    Rooms dirty and nasty

    This nursing home is a joke! It's a Adult Day Care. The faculty is not clean, short staffed. The management and leadership is a hot mess. It's all about profit and meeting the minimum standards to pass for the state. I blame the state for allowing this company to buy so many faculties and not run them properly. This place is dirty and nasty. I complain every day about someone from house keeping to come clean the rooms and the staff is not checking on the patients.

    PLEASE READ!!!!! This place was absolutely terrible from the facility to the front desk staff. My father was in there with critical respiratory care. Because of covid-19 visits were non existent at first. My children and I had to visit him from an outside window due to no iPad being available for a virtual visit, which he wasn't able to turn his head so that was unsuccessful. On his birthday 5/31/2021 I was finally able to get a "compassion visit", even though the social workers had cleared his visit because of his condition, they still told me I could only stay for 1 hour. he was in a dark room, no tv on, no music, that WE provided for music therapy. Mind you, my dad had severe brain injury, a ventilator, trachea tube, feeding tube and non verbal and paralysis. Not only did the staff not acknowledge his birthday, they didn't know it was on that day!! These patients haven't seen there families in over a year , the least you could do is acknowledge them. Luckily, my sister and I was always on the scene so they couldn't pull the wool over our eyes. On this day I decided to inspect his room. Because my father was a respiratory patient he was at high risk for any infections, his air quality and cleanliness was very important. To my surprise there was an excessive amount of black mold in his bathroom . I WAS LIVID!! his respiratory therapist(a guy) walked in and I said, "mold safe for a patient with respiratory issue?" He said that mold wasn't good for anyone. I then told him to go into the bathroom and take a look..at first he said he would get a nurse and i told him "your in charge of his respiratory therapy you go and take a look and I want this documented in his charts (I'm sure he didn't) the side of his bed was smudge with old dirt and dried up food..(mind you my dad had a feeding tube) Ironically/Sadly, the next day my dad went into respiratory distress and was rushed to the hospital and never returned. The next day my sister and I went to speak to administration (who came out an 1 1/2 hrs later) they had all his stuff packed ..less than 24 hours. I asked why and they said if he's not here he's not a patient. I asked them how do you know he's not coming back!!!..i was so upset. Needless to say..it took them over an hours to locate missing items. My dad never returned to that disgusting place. Sleep Well Daddy 6/13/2021

I was evaluated by a senior nurse, a PT staffer & an Atlantic Health primary care Dr. Social worker provided assistance to arrange home help care upon discharge. Being 77, it was unique to feel like the youngster in the crowd. No doubt most of the patient population are on a one way journey. The monthly birthday celebration included many who could not appreciate the festivities. Food service is adequate. Scrambled eggs at breakfast was better than decent. Snacks are offer. Spent a week last year at Kessler West Orange, a world class facility. For my current needs, Shady Grove was decent & approvable by UnitedHealth.

He was in the updated part of the facility and seemed to be treated well by staff and nurses. However, he was soon moved to another part that "cares" for people with dementia - and it was downhill from there. The area was extremely dingy and smelled awful. During several visits, the odor was so bad that it cause us to gag and have to step outside. Then one day, we showed up to find him wearing someone else's clothes even though he had two bags of his own in his room. At another point, they lost all of his clothes after taking them to laundry, despite us saying we'd take care of that ourselves after multiple staff told us the "laundry system is a mess." The bathroom in his shared room also malfunctioned several times during his time there. Adding to the frustration was the extreme lack of communication. On almost every occasion, it took several days to get a response. In fact, weeks later, we're still waiting for a call back from someone. And when we finally spoke to the administrator, it was no help. He seemed resigned to the fact that this was how things operate there. That same attitude was shown from the social worker there too. While she promised to keep us informed when he was admitted, that wasn't the case at all. She was most communicative when it came to discharge, which is revealing. We were thankful to finally find a nurse who told us to talk to her if we wanted information, but there's no excuse for it being that difficult. It is never easy to have a loved one in a care facility, but Spring Grove did nothing to make it better or ease concerns. Not everyone may have a choice, but if possible, explore options other than Spring Grove!!!

I chose the location because it was close to my sister and she was my primary care giver during rehabilitation. My Sister and Brother-in-law transported me from the hospital fto Ashbrook. When we first walked in we walked smack into the smell, hitting it like a putrid brick wall. The hallway on both sides was lined with dinner carts. All I could think was in the event of a fire, nobody was going to get out. Room was small, furniture was shabby. Bathroom looked like it hadn't been upgraded since the 1950s and it serviced 4 people ( 2 rooms, one on each side) . No one told me about this arrangement and the next morning I walked in on a gentleman. The first night I had my physical evaluation which consisted of me walking up and down in the narrow space between the bed and the wall. There was no call button by the bed for emergencies . The food was such slop that it made me gag to look at it. About 15 minutes after I got there I was on the phone to my sister saying come and get me, I'll rehab at home with the visiting nurse service. My sister said she wasn't about to leave me there and she was already contacting The Visiting Nurse service. I was out of there by the next morning. If you have a choice, don't condemn anyone you care about to this place, it's like the 7the level of Hell!!

While she was home she was able to walk and talk. When she came home from this place after a month she couldnt do either anymore. Literally let to sit and waste away in a "rehab". This place is quite the opposite. They should call it a deterioration camp for the elderly. Your possessions will go missing, you will not be cared for, and you might as well sign your life away.

    My mother was admitted to Complete Care at Clark following a partial hip replacement. Less than 24…read morehours later, I demanded her discharge due to verbal and physical abuse by a night-shift CNA and unsanitary conditions within the facility. During the night, the CNA refused to assist my mother to the bathroom and told her to "go ahead" on her own. My mother explained that she had undergone hip replacement surgery just two days prior and could not ambulate independently, the CNA responded, "I don't want to touch you." When my mother asked for help getting back into bed, the CNA forcefully flung her leg, causing severe pain. When my mother screamed, the CNA stated, "I told you I didn't want to touch you." Later, when my mother again requested help to use the bathroom, the same CNA questioned her need, saying, "You really need to go again?" She then made inappropriate and distressing comments, asking who would help her at home. When my mother said her husband would assist her, the CNA replied, "If you do this at home, your husband is going to leave you." This behavior was cruel, degrading, unprofessional, and completely unacceptable--especially toward a vulnerable post-surgical patient. No one should ever be treated this way. In addition to this abuse, the facility itself is filthy. The room appeared poorly cleaned and unsanitary, which is especially alarming for a post-surgical patient at high risk for infection. These conditions only compounded concerns for my mother's safety, dignity, and overall care. I strongly urge others to carefully consider this experience before entrusting their loved ones to this rehabilitation center.

We moved Mom…read morehere hoping she'd get the care she deserved in her final days, but what we got was neglect and distress. The worst moment came when they moved her to a private room. My mom, who was actively dying, couldn't be off oxygen for more than a few seconds. When I arrived, they were wheeling her to her new room--WITHOUT oxygen. She was gasping for air and in a panic, crying. I begged them to hurry, but the aide pushing her wheelchair moved at a snail's pace while my mom was terrified. I even grabbed the wheelchair to try to push it faster, but she wasn't responding. I yelled, "Someone please help!" but the staff just stood in the hallway, gawking. While the mistake was eventually acknowledged, my mom felt totally unsafe--and she was. She died the next day. I deeply regret trusting Mountainside with her care. I cannot recommend this facility to anyone. Please be cautious and consider other options if your loved one needs serious care.
