This skilled nursing facility is the best of its kind! The nurses and care staff are all so helpful and intelligent, it's easy to see that the residents are happy and well taken care of. Aides respond quickly to call lights and therapists go above and beyond in rehabilitation services. Food is awesome. Housecleaning services stay on top of their duties. Words cannot express my disappointment with this facility. During my mother's stay, I witnessed both in person and over the phone the lack of response to her needs. I was lied to about whether or not she ate her meals, she called in desperation in the middle of the night as she could not reach her call button, and before staff recognized I was on speaker phone with her, they apologized for their lack of response and that she could not reach her call button. Numerous other complaints were filed by me through the Ombudsmans office, and the State of Oregon did not begin an investigation until six months later. All complaints were reported to be unsubstantiated, with every claim listed as the facility provided adequate care and services. Had we not had family consistently dropping in, I shudder to think what would have happened. Numerous calls were made because they were not responding to her call button, and still she was ignored. It appears Bend Transition Center and the State of Oregon want to just whitewash over this. Because I continue to have my hands full caring for my mother, who is now in an excellent facility, I will not be bothering to continue fighting this. However, please, please be attentive if your loved ones stay at BTC. read more

Bend Transitional is an excellent short term Care Facility that takes very good care of its patients after leaving the hospital. The rehabilitation staff really know their jobs & helped me gain great strides in my muscle retraining & grip strength. Our nurse case manager Megan was very kind & the Administrator Heather listened to my questions & answered them all to my satisfaction and Michelle the head of nursing & Shelly the dietitian made sure I had the care I needed in a timely manner and my meals were geared specifically to make sure that I received types of foods that would best help in my recovery while also taking into account my preferences. It was kept quite enough at night that I was able to get some much needed sleep in, & I wouldn't hesitate to recommend Bend Transitional Care to a friend or family member. Two thumbs up in my opinion!! read more

My Mom spent over 30 days at BTC. First let me say that you need one person to advocate for the patient and taking notes. It's a little tough navigating the system if you have not done it before (I had not and Covid restrictions were in place). Take down the number for the Nurse-in-Charge (NIC) for the wing your loved one is located (on the front glass door). Do not be afraid to call the NIC and also give the patient that number in case they have put the call light on and have not received a response for 60 mins. It is okay to call. The pluses: CNAs, PT, OT, provided decent care. The nursing staff was off and on. I do realize they are understaffed and BTC have hired visiting nurses -- so there's that. The front desk was helpful and nice, when they were able to answer the phone or be at the front desk. Transportation to appointments and from St. Charles was excellent. What you need to know: After the call button is turned on the CNA comes in and then turns off the call button. The CNA will likely say "I'll be right back." It may take 10 minutes or 60 minutes for them to return so keep that in mind. If the patient asks for pain meds it may take 60 minutes since the CNA has to get the Nurse. Pain meds will be given at request (not as written on the patients prescription). Don't expect to leave messages and get call backs. Just keep calling until you get a live person. Care team meetings will happen once and include: OT/PT, NIC, Regional Care Manager (RCM ext 1006) and Discharge Coordinator (ext. 1002); this team will give you a feel for the patient care plan. Good luck getting care records. I asked three times for a report on my Mom's weight. I had to keep calling and calling to get the information. Minuses: Dirty patient toilets, clogged patient sinks, BM left in shower for 2 days because there is no maintenance on the weekends, dirty floor in room -- medical supply packaging strewn around and under the bed, CPAP breathing equipment laid directly on the floor (I have a picture), patient bruised when CNA took control of electric wheel chair and caused a 10" bruise when they ran my Mom into a pole by the scale (I have a picture). Disturbing: When my Mom was transferred from her wheel chair with portable oxygen to her bed with an oxygen concentrator my Mom texted me that she was not feeling well when she got back into bed. Well......the CNA forgot to put the oxygen concentrator "ON" so my Mom was without oxygen for a while. Sorry don't know how to attach pics from my phone. read more

Bad food not worth eating my dad lost weight here he said much longer he would of died!

Husband was transferred there from St. Charles, after approx 2 months in hospital. He was put in a room at the end of a hall that had no door to it, only a ripped curtain. Headboard not attached to the wall, no chair to sit in, very thin mattress, hardly any storage to put belongs into and basically no privacy. Food was pretty bad too. It was worse than a cheap motel room. Husband was often ignored when a request was made. My husband passed away there and I've been so hurt and angry. Later when I went pick up his belongings, I noticed an item missing. Social services woman, Tera, went and found what I was looking for. When I got home and unpacked everything, I found two items missing, a shirt and a book reading aid. I called on 11/30 to notify BTC of the missing items, and as of today, 12/17, I have not gotten a response to my issue. I later found another shirt missing and advised receptionist, who said she would give the info To Tera in social services. I have left a voicemail for the administrator and social services and have had no response. Would you want a loved one to be treated there? I hope not. In the meantime, I am still waiting for help. read more

After a nasty bacterial meningitis infection my Mother spent several weeks at BTC during June and July 2016. I visited many times at all hours of the day and I am shocked to see the negative reviews posted here. All of the staff were extremely nice to me and my Mom. I showed her this website and these vile comments posted by others and this is her response: I strongly disagree with the negative reviews. No doubt it is a question of expectations. I certainly did not expect the bed to be like my bed at home. However, I found it a welcome change from the hospital bed I had spent the previous 10 nights in. I was a resident of Bend Transitional Care for 6 weeks and I slept very well on the bed provided. At first I had to have assistance to get to the bathroom. Certainly I did not expect an immediate response to my call light. But I never had to wait beyond my body's demands. I found all of the staff to be friendly, kind and considerate of my needs. A number of them I will miss especially but all of them were professional and wonderful. I am in awe of their choice to care for people and to make such a difference in their lives. The on-staff Nurse Practitioner deserves special mention, withholding her name. She was especially attentive to my needs for 6 weeks of intravenous antibiotics and the strain that puts on the body. She conferred with an antibiotics specialist and pressed on to get our referral from OHSU in Portland to an Ear, nose & throat specialist here in Bend. I found the food only slightly different from that offered at OHSU. There is a menu selection for each meal, a variety of selections, plenty of fresh vegetables and fruits. I found every meal enjoyable. My medications were given regularly. At the beginning there was a slight delay in obtaining one medication. Otherwise, no irregularity. Elder abuse? Gasp! These dedicated medical people made Bend Transitional Care my home away from home. I never thought I could stay in a facility for 6 weeks. But once there I was thankful there was an opening for me. I had no desire to leave until I realized I had only one week left of antibiotics. Then I wanted to go home! Their therapy program is outstanding. Almost every day I was scheduled for 2-3 sessions of therapy. There is a large gym with helpful machines. The therapists are friendly, considerate and knowledgeable. They were valuable help in getting me back on my feet, walking some distance and gaining some confidence in returning to normal. read more

I have been blessed. I fell again had surgery twice during the busy Holiday Season. Before discharge the Hospital got with Bend Transitional Care they took me in and really are taking good care of me getting me back to where I can walk and build strength and on the right track again with Physical Therapy and Occupational Therapy exercises. Mother in law came here for recovery after release from hospital to recover from fall that broke her pelvis and tailbone... step sister left for 2 hours, returning to find her 82 year old mother covered with dried feces. Obviously unattended and not looked in for some time... staff was unaware and mostly uninterested. If you have a loved one in here, buyer beware. Checking her out and taking her home! read more

If I could give this facility zero stars, I would. My 75 year old father was sent to Bend Transitional for a few weeks of antibiotics in hopes that his foot would heal where he had some gangrene. Upon his arrival, I spoke to the nursing staff and made it clear that he needed to be moved closer to the nurse's station (he was put in the back corner of the facility) and that he needed fall alarms on him at all times. Because of my father's issue with urinary urgency, and his foot being injured he was compulsive and unsteady and would get up on his own to toilet. He had almost fallen at the hospital, so I knew that this was going to be an issue. After assuring me that they would make sure that alarms were on him, I showed up the next day to find a huge bruise on his forehead where he had fallen off the toilet. My dad said that he laid on the floor for probably 15 minutes before anybody found him. When I called the RN on duty to his room, she was "shocked" and "had no idea he had fallen." She talked to my father in his face as if he was a 5 year old, asking him about the fall. I asked her if she had even assessed him, and she said no. This was 2 hours into her shift and she hadn't even been in his room. The CNA on shift that morning told me that she was told in her report from the PM shift CNA that he had fallen. But the RN said no one told her. To add to this, no one called me to report it. After assuring me that it would not happen again, they were going to move him to another room. When I arrived that evening, they had moved him closer to the nurses station, but had given him a twin size bed. Mind you, my father is 6'3", 300 lbs. I asked them for a bigger bed and was told they would. The next morning, day 3, I show up to find out from the CNA that my dad had fallen out of the twin bed that they assured me would be changed the day prior. Again, no call from them to report the fall to me. After getting angry with them, they found a queen. That evening he fell again. He had no alarms on him and was able to walk to the bathroom where he fell between the toilet and the wall and laid there for 30 minutes until he realized there was a pull cord to alert the staff. This fall, again was not reported to me until I showed up to the facility the next day. And according to the MD, she didn't have 3/4 falls reported to her. All falls must be reported to the MD. PERIOD. Now let's talk about the MD. My dad has horrible peripheral vascular disease and we knew that there was a chance he would lose his toe, foot at most. I won't get into too many details, but on day 19 of his stay there, she really pushed towards palliative care seeing my father (taking advantage of my trust in their judgment). That he was "inoperable" and the gangrene was going to make him septic and he would die despite treatment. Based on the records from St Charles Medical Center and the vascular surgeons who were on his case, there was nothing they could do. Come to find out later on after transferring him to the Portland VA, that actually there was a lot that could be done. They were lazy and didn't give a crap. Within 2 hours of being at the VA facility, a surgeon saw him and said that he could revascularize his leg and likely save his foot. Not only was my dad able to survive a 5-hr surgery that saved his foot and his leg, it saved his life. Had I trusted the vascular surgeons in Bend and the doctor at Bend Transitional, I would have put my dad to death. But really what adds to my disdain about this place, is the VA facility social worker contacted skilled nursing facilities in Bend to get him some rehab after his surgery and Bend Transitional said that they wouldn't accept my dad because he was "non-compliant." No. That is not the case. He was no different from any other patient they could have. They were absolutely negligent in their care of my father. They simply did not want to deal with him. I would have refused to use them anyways, but to hear this, is what prompted my review. If you care about your loved one, heed my warning. Also, pay attention to the other reviews of this facility. My husband had a kidney removed and was sent here. The staff were friendly but the care wasn't great. Didn't answer the call light in a timely manned. The call light fell on the floor (not attached to the bed) so my husband got out of bed crawling on the floor to find it and fell. They called me at 1:00 am to tell me they had called 911 (NON emergency line). He falls all the times. If he asked for a Tylenol it would take up to an hour to get it. He asked for a sleeping pill at 9:00 and then he fell asleep and they woke him up at 11:00 to give him the pill. If you received good care there then that is good. Our experience was not great. Breaking him out tomorrow no matter what they say! read more

My aunt has been a patient several times at this facility. Where the patient care nurses seem caring, the administration is quite lacking in professionalism and do not grasp the importance of family/patient interaction. Not living in the same town as my aunt I was constantly at the mercy, of people being away from their desks and not answering the phone, nor getting back to messages I left. Very inadequate communication. Administration actually told me, "well we are not at our desks, nor are the staff every minute of the day". Of course you aren't, but returning phone calls or making certain a patient can reach their phone to answer their own calls should be a pretty simple task. read more

Very unprofessional! My Uncle came here from the hospital after a stroke and it was terrible. The people didn't have time to return calls but they sure were there to sign him up for care. When we visited he was always laying around in bed and sleeping. He was fully ambulatory when he went in and he actually got worse by the time he left! They put him in a wheelchair because they were too lazy to walk him. He lost 19 lbs while there. They couldn't even show up for a Family Care Plan. Don't go there unless its a last resort and then family had better visit every day. read more

Worst care ever definitely elder abuse you would be better off letting a transient care for your loved one in a dirt lot then in this place

Grandma went there after having Surgery. While she was there they isolated her in her room brought her food to her in her room, I told them repeatedly that she has dementia and need to encouragement when eating and at the very least needed to be in the dinning room with other people eating and each time I would go see her she was in bed and I was told they bring her food in and she just wont eat to which I said is anyone encouraging her to eat or is she being fed in the dinning room to which I was told by the CNA every time that they didn't know she needed to be encouraged and that needs to be in her file. She lost 13lbs while there and was left in bed so much she ended up with a bed sore. If she would have gotten a bed sore while we have been caring for her and it got reported we would be investigated for elder abuse. Then to top it off she was being charged almost $1200. a week after medicare for the unacceptable care. When they were questioned about the bill and told she only makes $870. a month on S.S and where there any programs they had that could help I was told to put her on medicade and they released her. It has been a struggle to get her to eat being as she went so long there with out eating but we are getting her to eat now. And don't get me started on the lost hearing aide last year when she was there and because the intake person didn't put on her paperwork I was out of luck. My father is a patient there currently. I live in another town but have been visiting there a few weeks in a row. What I have experienced and heard from family members is very frustrating and concerning to me. One night I was visiting him and he let his CNA know that he needed to go to the bathroom. He was told they would be right there... over a 1/2 hour later and on my way out I stopped by the desk and let them know that my father still needed to go to the bathroom. With no eye contact, I was rudely told by his CNA that he would be right there. In retrospect I think I should have stayed and made sure they did it right then and there but I shouldn't have to do this!!! He had been approved by a dietitian to start eating a regular diet and yet for the next 3 days they continued to bring him the softer foods for his old diet. The most recent incident I heard about was that he was in a lot of back pain and calling for a nurse; it took over 3 hours for them to respond. 3 hours!!! When I was 1st visiting I was a little lost and confused about where to find his room and with several staff around not one person made eye contact or offered to help. I work in health care and these situations are uncalled for. When I have been there it doesn't look like a staffing problem but I understand that being and health care myself but that is never an excuse to be rude are not provide excellent health care. I don't think there are any excuses for making an elderly person wait for over a 1/2 an hour to help them go to the bathroom or to respond to a call button when they are in severe pain. I am not in control of his health care but if I was the one calling shots I would be finding a new place if possible. I sincerely hope that this review is taken seriously by the management of this facility and is helpful to others if you are considering this center. I hope it gets better, I really do. This is very agonizing and frustrating and I feel helpless. Bring your own mattress,bring your own ice compress for pain, learn to hold back urges to urinate for 45 minutes, learn to identify each one of your prescriptions before you take them. I have a long list of legitimate grievances. By appearances it is wonderful clean and modern but in getting into actual services, beware. Most importantly the distribution of prescription drugs is a nightmare. After 2 weeks it did not improve after numerous complaints. The beds are broken down foam and a detriment to your spine. The food is mostly processed with a lot of sugar. Very little fresh/whole. There are a few wonderful staff members and many incompetent uncaring ones. The administrative staff is primarily concerned with keeping costs down and avoiding liability at any cost. If you have to be there, make sure that you have family to oversee your medications and concerns and can follow through for you. I have had staff share common concerns and scary stories. I am being penalized for insisting on early discharge at this point but feeling that I will be way ahead of the game as a result. read more
