Within just three days, Lena and her team had coordinated everything needed to bring my father-in-law home from the hospital. Communication was outstanding. They maintained a family group text so we could stay in constant contact and always know what was happening with his care. My sister was in an assisted nursing care facility for 6 months, and the thought of her continuing to reside in a similar environment was heartbreaking. While there, the numerous and almost monthly visits to the ER due to UTIs, inadequate doctor and care oversite, lack of consistent and timely drug and therapy services was truly unacceptable for her health. My sister is now living with my family in our home, and CrossRoads has allowed us to find an alternative path to her care along with using other support teams. From CrossRoads, we received scheduled physical, occupational and nursing services. Their exceptional services have allowed my sister to remain in a home environment and us to build a secure loving environment for her long-term care. I have used other home care services, and would consistently request for CrossRoads services. There is one shower, so being a robe and flip flops and maybe a little shower bag. You will have your own bathroom, frig and TV. You can also Door Dash if you want, but the food the good enough that you won't need it. Hope this information is helpful for other's staying here and I wish you a pleasant recovery.

I…read morehave a hard time believing the information about our switching to a different organization for hospice care didn't get communicated internally. It was communicated enough to stop the in-home care services, so it could have been communicated to your sales team in hospice. It's not logical, so I'm left to assume it was just your business development tactic. Plus, the new organization we switched to (By the Bay Health with UCSF) called and notified you, as well as I told the nurse case manager, that's 2 very concrete confirmations that we were switching to another organization, so there's no reason you wouldn't have known. I also failed to mention that it took a week before an LVN or any of the providers came to see my mother after her hospital stay. I also had to call daily to find out when providers were going to come see her, as there was no communication and no sense of urgency to help her. The only reason they started to send people was because I kept calling. Also, the nurse case manager never even came to see my mother. She called to schedule a visit to my mother 2 weeks after her release from the hospital -- only after she heard we were going to switch to another organization and transition to hospice care. It was so unprofessional of her to call and try to do that after our new organization had notified you all that we were ending in-home care services and didn't want your hospice services either. You develop a bad reputation this way and people actively tell others not to use your services with this kind of service and these practices. I assure you that I and others who gave you poor reviews will spread that word.

She initially needed more care than most assisted living facilities we're willing to take on, but she was welcomed with open arms at Portola. Within a few months she was thriving and needed less care. They made her feel like a family member and treated the rest of the family as if we were a part of that. She had a few hospitalizations that took her away, but she always raved to the doctors and nurses about Portola Gardens and stressed that she wanted to get back as soon as possible. I feel that my mother in law was ready to die when we first brought her here, but through their loving care and sensing her personal needs, she not only blossomed, but she had a purpose in life. They helped her to want to live again. They had numerous group activities that she loved being included in. She had her posse of friends and through her gift of gab she had a way of making other residents smile and this gave her great pleasure. The staff knew all her quirks and anticipated her needs before she would have to ask and this gave her a great sense of security. Her room was lovely and they were always helpful when we wanted to make it feel more like home for her. The place always smelled fresh and clean and never of chemicals. I'll never forget the first day when we toured the place and we walked into to dining room and it smelled like Thanksgiving day. She raved about the food. We were always invited to be a part of holiday celebrations and when we ate there, we raved too! I can't thank them enough, because not only did they give my mother in law her life back, they gave us ours back because we always knew she was being expertly cared for and if there were any medical issues we would be informed immediately. Covid had its challenges, but they went above and beyond to make sure we could communicate with her and they came up with creative ways for us to interact. I must also add that they kept her safe in the pandemic and this was a woman that had endured almost every medical condition that could be acquired. There safe practices gave us great comfort during a very scary time. I can't give a higher recommendation and I will be forever grateful to the staff for making her last days as happy as possible. The caregivers do not seem to have any supervision or accountability and routinely fail to follow basic safety protocols and routinely disrespect VULNERABLE residents. While a few staff seem compassionate and want to provide care, the overall atmosphere feels inconsistent and dismissive of legitimate concerns. Families should look very closely at how care plans are followed, how staff respond to resident limitations, and how complaints are actually handled. Proceed with caution at this facility.

Health Link Hospice provided services to my sister who was at the late stage of Alzheimer's…read moredisease. Their in-person providers (nurse, aide, spiritual counselor) were terrific - competent, compassionate, well-organized, and helpful to both my sister and to me as her HPOA. The nurse saw her twice weekly checking her vitals and assessing her. The aide helped her caregivers with her daily needs. Health Link ordered hospital-quality equipment (a really good bed and other equipment that she might need as she progressed to end-of-life.) The office staff were also available and efficient -- handling paperwork issues, etc. And Health Link has 24-hours -- even tho' we didn't need them then. And their advice was so helpful to me and to the caregivers at the Memory Care facility where she lived. Five stars plus!!
