They called me one afternoon saying she got out of the facility and ended up at stop and shop and didn't know how she got there and her alarm monitor bracelet wasn't working. She could barely walk and it was 90 degrees that day I guess someone visiting a resident gave her a ride there. Then she feel and broke her hip slipping on her own urine and died shortly after her hip surgery. Now it has been 4 months and I paid for the last month she was there in full and she died on the 6 th of May they owe me a good amount of money and will not send me my check. I have contacted them 4 times and sent an email they tell me corporate has to issue me a check but still nothing! This facility should not be in buisness they need to be shut down!!!

Because they don't pay their workers what they deserve, they cannot keep good help, so they hire agency which knows NOTHING about the clients. I received a phone call that my Mom fell and was going to the hospital with a cut above her eyebrow. When I arrived at Charlton I was horrified, not only did she have a huge cut above her eyebrow, she also had a broken nose which they did not mention on the phone call to me. The nursing home couldn't give me any answers as to who put my Mom where she couldn't be watched. I never got any answers and my Mom died 2 weeks later. Yes I blame this facility for the loss of my Mom and I WOULD NOT recommend this place at all!! My Mom lost her life because of the neglect in Southpointe. People beware!! Research before you put your loved ones in one of these places.

My grandmother stayed in this nursing home for years (at her own request) until finding herself in an assisted living home which was much better. She told us stories of nurses not caring about people, not taking proper care of them, not changing them when they needed to be changed, and sometimes even physical abuse. Her clothes and personal belongings got stolen several times, her medication was messed up several times, and when she needed any medical or personal attention, the nurses never seemed to care and always sent her to the hospital instead of dealing with it themselves. They truly don't care about their job and a whole new staff should replace them. I didn't want to believe any of this until my sister, who a temp nurse, worked there this past weekend and said she has NEVER seen anything like it. All of my grandmother's stories were true. The place isn't up to code on ALMOST EVERYTHING. Do not bring your loved one to THIS kindred location. It may be the worst in the whole state.

When I was presented with a choice of 3 NH to choose from I did not hesitate to choose Crawford for the care of my Mom. The Nurses, CNA's, Social Work Department, discharge planning, Occupational and Physical therapy departments were great. My mother was treated with respect and dignity and was well cared for. The Nurses answered my questions and addressed my concerns satisfactorily. Mom especially liked bingo! Other Residents and families reached out to help my Mom adjust to her new surroundings. I would like to especially thank Laura & Melissa (nurses) Jess and Paulina (CNA's), Connie from Social Work Department and Melissa from discharge planning.These individuals are caring, compassionate and helped me to have peace of mind while my Mom was there. Should she need further services Crawford will be my first choice to care for my loved one. I highly recommend this Nursing Home. Thank you everyone. RachelBoutin

    A loved one was sent here after a surgery to recover and gain their strength back…read more - They developed infection because they weren't cleaned properly and started to develop sores. - While they were confused and heavily sedated, they sent a billing office staff member to speak to them about payment/insurance. - Transportation to medical appointments was unreliable and staff also did not wake up patients on time to get ready for appointments. - Physical therapists, that are supposed to provide therapy 5xweek, come when they feel like it. They wheel patients back and forth tho therapy and write in the notes that patients walk to therapy. - Because our loved one was left unattended, they fell and were admitted to the ER/hospital. We quickly found another facility to properly care for our loved one. Some of staff were kind and attentive but there was no accountability for those that were not.

Fell out twice second time broke femur in half which ultimately killed her You could improve these facilities by hiring staff, training staff, and care for patients instead of considering them a burden you have to check on occasionally until your shift is over. You also could ban the use of cell phones by staff. I also don't care if these are obviously what you claim are current standards. If so start implementing them

    spent 3 weeks there, first week alone, then they filled the other bed in the room... The man behind…read morethe curtain was elderly and needed a Hoyer lift to get in and out of bed and be changed and what not... Had trouble with many of the CNA's not speaking English very well, but they usually respond to the call light quickly except for two different times when there was a long delay... there was a lady across the hall who was out of it and she would scream all night multiple days per week... they did not seem to have any solution for that. So Sleep was a problem. I guess the physical therapy and occupational therapy I experienced there was somewhere near adequate. Staff is generally very friendly, did not meet anybody i did not want to have caring for me. They accommodated me having some food brought to me from my house or the grocery store and let me store some in their refrigerator .. saw the doctor one time, and the Social worker and case manager didn't seem to have much interest in what I needed. Staff nurses seemed to be overwhelmed by Patient demands most days, often rushing if they appeared at all if I asked to see a nurse... I would say that the vibes were mostly good, but they could use more staff to handle the patient demands. I'd say seven of 10 were competent, and pretty much everybody was friendly. I give them a four star rating because they are much better than some other places I've been but there's obviously room for improvement, what's needed are mostly more staff and better food, and better case management/social worker availability and care.
