It happens so often, we usually check our order before leaving the parking lot so we can take our order back to the restaurant and have them correct it when there's a mistake. If we're in a hurry and we leave without checking our order, it's wrong. Every time. Their food is good, their staff is friendly, but their take-out order quality control needs improvement. We get a lot of take out, and the only restaurant that has a problem getting our orders right is Panera (this includes Panera Restaurant on Campbell and also the Panera on Battlefield).
