The loving and caring activities Group as a whole; help each of us break away from the cares of life & motive us to smile, laugh & forget some of a pain and suffering, even if it's just for a couple of hours, 1hour, for a few minutes or just for 1 moment of the day. From janitorial were they keep the center very clean, next to godliness to housekeeping who makes sure you have the proper linen and bathroom toiletries you need. Then have CNA to ensure your well taken care of if you need a shower to aa diaper change while the NURSES are here for your medical wellbeing and on call Docters for case and emergency. With that said you can NEVER go wrong being here @ Hayward Healthcare & Wellness Center. She saw when we would visit our LO that we were struggling. He has Advanced Dementia, and she took the time to explain to us, what we would be going through. She explained to us that her own Father went through the same thing, so she knew what we were going through. Leila also answerd our questions we had, which we had many, she made us feel not guilty for leaving our LO there, she made us see that it was the best thing for him, since he would be looked after 24 seven, which is the care he needs.     My dad has been receiving excellent care at St. Francis Healthcare Center. He was admitted after a…read moremajor surgery with a feeding tube. Every staff member-- the doctor, nurses, CNAs, physical therapists and office staffs, has shown genuine care and professionalism. The doctor checks on him several times a week. The nurses and CNAs are attentive and gentle. They always treat him with kindness and respect. The dietitian and kitchen staff work tother to thoughtfully adjust the meal plan to meet my dad's special needs. Physical therapists are excellent. They approach each session with patience, encouragement, and deep expertise, helping my father regain strength step by step. Most importantly, the entire staff -- from management to clinical teams -- is caring and easy to communicate with. We are truly grateful for the compassion and teamwork here. As Thanksgiving approaches, I want to thank the St. Francis team and wish you all a joyful and restful holiday!

I was inside the home plate with my mother-in-law and the experience was very bad. They have a very decadent care and especially the elderly suffer Ahy. My mother-in-law came out 2 times and they dropped 3 times and no one was responsible for what happened. The social worker Cinthia is the worst of an unscrupulous person. The only thing they want is to have patients to take their money from the government. My mother-in-law is a person who needed assistance to go to the bathroom and they left her with pampers all day until we arrived and changed her. In the place they have to socialize. They don't have cameras and I saw how the patients fell and they didn't even realize the hygiene is terrible. Please all the people who take their loved ones be very careful and don't leave them alone. Because they suffer a lot

Gateway was dirty, musty air, and had the smell of dirty bedding. Quite a surprise when I went inside Emmanuel Post Acute for the first time. It's painted, clean, and there's no foul odors. The nursing stations are well staffed. I went to visit a friend, but he was asleep. The nurse advised me not to wake him. So I don't have his opinion on the care or food there. However, one thing I can comment on is Covid-19 screening. Temp checks, sanitizer, and masks are available at the front lobby. But no visitor screening is done. Maybe half the staff are wearing masks.

Mind you, she had fall prevention on her charts. There were no fall prevention measures. Taken? And if that was the case, how is it that she still managed to fall out the bed? So thursday morning, I get a call at 530 stating that she passed away. I'm still trying to wrap my mind around this. If I was dying, I would almost rather they roll out a Yoga Mat. In front of this care facility and just let me die in the street before being admitted, there, do not take your loved ones to this place. If you truly love them

    This is the absolute worst pharmacy in the Bay Area. You'll sit on hold forever, they NEVER deliver…read morethe medication on time. They deliver to "Skilled Nursing Facilities. They are located in Hayward, CA and they service (and I use that word lightly) nursing homes in San Francisco. For the last few weeks, they managed to deliver some prescriptions on time. But, I've lost track of the times I've been told that they run out of insulin and blood pressure medications. While that doesn't affect me, it affects other patients & there are two long-term floors with a large number of dementia patients. If they don't have family or friends to check to make sure they are getting their their meds on time, it could cost them their lives. I've had serious pain medications not delivered within 24 hours. Why would a nursing home in San Francisco use a pharmacy in Hayward when there are so many other pharmacies in San Francisco? My thought is that it must be about greed. They care less about the patients and I've seen medication for the day before or the day before that arrive two days late at 3:00am or 5:00am. This review is to advocate for those who have no voice and with the many dementia patients who can't speak or show they are in pain. When looking into an Assisted Living Facility or a Skilled Nursing Facility in San Francisco or the Bay Area, please ask which pharmacy they use. I asked for well over a year for the name of the pharmacy and was told time and again, it wasn't the nursing home's policy to give out that information. But, as you can see, I don't give up easily and was finally given the name by various staff members at the nursing home. By the way, when the residents meet regularly, the lack of medication on a timely manner is one of the regular complaints. If you are considering helping parents or other loved ones look into Assisted Living or Skilled Nursing Facilities, please do your homework and I mean dig deep, ask to pick patients to speak to, go online and check out reviews. But, please read carefully. All reviews are not as they seem. I hope this helps even one person make the right decision. It's a shame we can't respond to reviews or chat with others on Yelp. I thought there used to be a way you could communicate with other Yelpers but, I must be wrong. Thank you for reading.

It was…read moreour first time being away from my grandma after she was admitted to the hospital due to congestive heart failure and stroke and ended up needing to go to a rehab facility. We were scared but they were welcoming and allowed for us to bring items from home to make our grandma's stay feel more comfortable. The staff was attentive to her, and we felt so relieved that we could visit anytime and know she was in good hands. My grandmother was difficult but they worked closely to make sure she gets the care and attention she needs. Kudos to all the nurses, therapists, aides, and everyone else working together to make the facility feel like a safe haven for our grandma. A special shoutout to Kulvender Kaur, my grandma's nursing assistant. She took amazing care of her, and went above and beyond for our grandmother. At first, my grandma was a bit scared, but Kulvender was able to earn her trust and treated her like her own family. She truly is the best and we are so grateful for her care. Thank you so much, Kulvender! She deserves a raise and all the blessings in the world. We're so grateful to have found this facility and for my grandma's stay. After three months, my grandma made a remarkable recovery from her stroke. She can now speak clearly and walk again. She is now back home with us again. When I finally reached her case worker I was told to call her social security worker, Carol. Carol told me to meet with her physical therapy team and nurses to understand her care and discharge plan. After the PT team did not show for the appointment, I rescheduled. They met and said that Carol was managing the discharge plan. The. I was told to speak with a nurse. I was constantly sent from one person to another. They seemed very disorganized and I couldn't accomplish anything over the phone and when I was onsite, it still took forever to find someone that knew what was going on. As for care, she contracted pneumonia while there. They tried to discharge her with it, which her insurance objected to. They told me that she would be going home in 48hrs. Then she died. While the EMT's were trying to revive my mother, a nurse from the facility called and asked me what I wanted to do with her body. They didn't bother getting this info upfront and I was forced to send her to their default mortuary. When I called to ask to collect her belongings they told me they had to find them. They may have been thrown out. She was 72. Walked 8 miles every day. This is not a place to send loved ones.

We are big fans of the Belmont Healthcare Center and we feel blessed to have had their help in caring for our loved one. All departments coordinated to provide a great amount of support for my Grandmother. We are truly thankful to the nurses for staying on top of things with medications, frequent and thorough diaper changing, consistent feeding times for meals, maintaining my Grandmother's warmth and hydration levels, celebrating her birthday with balloons and a special sign, and giving her a wonderful quality of life during her stay. The wound care specialist did very well to help her with the stitches and healing process for a wound my Grandmother sustained before arriving at the facility. The activities department provided an environment where my Grandmother could follow the news, be in a social environment, and participate in all types of enriching games, exercises, and entertainment opportunities. The office staff took great care of us a family, giving us tremendous support and guidance during our challenging times, expertly filing all necessary paperwork for insurance purposes, which was a huge relief for us. The physical therapy department worked hard to help my grandmother regain her function as much as was physically possible, taking her for walks and keeping her moving with targeted exercises, and they kept us updated on her progress with detailed feedback. The cleaning staff were very friendly and they did a fantastic job keeping the rooms and common areas looking sparkly clean. The medical staff were very knowledgeable and helped us understand my Grandmother's condition while making sure she received the proper treatment. The kitchen staff worked hard to provide quality meals that met the special dietary needs of my Grandmother. The front desk staff workers were always welcoming and helpful when we arrived to visit. When I reflect on how all these workers I mentioned above worked together to make Belmont Healthcare Center work, my heart is filled with gratitude and my mind is at ease knowing that my Grandmother was very well taken care in a clean, caring, uplifting place.

My mother's dentures were disposed of without her knowledge or consent and when I inquired about what could be done about it I was told an investigation would be launched. I was told this the same way that one might explain to a child that if you don't go to sleep Xmas evening, Santa won't leave you any gifts.... Numerous other instances I could list but the bottom line is ....Unless you really don't care about the person DO NOT UTILIZE THIS FACILITY.

See her below enjoying her crossword puzzle (but, don't worry, she had the opportunity for physical therapy before she got far into the puzzle!) The PT workers, notably Sanjeeta and notably Reggie (Reginald), have strong talents (and leadership skills) to help your loved one spend no more time in bed than is helpful for your loved one. There is a cheat sheet of exercises for your legs, hips, and arms which will get you strong. There is a gym, with the key equipment which can also help. If you do visit the facility, please take care not to get in the way of the hardworking yet cheerful staff as they do their important work. An appointment is not necessary. A greeter checked that I am fully vaccinated (I am actually double boosted.) and confirmed that I didn't have a fever. I saw two guys doing us all the favor that they took their iHealth test while they waited to go in to meet their grandmother. On May 13, I saw a staff member, probably a little over 30 in age, who was bringing the lunch out into mixed sun and shade for a guy who hasn't seen enough sun in the past two months. The guy has a Slavic accent (I think) of some sort, and both of them agreed the guy is overdue for a haircut. The staff member was so very, very helpful! And during what seemed to be the break time for the staff member who was making phone calls, he stayed with our likely Slavic friend. It was a "five-star" recommendation for Windsor Gardens. I am sad that I didn't feel eligible to ask the staff member his name. There isn't always enough parking in the lot under the redwoods; so the most recommended parking is on B Street or along side streets. Actually, if you are health conscious, a good way to reach this facility would be to walk a little less than one mile from Hayward BART; or you could look up a connection via #95 AC Transit to 6th and D St. (actransit.org) which would get you even closer to their doors. The #28 bus also stops right close on B St. as it travels between Hayward BART and Castro Valley, Bay Fair, and San Leandro BART.
