They are located inside the Bon Secours Medical Center. The parking entrance is to the right shortly after coming onto the road. The first set of parking you'll pass is the physician parking area. Keep going around to the main entrance. There is signage pointing to the main parking area. There was plenty of parking available, including handicap spots. The most recent visit here was on July 17, 2024. I wasn't the actual patient for these visits, but it was for my mom after having a wrist fracture. They don't have the hand therapy at all locations, so this was the closest one. The visits would occur twice a week for eight weeks. The inside of the place is quite large in the therapy area. It is always clean and well maintained. You could see that they can cover many different types of therapies there. The overall process is pretty straightforward. On the first visit, they go over the insurance information and what the copays, if any, are for each visit. This was also the time to fill out any paperwork and such. For subsequent visits, it was just checking in. The overall therapy process went very well over those weeks. The first part of it was for healing and the other part was for strengthening the wrist. There was a genuine care and concern throughout the process. There were times where they changed it up a bit since it was affecting her shoulder (the one she had a replacement on the year before). If you are in need of therapy, this place is worth checking out if it's close to you. There are several locations throughout the area.

As with any place there are always pros and cons : The positive pros are about some of the wonderful staff - First is Samantha Alzola the Social Worker. She's very professional, caring and provided my sister and myself numerous guidance and assistance while navigating these new, uncharted waters. Always pleasant to deal with, no matter how much her work load was. Next are the individual care givers , the L.P.N.'s and C.N.A 's. First is Andrea Dunn, PA. she was very knowledgeable, approachable,and runs the all the medical care . She spent time to explain Mom's situation and all options to help us make hard decisions. Andrea was easy to get along with and not intimidating at all. Next are the caregivers who after 3 months I got to know quite well. there are always "earth angels" and 2 who no longer work there that I want to mention as they were instrumental in mom's care : Maria CNA and Markita LPN. Nikierra LPN(Nikki) was a wonderful, kind, caring and a compassionate caregiver to our mom. Also were the CNA'S who showed extreme kindness and love whenever they had to take care of mom. They are Cheree, Danasia, Shanice And Darlene. Darlene was the first caregiver when mom arrived who filled us in with all the important details and guided us on the way things worked. Lastly was a speech therapist Becca who showed kindness and compassion whenever she dealt with mom, whose hearing was extremely lacking. Since this facility is relatively small with only 3 wings and 60 beds I believe the caregivers really get to know the patients and their family , if they choose to. Here are the cons that I'd like to share: Within one week mom's left hearing aid was missing. We believe it was thrown away on one of her meal trays. I reported it to various staff members and got the same reply, "the administrator is aware of it". The Administrator Madison, never contacted either myself or my sister about the hearing aid so we assumed it wasn't too important to her otherwise she would have addressed the situation. We decided not to pursue the missing hearing aid since mom still had 1 left. The next negative experience was when my sister arrived and was told by my mom's roommate that my mom Fell Out Of Bed !! That was quite surprising especially since mom was bedridden and not able to move by herself. First of all neither of us were called and notified. Once my sister began questioning staff we then learned that while a CNA was washing her, she turned my mom too far and that's how she fell out of the bed. Yes they took ex-rays and no bones were broken but not to be notified was quite alarming and poor administration protocol. Other staff could not believe we weren't called. There was a meeting arranged with the head of nursing Shannon and my sister a few days later. The results were a bunch of excuses why we weren't called, like they didn't know I was the POA ( which was clearly a lie, as that is the first form along with end of life wishes you fill out!) . All we got was lip service and a lot of excuses why we weren't called. This to me was totally unacceptable and when I saw Shannon a few days later and tried to speak to her she was extremely rude and unprofessional to me. This is my personal experience with Bayside Health and Rehab from June 2025 thru beginning Sept. 2025

    My husband was transferred to Bayside after being in ICU and step-down for over a week following a…read morehemorrhagic stroke. He could only eat a mechanical diet (finely chopped foods) or pureed food, but he was receiving the same type food as everyone else (which was a choking hazard). His liquids were supposed to be without ice and thickened using a thickener (I provided some packets), but they brought him ice water to drink (another choking hazard). I would point this out each time they brought his food and/or drink wrong, and I would get "Not my job" or "Not my problem". At the end of the third day, I had heard those two statements over 16 times (I stopped counting at that point). It was four days before I spoke with anyone other than the CNAs, Nurses, other medical staff, and housekeeping (they were the nicest). None of the medical staff identified the role or advised if they were a nurse, CNA, volunteer, etc., with the exception of the CNA the first night - it was her first day on the job and her third day after completing her training. I watched as patients rang for assistance, and saw that for some it took well over an hour for a response (medical staff were at the central desk, most of them talking not working). The gentleman my husband was sharing a room with rang for help getting to the restroom, and after over 75 minutes someone (nurse or CNA) came in and when she found he had wet himself chewed him out for not waiting!!! Then she said it wasn't her job to clearn him up!!! He was supposed to have daily physical and occupational therapy as well as speech therapy. In the six days he was there, he had a single session of speech therapy and approximately 60 minutes of physical therapy. We started working on getting him transferred, and were fortunate to find a bed at Lake Taylor, but getting him discharged from Bayside was another problem. Lake Taylor had a bed ready for him on Tuesday, and they had sent their paperwork to the person "in charge" of transfers, but she did not return any of my calls or messages. By Friday of that week, I had arranged for medical transportation to take him to Lake Taylor that afternoon. After he was on the gurney and ready to be moved, a woman came running down the hall saying "I forbid him to leave". She was the person I had been trying to get all week. Long story short (too late), I gave her 5 minutes to provide me with the needed paperwork, or the gentlemen with the gurney were leaving with my husband and taking him to Lake Taylor. We left in about 4 minutes and 47 seconds. He has been at Lake Taylor for about a month and a half, physical and occupational therapy 5-6 days/week and some days more than once, his has finished speech therapy, and is improving. The difference between Bayside and Lake Taylor wasn't night and day, it was 2 miles down in a coal mine and day. DO NOT SEND YOUR LOVED ONES THERE!!!!

I attended therapy for three months, and every day I was welcomed and well cared for. Libby and the team always checked in on me during my exercises, making sure my form was correct so I wouldn't risk further injury. From start to finish, I felt respected, supported, and never out of place. Everyone was courteous, attentive, and treated me as if I were part of their own family. I honestly can't imagine a bad experience here -- my time was nothing but positive. Thank you, Libby and the team, for your dedication and genuine care. It truly made a difference for me.

    This Southeastern Physical Therapy location is located on McComas Way. It is fairly close to the…read moreintersection of General Booth Blvd. and Nimmo Parkway. The easiest way to get in is from General Booth Blvd. southbound where Jose Tequillas is located, as there is no direct access from the road there. So, what brought me here in the first place. Achilles Tendonitis in the left foot. In April 2018, I was walking to get my lunch at work and I felt something pop with my left foot which didn't feel good. The ground is flat too. So, after seeing the specialist and finding out it was Achilles tendonitis, I was referred here for physical therapy to get this issue resolved. The good thing is that I started this process pretty quickly after it occurred, so it minimized the effects and length of time needed for physical therapy. When I called the office, I was surprised at how quickly they were able to get me in. I was there for my first visit and consultation just two days after seeing the specialist. My therapist for the duration of this issue was Crystal Barnes, who is also the clinic director. The first visit starts with a consultation to go over the issue at hand, take measurements, and the such. After that first visit, it would then be just working through the physical therapy process to get things back to normal. The physical therapy part of this adventure wasn't as easy as I thought at the beginning, but they do start you off slowly with the exercises and then eventually work you up higher as things improve, including adding additional activities. This part is normally done with their physical therapy assistants. Crustal also did some deep massage and dry needling as well. For both of those, you will really feel it and may hurt, especially on the first round, but it gets much easier after that. Over a few sessions, you will definitely tell the difference. I was going there two times a week for a little over two months. After a few months, the issue was finally resolved and no longer needed to continue the physical therapy. Fast forward a few months. I have no recurring issues with my foot at this point for the Achilles tendonitis. I am also still doing some of the exercises as done in physical therapy to this day to keep certain this issue doesn't come back later. The entire staff here is great to work with. The ones at the front desk always greet you with a smile and nice to have small conversation with when it isn't too busy at the time. Crystal has been great with my foot and the issue. If for some reason I do need physical therapy in the future, this will definitely be the place I come back to. If you need physical therapy, you need to check them out as well.

There is an entrance to the same road along Sandbridge Rd. by the Sage Kitchen. My final visit for physical therapy here was on March 6, 2025. There are two parts to this business, as there is another business in between them. The other part is at 2041 Fisher Arch, but they are all the same group. In all my visits here, I didn't have any issues with parking. The outside of the buildings are easily seen from the road. I first went into the main building, but was directed to the other one on that first visit on January 27, 2025. This location is pretty close to me, so it worked out well for location too. A few months prior, my left shoulder had been bothering me, and I had a hard time moving it in certain ways. At first, I thought it was just a pulled muscle or something, but it didn't get any better, so my PCP referred me here for physical therapy on that shoulder. Since the new year had just begun at that time, I knew I'd have to satisfy the medical plan deductible first. I had planned for this at the end of the year before when setting up my FSA. This place is part of the Sentara Hospital Outpatient location. I did find that the overall cost was similar to other non-hospital physical therapy locations. The first visit was primarily the evaluation visit and to set up a schedule of how often to come in. The visit schedule would be twice a week. My primary therapist was Kayla Waterfield. I also worked with Breanna Taylor in some of the sessions. Talon was the primary therapy assistant that I worked with when it came to the exercises too. Even though the first sessions were the lightest in terms of the exercises for it, I felt it most during that period. Over time, new and more complex exercises would be added to the routine. At least I wasn't shouldering this alone. The last part of the therapy left was trying to get the arm to my back. By the time I was discharged, that had gotten much better as well. Now that it's been a few months since I finished the physical therapy, the shoulder is still doing much better than before. I may move it the wrong way every once in a while, but in the beginning, it didn't matter which way it was moving, so this definitely got me into a much better situation. Overall, I have had excellent experiences here. All of the staff I worked with were very accommodating, knowledgeable, and caring. That makes this place a location I would come back to for my physical therapy needs in the future. I highly recommend them, so if you are looking for a therapist in the area, you should definitely give them a shot.
