I am writing to share my experience as a patient who underwent total knee replacement surgery and…read moresubsequently spent more than five months in postoperative physical therapy. From the very beginning of my recovery, I consistently reported instability, pain, persistent swelling, a clunking sensation, and an overall feeling that something mechanically was not right with my knee. While I understood that recovery from knee replacement can be challenging, my symptoms never felt consistent with what I was being told to expect. I repeatedly expressed these concerns throughout my course of therapy. In addition, I was often told that my progress was well behind what is typically expected following a total knee replacement. Despite my repeated reports of instability, pain, persistent swelling, clunking, and the feeling that something mechanically was not right, no additional hands-on assessment or targeted testing of my knee was performed to determine whether there might be an underlying mechanical issue. Instead, treatment largely consisted of visual observation, massage therapy to address swelling and discomfort, and continuation of many of the same exercises month after month. As my symptoms persisted and my progress remained limited, I became increasingly concerned that the underlying cause of my difficulties was not being adequately investigated. As the months passed, I became increasingly concerned that my recovery was not following a normal course. I ultimately decided to seek a second opinion from another local physical therapist. Within minutes of reviewing my history and performing a brief assessment, he recognized excessive laxity and spacing in my knee and determined that my symptoms warranted referral to a revision specialist. That referral led me to Dr. Nazarian at Rothman Orthopaedics. Following his evaluation, it was determined that revision surgery was necessary. One question people may ask is, "Why didn't you just go back to your surgeon sooner?" The reality is that after surgery, patients are encouraged to place tremendous trust in their physical therapist. Once the operation is complete, much of the day-to-day management of recovery is effectively handed off to physical therapy. Physical therapists are the healthcare professionals who see patients multiple times per week, monitor progress, evaluate setbacks, and help determine whether recovery is progressing as expected. As a patient, you are expected to trust that your physical therapist is appropriately evaluating your recovery and advising you when something falls outside the norm. Because of that, I trusted the process. I trusted that the instability, pain, swelling, clunking, and persistent feeling that something was mechanically wrong with my knee were being properly evaluated. Looking back now, I wish I had sought another opinion much sooner. What frustrates me most is that I spent over five months attending therapy, using the majority of the visits covered by my insurance, while continuing to report the same symptoms from the very beginning. I cannot help but wonder whether a more thorough evaluation or earlier recognition that my recovery was not normal could have led to answers much sooner and potentially saved months of frustration, uncertainty, discomfort, and the need to spend so much time addressing a problem that ultimately required consultation with a revision specialist. The purpose of this feedback is not simply to share my experience but to encourage patients to advocate for themselves. Do your research. Ask questions. Listen to your instincts. If something does not feel right, do not be afraid to seek another opinion. Not all physical therapists bring the same level of experience, critical thinking, or ability to recognize when a patient is not following a typical recovery path. At the end of the day, no one has a greater stake in your recovery than you do. Based on my experience, I highly recommend Steve Pete at JOYNT in Lewes. He listened carefully to my concerns, performed a thoughtful assessment, recognized almost immediately that my symptoms were not consistent with a routine recovery, and helped direct me toward the specialist who ultimately identified the need for revision surgery. Choosing the right physical therapist can make an enormous difference. A highly skilled clinician may help patients obtain answers sooner and potentially save significant time, money, heartache, and unnecessary pain.

DO NOT GO HERE! Very dangerous. I went to see Dr Harriott because of a very slight tear in my…read moremeniscus, upon looking at the MRI, he wasn't even sure it was a tear. I was concerned because I have a heart condition, so I must be able to continue with aggressive cardio exercise. I very clearly explained this to him, so that he understood that my goal was to preserve my very good and structurally sound knees, in order to stay healthy long term. He strongly recommended hyaluronic knee injections. He sold me on them by saying it was a completely natural substance found in rooster crowns, and there were no potential side effects, since it was the exact substance in the knee, etc. He indicated there were imitation and genetically engineered products available, but he only used that actual natural substance from roosters, so I allowed it. (He later confessed that this was not true and that he actually used the genetically engineered product) TRUSTING HIM WAS ONE OF THE BIGGEST MISTAKES OF MY LIFE. I should have known better. After the first injection, I began having significant knee pain, in and around the injection site, migrating up and down from the spot of the injection, even though I was very cautious with using the knee and following all instructions as far as icing, etc. I was very concerned and went over with him at great length prior to the second injection, yet he insisted that the injections were completely safe with no potential side effects, and I should just ice it more afterwards. I reluctantly received the second injection. My knee deteriorated significantly afterwards to a point where I could do no cardio exercise anymore. Prior to the 3rd injection, I called and explained the situation, and said I didn't think I should get the 3rd one. They explained that in order for it to work, I must get the 3rd injection and insisted that I come in for the third one - before 30 days or the treatment will not work properly. I waited an extra week due to the pain I was in, and reluctantly went in. I tried again to explain to the doctor about the deterioration of my knee...He said its not possible to have any type of reaction or negative effect from the injection, and that I should ice it more. He insisted on the third injection, so I relented, hoping that it was true and that I needed to complete the treatment in order to see a benefit. Big mistake. I could not even walk after the third injection. I said to him that I can't put any weight on the leg as he held the door and waited for me to leave. I will never forget the sadistic and unempathetic look on his face while he held the door open for me, as I struggled to put any weight on the leg. He then left me in the room to limp out, get the door myself and struggle out of the office to my car. I had to drive home with my left leg on the gas and brake pedal. i WANT TO EMPHASIZE THAT I AM A HEALTHY INDIVIDUAL WITH AN EXTREMELY HIGH TOLERANCE TO PAIN AND HAVE PLAYED SPORTS MY WHOLE LIFE. i HAVE HAD MANY INJURIES AND THIS INABILITY TO PUT WEIGHT ON MY LEG WAS THE WORST - INCLUDING BREAKING MY ANKLE IN TWO PLACES. It is now a year and a half later, and I still have extreme pain in the spot of the injection and up and down the knee. There was ZERO pain on this side of the knee prior to the injection. Further, I have done the research and in fact there are substantial side effects to these injections, that outweigh the potential benefits. Good doctors have gotten away from giving them. Money doctors without conscience still do them, gotta keep up a lifestyle I guess.... In summary, this man should not be a doctor. He should have done the world a favor and stayed in engineering. I can't imagine what prompted him to change fields other than $$$$$. It certainly was not an empathetic desire to help people. I wish I never went to him, or Premier for that matter. I would have been much, much, much better off.

While the administrative and nursing staff was relatable, attentive and considerate, the experience with the physician was nothing short of disgust. He was condescending, arrogant, dismissive, belittling and more within the first five minutes of the appointment. While a physician may be an expert, with proven best practices, at the very least should speak with a patient and discuss their concerns. Dr. Carroll presented a very "my way or the highway" approach to my mothers care plan without any consideration for her thoughts. Insofar as intentionally turning his head while she was speaking with crossed arms and shaking his head. All over a cortisone shot. I'm sure this message will fall on deaf ears. If you're reading, Dr. Carroll, I hope that even just for a brief moment you'd consider that your patients are deserving of more than just a lecture. That you would have a bit of compassion, humility and understanding and not be immune to the day to day you see, and meet people where they are.
