She was left to sit with soaking wet diapers for hours and hours. It took three plus days for the nurses to look at her bottom after she kept repeatedly telling them she was burning there. She got 2 bed sores, plus a uti and had external yeast infections under her breasts and in the creases by her thighs. The director of nursing is shameful: she is in charge of the nurses and CNA's. What happened to my Mom is a nightmare. She has been hospitalized multiple times now for these wounds. DO NOT SEND A LOVED ONE THERE BECAUSE THEY WILL BE NEGLECTED!!!

My grandmother was transferred to this facility, and although I tried to remain hopeful, those hopes were quickly crushed within the first week as everything began to decline. This is not a facility where family members can relax or trust that their loved ones are receiving proper care. We have had to stay vigilant every step of the way. My grandmother is here to be rehabilitated, but how can anyone heal or progress in an environment where the standard of care is deplorable? There is absolutely no sense of urgency among the staff. These are elderly patients many with serious medical needs yet call buttons are ignored for up to 50 minutes. That level of neglect is unacceptable. The dismissive, unpleasant attitudes displayed by several staff members are also deeply concerning. These patients are the very reason this staff is employed, yet they are treated as burdens rather than human beings deserving of dignity and compassion. One staff member in particular, Farrah(nurse), stands out for her unprofessionalism. "Nasty" does not even begin to describe her demeanor. She attempted to argue with me, claiming she was not the tech assigned to my grandmother's room. Regardless of her assignment, she is still a nurse, and nurses are not exempt from providing basic patient care. Her refusal to assist was unacceptable. During what we assumed would be a normal visit, my mother and I were met with yet another disturbing situation. My grandmother who suffers from gout her hand swollen like a balloon, along with severely swollen ankles and feet. This was due to the fact that only one staff member carries the key to the supply closet, resulting in unnecessary delays in care. Again, a complete lack of urgency for a patient dealing with significant fluid retention. When she needed to use the restroom, we waited approximately 30 minutes with no response to the call light. Eventually, my mother and I had to help her ourselves. What exactly are nurses being paid for if family members have to step in to perform their duties? We even timed the response on purpose just to confirm what we already suspected. I attempted to speak with Connie, the floor manager. Not only was she unavailable at her office, but when we finally reached her, she was dismissive and indifferent. Her lack of concern explains a lot about the culture of the staff under her supervision. What makes this worse is that the entire facility continues to show no accountability for their actions. Instead of addressing the issue with honesty or empathy, they sent the nurse practitioner into the room to "smooth things over." Rather than listening to our concerns, she chose to boast about her credentials, which came across as dismissive and unprofessional. This approach did nothing to resolve the situation or acknowledge the seriousness of our concerns. At this facility, any attempt to ask reasonable questions about a loved one is immediately met with accusations of being "hostile." Instead of offering clarity or compassion, staff respond defensively, as if they're already guilty of wrongdoing. The overall tone and attitude throughout this facility are unacceptable cold, dismissive, and completely lacking the empathy that families deserve. I fully intend to escalate this situation as far as necessary. The care here is unsafe, unprofessional, and emotionally damaging, and changes must be made immediately.

I dealt with various staff members there from time to time over the course of several months and…read morethe service was typical with what you would expect from a lower-end/lower-budg assisted living facility... not great. Most assisted living facilities are very sad and depressing places to be and work, and they have a very high turnover, and a hard time keeping any type of talent in terms of the workforce, unless you go to the fancy, schmancy ones (these are the ones that do not take medicare - and are typically expensive. But hey, you get what you pay for too.) This could be a 2 star or even 2.5. Then I spoke with the manager Mr. Jeff Valdez, and actually had a very pleasant phone conversation with him. And we followed up with an email. He even stated if I needed anything else don't hesitate to reach out. A couple weeks later I reached out, emailed, called... Another week went by and repeated the process, and Mr. Jeff Valdez turned in to a ghost. And I know he read my emails because I had read-receipt set, and they came back as read. So I don't get it, but that's what we call "extraordinarily poor customer service." Non-courtesous response = 1 star review. You can do better.

They were both very kind and caring!…read moreYes the food quite often was cold but I found out if you ate in the dining room the food was much warmer. The office was always on top of everything. The Physical and Occupational Therapy Departments were amazing and some of the best I have ever been to. They had lots of things to do, like every Wednesday they had an ice cream social, with entertainment, bingo, bright minds, Bible study and a church service every other Sunday, just to name a few. If I ever have to go back to a nursing and Rehab place, this is the one I would choose!

I would NOT recommend Palm City nursing home for rehab unless your loved one can fend for…read morethemselves. My mom spent only few days there before I had her moved. She is under 90lbs and 4 feet something tall. They had her in an oversized wheelchair. She couldn't reach the floor nor could she rotate the wheels to go anywhere. I was told she was sliding down, so when I leave, move her to the lunch room area where she could be watched. Really, how about a proper fitting chair? During one visit she needed to use the bathroom so I pushed the nurses call button. Twenty minutes later two therapist came in. I'm assuming for her session because (assuming again), they don't usually handle bathroom duties. They decided to walk her to the toilet. Well, I'm sure you guessed it, she didn't make it. She's 95 years old and waited twenty minutes already! Also, one of the cabinets in the room was filled with the last persons belongings, so how thorough is their housekeeping team. Also, there was an alarm sounding and I asked the person at the nursing desk what it was for, they weren't sure. I would think alarms would be part of their training to know exactly what had happened.
