When we were faced with needing to find a hospice care team last November, I turned to Yelp and found Angel Palliative Care and Hospice. From the very first call, Lena walked me through how hospice works and what to expect. Within just three days, Lena and her team had coordinated everything needed to bring my father-in-law home from the hospital. Communication was outstanding. They maintained a family group text so we could stay in constant contact and always know what was happening with his care. The head nurse, Yana, was a true professional, and the emotional support provided by the entire Angel Palliative Care team helped our family get through an unimaginably difficult time with far more peace and confidence than we thought possible.

They helped my grandma, Anna, and I truly cannot recommend them highly enough. From the start, the team was incredibly professional and organized. All the necessary equipment and medications were provided with perfect accuracy and timing, which took a huge weight off our shoulders. You can tell this is an excellent team that really works together and supports one another; there was always someone available to help us whenever we needed it. What really stood out was their responsiveness during an emergency. We had a frightening moment when my grandma's CO2 levels dropped, and my mom called them in a panic. In less than an hour, they had someone at the house to fix and replace the pipes, and her levels returned to normal immediately. They were always reachable and super responsive, which provided our family with much needed peace of mind. If you are looking for a team that is both highly skilled and genuinely supportive, don't hesitate to reach out to them. They gave my grandma the care and dignity she deserved, and we are so grateful. Thank you Lena, Natasha, and the rest of the APC team!

I got to know several of the caregivers personally as we cared for my father together: I especially treasured my father's time with Yvonne, Rihyana, Sebastien, and Kaye. All four of them were outstanding: deeply empathetic, and highly skilled. I also felt very comfortable leaving my father alone with Home Instead, as they were trustworthy, and even kept my father's apartment clean. I had a few concerns along the way, and the Home Instead administrators were responsive, and made all the changes I requested. Thank you for making a difficult time in our family's life a little less difficult and a little more joyful. Home Instead was everything I was hoping for in hospice caregiving.

After years being responsible for providing caregiving and establishing services for my mother and…read morenow my sister, who requires 24 hour care, I highly recommend CrossRoads for home care services. My sister was in an assisted nursing care facility for 6 months, and the thought of her continuing to reside in a similar environment was heartbreaking. While there, the numerous and almost monthly visits to the ER due to UTIs, inadequate doctor and care oversite, lack of consistent and timely drug and therapy services was truly unacceptable for her health. My sister is now living with my family in our home, and CrossRoads has allowed us to find an alternative path to her care along with using other support teams. From CrossRoads, we received scheduled physical, occupational and nursing services. Their exceptional services have allowed my sister to remain in a home environment and us to build a secure loving environment for her long-term care. I have used other home care services, and would consistently request for CrossRoads services. Her medical needs are addressed by a caring, effective, reliable, proactive and incredible staff at CrossRoads. I could not have asked for a better team to help with this unprecedented emotional and physical challenge my family has undertaken. Our physical therapist Jason, engages, motivates and physically prepares my sister for mobility and flexibility and encourages her to maintain and maximize her existing mobility. My sister enjoys their conversations. He is responsive, consistent and timely. We could not have asked for a better partner in providing the physical and emotional support my sister and my family require. He is simply the best and anyone would be lucky to have him on their team. I worked with Shanel, nurse and case manager, for both my mother's and sister's home care. She is incredibly responsive and experienced in addressing the medical needs that arise. She is proactive in recommending solutions and next steps, all of which provides assurance that as medical needs arise, they will be addressed immediately. Shanel de-escalates the stress and uncertainty surrounding medical situations. Shanel provides the confidence that home care is a viable and very workable situation in partnering with my sister's medical team. It's very obvious that Shanel enjoys her work and patients, and truly focuses on the customers' needs. When my sister arrived at our home, Sabrina, Occupational Therapist, immediately identified tools, equipment and recommendations that provided additional comfort to my sister and to our family and support caregivers in mitigating our physical demands. She was incredibly efficient and facilitated accommodating our needs immediately with her experience and resources. I feel our sister received the best care from Sabrina and CrossRoads in tailoring our home as effectively as possible in maximizing my sister's care. Equally important as the effective direct skills of the CrossRoads staff, is the intangible support they provide my sister and to us. We don't feel alone in caregiving, many times a heartbreaking process. We feel there is an incredible team available to support the care of my sister. My sister is engaged and enjoys interacting with the entire staff providing for good moments.

We've had excellent care from Caroline Noel, Physical Therapist, and Serge LaFuente, RN. Both visited regularly and were super sensitive to my husband's needs despite the fact that he is a "difficult" patient. Our physical therapist, Ms Noel, seemed to have a special understanding of my husband's situation that helped reduce his pain whenever she visited. I also appreciated Kyle Yenko 's friendliness and care for our situation.

It was communicated enough to stop the in-home care services, so it could have been communicated to your sales team in hospice. It's not logical, so I'm left to assume it was just your business development tactic. Plus, the new organization we switched to (By the Bay Health with UCSF) called and notified you, as well as I told the nurse case manager, that's 2 very concrete confirmations that we were switching to another organization, so there's no reason you wouldn't have known. I also failed to mention that it took a week before an LVN or any of the providers came to see my mother after her hospital stay. I also had to call daily to find out when providers were going to come see her, as there was no communication and no sense of urgency to help her. The only reason they started to send people was because I kept calling. Also, the nurse case manager never even came to see my mother. She called to schedule a visit to my mother 2 weeks after her release from the hospital -- only after she heard we were going to switch to another organization and transition to hospice care. It was so unprofessional of her to call and try to do that after our new organization had notified you all that we were ending in-home care services and didn't want your hospice services either. You develop a bad reputation this way and people actively tell others not to use your services with this kind of service and these practices. I assure you that I and others who gave you poor reviews will spread that word.

She initially needed more care than most assisted living facilities we're willing to take on, but she was welcomed with open arms at Portola. Within a few months she was thriving and needed less care. They made her feel like a family member and treated the rest of the family as if we were a part of that. She had a few hospitalizations that took her away, but she always raved to the doctors and nurses about Portola Gardens and stressed that she wanted to get back as soon as possible. I feel that my mother in law was ready to die when we first brought her here, but through their loving care and sensing her personal needs, she not only blossomed, but she had a purpose in life. They helped her to want to live again. They had numerous group activities that she loved being included in. She had her posse of friends and through her gift of gab she had a way of making other residents smile and this gave her great pleasure. The staff knew all her quirks and anticipated her needs before she would have to ask and this gave her a great sense of security. Her room was lovely and they were always helpful when we wanted to make it feel more like home for her. The place always smelled fresh and clean and never of chemicals. I'll never forget the first day when we toured the place and we walked into to dining room and it smelled like Thanksgiving day. She raved about the food. We were always invited to be a part of holiday celebrations and when we ate there, we raved too! I can't thank them enough, because not only did they give my mother in law her life back, they gave us ours back because we always knew she was being expertly cared for and if there were any medical issues we would be informed immediately. Covid had its challenges, but they went above and beyond to make sure we could communicate with her and they came up with creative ways for us to interact. I must also add that they kept her safe in the pandemic and this was a woman that had endured almost every medical condition that could be acquired. There safe practices gave us great comfort during a very scary time. I can't give a higher recommendation and I will be forever grateful to the staff for making her last days as happy as possible. We grew quite fond of all the staff at Portola and will always consider them family.

The caregivers do not seem to have any supervision or accountability and routinely fail to follow basic safety protocols and routinely disrespect VULNERABLE residents. Families should look very closely at how care plans are followed, how staff respond to resident limitations, and how complaints are actually handled. Proceed with caution at this facility.
