This place looks so good - how could say no. Well, just say…read moreno. The Rehab is awful, they stand you up and that is about it. They say you refuse do to Rehab. NOT! They never clean the rooms. Dirty . I cleaned the room everyday when visiting my husband. They wrapped his leg so tight it bugled at the top it looked awful. Then they lied and said I grabbed the nurse when she came in. I touched her wrist. THEY DON'T CARE - I Moved him OUT. Wasted 3 weeks there had to start Rehab over. Oh plus he got a bed sore because they just let them lie in bed. We placed my mother, who has dementia, in traditional assisted living in April, 2020, during Covid. Before welcoming her with open arms, they had her room all set up with her personal belongings so she would feel at home when she arrived. Our deep gratitude goes to Nikki Vawter for this. She is an absolute Angel! The nursing and caregiver staff are absolutely amazing, warm and giving! During Covid she was kept safe and continually tested. Also during Covid, Nikki was able to FaceTime with mom's children as we were not able to visit at the time. Mom is so happy and content. She is a life long faithful Catholic and a social butterfly! She is able to go to the beautiful church any time she wants, as well as socialize with the other residents. And she loves the numerous activities! We very highly recommend this facility for any senior needs, from independent living to nursing home care!

In striking up conversations with them, I heard plenty of unsolicited positives--lots of on-site activities and off-campus trips for residents, a beautiful wellness center with a pool, restaurant-quality food, well-maintained grounds and flowers, good employee benefits and great co-workers, and very helpful staff that put residents' care first. If I were looking for independent or assisted living, I'd sell my home move there in a heartbeat! Kudos to St. Leonard's entire team for making everyone feel so welcomed.

September 4, 2026, Friday 0915 AM I left in tears! These…read morehealth care providers there do not care about human beings, especially on an upcoming Holiday, Labor Day weekend! Traumatic and negative experience, crying for several hours!!! I never got Help here!! I felt discriminated, for being Bi-Racial and for being an older female patient! Shame on them!! I felt dis-respected because they did not want to work today, being a Labor Day Weekend holiday, coming up!! I thought the Labor Day holiday began after midnight on Friday! Labor Day Weekend coming up and no one has the moral kindness and professional integrity, to help me, at all!! I have been there before many times, and no issues, except today, with Ms. Missy and with Ms. La Tara, an LPN, Office Manager there! Missy and La Tara gave me the most negative and most dis-respectful experience, as I am a Medicare/Health Springs, Senior Citizen! I am a human being!! I never met them before!! Ms. La Tara, office manager, gave me a 10 dollar gas card,for reimbursement for me for my gasoline, used for this traumatic trip to them today! I wanted to be seen by a Doctor today and not be traumatized and dis-respected as a Senior Citizen! I went on Thursday, September 3, 2026, in person, to the Reception Desk, to get an appointment, for a Dog Bite/Attack, (Dog Bite happened in April 2026, I was experiencing new pain in that area, of my Hamstrings, back of left leg, where the German Shepherd, loosed, a next door neighbors dog, neighbors dog bit me, and for the last 3 days losing sleep, and having pain, day and night in that area of the past big violent neighbors dog bite/attack! The Receptionist explained to me that my usual kind Provider was not available, and would not be back till September 14, 2026, So, the desk receptionist (The Receptionist Trainer and experienced Receptionist) told the newer Receptionist, find her another Provider, and they gave me, an appointment for Friday morning, at 0915 AM, September 4, 2026, with a new provider: a Dr. Soto. Missy, the intake health care worker was rude and aloof and said that she would need to go get the office manager: Ms. La Tara, involved for this appointment! Why? I felt that this being Labor Day weekend, every one there was only concentrating on their long weekend of fun! I have always been professional and kind and polite always! Ms. Missy and Ms. La Tara decided on their own that they would not see me today, WHY? They were rude and not professional to me! I told them that we drove from Xenia, Ohio, in heavy traffic, for this early morning appointment, with Dr. Soto! I never saw Dr. Soto, and I never got Medical attention!! Ms. Missy and Ms. La Tara LPN, said I go do Physical therapy or go to the Emergency Room!! We cannot afford the Emergency room cost and the distance and the gas and very long waits for going to the emergency room! Also, I had this appointment with them today, Sept. 4, 2026 ! I have been going there for a while and felt more comfortable there, until today!! How did these two people: Ms. Missy and Ms. La Tara LPN, have such power and authority to decide that I would not receive medical help today!~I am in excellent health otherwise, just wanted a Doctor, to help me with my old dog bite (Back in April), with the recent shooting pain in that area of my leg! This place is TERRIBLE! I don't even know where to begin. They are not a 5 star please do not…read morebelieve that. My grandfather was there for 6 months and it was the most horrific experience My family has ever been through. They are EXTREMELY short staffed. They had 2 aides to 30+ plus residents many times, they use agency staff which are always different aides and nurses , and they have no clue what's going on. We had to BEG for my grandpa to get showered. I called the D.O.N on a weekly basis to ask when he would get his shower and nothing ever changed. My mother started giving him his bathes along with all his personal care. My family was with him 12 hours a day and he still did not receive proper care . He would be left in soiled diapers, we would put on call light and would wait 20+ min and finally just did it ourselves. This place is terrible. My grandpa fell one night they never called us we arrived at 6 am and he was sitting in the hall naked bleeding profusely like it was no big deal. He needed stitches and had pneumonia when he arrived at hospital . Residents are neglected here. If you plan on putting your loved one here and you can't monitor their care 24/7 then don't expect a long term stay. There was one nurse Angie 2 aides one very short hair and the other black hair with tattoos that my papa loved and one dietary aide we really loved so much, the rest of the staff is horrible. My grandfather past away in the middle of the night a couple weeks ago which has been traumatizing for our family. We saw him at 7 pm and at 3 am they called and said he passed away. Only explanation we got was he had been throwing up, again no phone call to report the illness. The only thing we have come to terms of is that they must of had him laying flat on his back and he aspirated when vomiting. Since he passed we did not receive one phone call from anyone there not the administrator, D.O.N, no one to say sorry for your loss or check on us, goes to show these people are there for money. They could care less about the residents .. there were so many errors. We caught a nurse red handed giving him whole pills when there was an order for crushed only, she admitted to have been doing it for days and he had pneumonia that week he had the fall which could have easily been from aspirating the pills .. Same nurse was back in his room next day with an attitude . My mother then would not allow agency in his room at all due to all the errors. Please just do you research . Yes the building is clean and remodeled but everything else is terrible. I know things can sometimes be unbearable or hard to keep sick loved ones at home, trust me This was the hardest decision our family ever had to make and my mother stayed with him 8 hours every single day he was there, but please research thoroughly before making your choice if you have to go this route and check on your loved ones

The staff were friendly knowledgeable and efficient. They took very good care of her. I can't say enough good things about this hospital. I hope we don't come back but if we need medical care, this is where we will go.
