music playing loud down one of the hallways like it was a party jumping off in this spot. There are elderly and sick people here and these people are blasting dance music in the hallways ‍ like why... This is a nursing home/ rehabilitation/ hospice care facility TF is going on here. This place made me mad as hell !

I contracted with Alvita to care for my mother during the last two months of her life after she was injured in a fall. I paid for 24 hour home care seven days a week. Other than Fabienne, I found the caregivers sorely lacking in the most basic of qualities such as companionship and emotional support. Also, other than Fabienne, the one on one care for which they were hired (toileting, hygiene assistance, dressing, grooming, feeding, etc) was mostly delegated by them to the staff at the assisted living facility where my mother resided, which negated the whole point of hiring them in the first place. Chronic lateness was also the norm. Most arrived 10-20 minutes late for the start of the night shift The night shift crew was especially problematic. In addition to generally not performing the basic tasks for which they were hired ("it's not my job" was the common refrain, even though it literally was their job), they often did not even stay in the same room as my mother who was a fall risk. Some slept on the sofa in the living room during the night shift, some spent the night shift doing schoolwork in a separate room, and some spent hours of the night shift on their phone in the hallway outside of the apartment. The complaints became so numerous from the staff at the assisted living facility and from the hospice nurses that I eventually had to change agencies for the night shift. Three of the low points during the Alvita night shift require special mention. (1) There was the night that my mother sat in her own feces for the entirety of the night. When Hospice arrived in the morning, the waste was dry and hard and had been there all night, despite a 12 hour Alvita shift. (2) Many nights, my mother was put to bed at 7 PM for convenience sake without any social interaction or kindness on the part of the aide. (3) My mother suffered a skin tear on her arm the night after I complained about the aides sleeping on the job. I was told that she injured herself, but there are no witnesses other than the Alvita aide involved. On the opposite end of the spectrum is Fabienne. She was the primary Alvita daytime aide for my mother. Fabienne went above and beyond the call of duty. She exuded kindness and compassion. She took the utmost care of my mother and treated her with dignity and respect. When Fabienne was on the job, the staff at the assisted living facility and the hospice staff took a break because everyone knew that she had everything under control. This is the level of care advertised by Alvita and the level of care that I paid for, but only Fabienne met the mark. Not only did she meet expectations, but she blew everyone away with her level of care.

From the liaison to our home health aide, the care was outstanding. The communication between my family and Alvita was top notch and we knew everything happening every step of the way. The staff at Alvita is what truly makes this company so special. I highly recommend!
