While at the gym today, a very infirmed man had trouble and was in pain to the point of nearly fainting. I asked if he needed help since he could barely stand even with a walker. I looked around the Ground floor for help, but there was no one. I recommended that he press his emergency pendent, which he did. No one came. After five minutes, still no one came. More time passed. Still no one. I decided to go upstairs to the main desk to say someone needed help in the gym. They said they were aware of the emergency call but could not locate him. I then went downstairs and noticed a woman had finally arrived. However, she was rude, and ignored his request for a wheelchair, and began for forcefully push him backwards on his walker. When he mentioned his frustration about not being found after pushing the pendent, she just responded, "I'm here now." I said she arrived only because of me notifying the front desk, and she again slightly angrily repeated her statement. I was appalled at her attitude, but more importantly, her ignoring his request for a wheelchair, which he clearly needed. I went upstairs to reflect what I witnessed and to complain. I was then introduced to a manager. I said that I would hate to have my 84 year old mother treated this way if she had an emergency.

The upside of that is better quality in both food and service and programs. The residents are upscale and hip . The brunches pre Covid are amazing with unlimited mimosas even though my 93 year old mom never has more than one. My mom loves it there and that's the most important thing. After that it went severely down hill to the point that it became dangerous. They aides did not show up. I was told they were on their way including the destination they were coming from but then they said they were 3 hours away not 30 mins, and then wouldn't be there until tomorrow. Which then they didn't show up for a 2nd day! This left my wheelchair bound parents without anyone in the house! And my mother was transitioning to hospice care so needed someone. The team who knew this did not call me in the morning to let me know of the delay (and ultimate no show) and gave the excuse of we don't call people before 7am as it's too early?! WHAT? You are leaving people without care - the sooner we know of your incompetence the sooner I can try and fill the gap!! The wonderful nighttime aides I hired on my own texted to let me know that someone didn't show up at 6am when they were supposed to. Yet the coordinator wasn't calling until 7am?? Then when reaching out to owners, operating officers or anyone I spoke to in the selling process I was even more disappointed. They do not care about your loved ones. They do not know how to handle an emergency let alone business as usual. I dropped them as soon as I could find someone else to step in. It was the worst experience and we have had bad experiences. Do not believe the personable touch you get in the beginning. I actually thought I might be friends with them (Christine) but there was no response after frantic reach outs trying to find coverage for their no shows. Nope she wanted nothing to do with me. The treatment and utter lack of accountability or compassion was staggering. I can not stress this enough- BEWARE of this sham of an organization. Prior to contacting your facility, I confirmed with my healthcare provider that I was eligible for up to twenty (20) days of residential care. My primary care physician and healthcare provider fully cleared the admission, and the Director of Admissions at your facility acknowledged that all necessary information had been provided. However, after completing every required step, my admission was rejected by the facility's Administrator without explanation. Despite my request to speak directly with the Administrator, I was denied the professional courtesy of a response. At 83 years of age--and while experiencing severe, widespread pain--this lack of communication and disregard for my medical needs was deeply troubling. I consider this behavior to be unprofessional, unethical, and reflective of grossly substandard practices. As a result, I find it necessary to formally report this incident to ensure that others are not subjected to the same neglect. Wendell Johnson

BEWARE. DO NOT PLACE LOVED ONES HERE! My father was a…read moreresident here for over 6 years until he passed away. Neither administration nor social work put "care" over "business". Greenwich Woods had my "competent" father sign over Power of Attorney (from me) to an individual I did not know almost immediately. The following week, the facility petitioned the Greenwich Probate court to involuntarily conserve my father. If this happens to you - *bring your own attorney to the probate hearing*. My father had worked hard his whole life, owned a home and had savings. He died indigent, with this facility sucking every last penny out of him possible. There was ZERO communication with family. I am my father's next of kin and health care proxy. Greenwich Woods ignored my legal standing as Health Proxy. They did not make me aware he was hospitalized or had their medical staff at a probate hearing to put my father into hospice status. Between the wrongdoings of Greenwich Woods and the very crooked, abhorrent conduct by the probate court-appointed conservator, my family was robbed of the opportunity to say good bye to our father and his final wishes - contained in a deliberate living will created by an attorney - were not honored. The conservator posthumously declined an autopsy for my father, before his next of kin was called with news of his death. This decision made planned organ donation impossible and added salt to the wound of losing my father. Greenwich woods has no redeeming qualities to which I could even attribute one star. My father, who was bedridden, did not brush his teeth for all of the days and years he lived there. The nursing staff didn't help or insist he maintain personal hygiene. As a result, his teeth fell out. I could not be stronger in my recommendation to NOT send a loved one to this facility. Save your families, explore elsewhere.

I would strongly caution anyone considering King Street Rehab and Nursing Center for a loved one…read more My mother was admitted for subacute rehab after a stroke, and what we experienced was beyond unacceptable. On a daily basis, she was dressed in other residents' clothing. Call bells would go unanswered for over an hour. I spoke with the Director of Nursing repeatedly and was reassured things would improve, but they only got worse. Because she was left sitting in urine and feces for extended periods of time, she developed a severe UTI. Instead of addressing the neglect, they actually stopped giving her a necessary liver medication because it caused bowel movements and they didn't want to deal with changing her. That decision alone speaks volumes. Patients are routinely left sitting in wheelchairs in the hallway for long periods, with little to no engagement, like they are being warehoused. I personally witnessed a nurse handling a patient improperly. When I addressed it, she became defensive and nasty, and when I escalated it to her supervisor, there was no concern or accountability. I spent hours there every day because I did not feel comfortable leaving my mother alone. I had to bring her food because the facility's food is poor quality. At one point, they even told my mother she should not participate in recreational activities because she might feel embarrassed after her stroke, which is incredibly disheartening and inappropriate. The most alarming moment was when I walked in one Saturday morning and found my mother alone in her room, actively choking, with no staff around. I had to run out and find help myself. She also had significant bruising on her hands that was not explained. We documented everything and filed formal complaints. On the surface, the facility looks clean and modern when you walk in. But what happens behind the scenes, especially when family is not present, is deeply concerning. If you care about your loved one's safety, dignity, and proper care, I would strongly recommend looking elsewhere.

I personally witnessed her screaming at a CNA while on duty, which was very uncomfortable and unprofessional to observe. The way she spoke to staff was inappropriate and unacceptable in a healthcare setting. From what I heard, her behavior also appeared to be sexist and homophobic toward a young male CNA. This kind of conduct creates a hostile work environment and should be taken seriously. I believe her behavior and communication with staff need to be reviewed by management. The cna took care of my cousin and he was amazing. I feel bad for what he had went through.

If you are considering placing a loved one here, please read!…read more My mother was admitted to The Enclave in April 2022 for short-term rehabilitation after a hospital stay. At the time, she was living at home with daily care, fully capable of self-feeding, writing, shifting in bed, managing her wheelchair and even giving herself injections. What happened over the next 11 months at The Enclave was a steady and devastating decline, caused by neglect, poor oversight and what appears to be financial manipulation of Medicare/Medicaid benefits. Staff repeatedly insisted that my mother's decline was due to her "chronic condition" rather than their care. We now know that was not true; since leaving The Enclave, she has regained some strength and improved at another facility. Basic Care Neglected and Alarming Conduct by Staff * Staff ignored obvious daily needs: showering, grooming, nail care, phone accessibility; room was filthy with a sticky floor. Personal items were constantly vanishing. * Medication was mishandled, with refrigerated meds left out and refills left to the family to arrange. * Staff were unfamiliar with her catheter, delaying proper care for months. * Food was unnecessarily pureed into oblivion despite her ability to chew and swallow - they just did not want to be bothered to assist her eating real food. * A staff member referred to my mother as "the devil," hid her personal belongings and used her charger (and actually snapped at me when I moved it during a visit) - complaints to the caseworker about this treatment were met with ambivalence. Medical Negligence * First wound doctor exam came two months after admission. By November, she had multiple stage 4 wounds down to the bone. * Notes repeatedly claimed she used a spirometer (to prevent pneumonia), but she never once received it. Records looked like copy/paste entries from previous entries. * She endured constant infections, dehydration and pneumonia. Staff failed to ensure she ate or drank; food trays were left untouched without help. * In December-January, she became frail, unresponsive, and malnourished, yet there was no plan of care for feeding assistance. I thought she was going to die. Shocking Withholding of Hospital Care * Despite worsening wounds and repeated infections, she was not sent to the hospital until I traveled to NY and insisted they call an ambulance. * Records show a lung mass was first noted in August 2022 but only addressed months later. She was finally transferred to Greenwich Hospital in February 2023, where doctors were horrified at her condition and said she should have been admitted far earlier. Questionable Medicare/Medicaid Billing * On 10/31/22, right after disclosing that her wounds were too severe for her to go home, The Enclave reengaged her Medicare benefit for "urgent care" with no explanation to the family. This conveniently locked in three full months of Medicare billing (Nov-Jan), while they also collected her Medicaid spend-down (~$7,000). * When she needed placement after Greenwich Hospital, almost no rehab centers would accept her because The Enclave had exhausted her Medicare coverage. Final Impression The Enclave did not treat my mother with dignity or compassion. They ignored obvious care needs, allowed her to deteriorate, misrepresented treatments in the records and seemed more focused on extracting Medicare money than sending her to the hospital when she needed it. When I picked up her belongings after less than 24 hours in the hospital, her room was already cleared as if she had never existed. I guess they needed a bed for their next "patient." Just look at the before-and-after pictures of my mother. She entered capable and independent. She left frail, malnourished, with stage 4 wounds and permanent damage after less than 11 months. I also included a photo of ONE of her terrible wounds and her urine bag from the Enclave vs. the Greenwich hospital so you can see how dehydrated she was at the Enclave. This facility should be investigated. Don't be fooled by their lobby fountain, this is a house of horrors.

Benchmark helped us get a speedy move-in date, and did everything they could to keep our mother, who was in memory care, busy and feeling loved. We were delighted by the constant array of events that were planned for the residents. One of our favorites was the Brazilian jazz lunch that had a huge live band a buffet to fit the theme. Many people got up to dance and all the residents were smiling. We noticed that all the staffers went out of their way to talk to all residents in the halls when passing by - always sharing a witty line to help the residents feel a part of a larger community. We feel it takes a special understanding of this community to do a great job and it comes from the top down. The company does a fine job on all levels. Highly recommended! Thank you again to all at Benchmark for taking great care of our mother and giving our family peace of mind during a difficult time. As they say, it takes a village- and Benchmark is best village. My mother had no input in the selection due to her illness and because we told the rehab facility we were not interested in leaving her in NJ they decided to discharge her with little notice to our family. Dylan was extremely accommodating and met my sister and I on a Sunday. He arranged for someone from Sunrise to observe and document her condition at the NJ rehab and jumped hoops to help us move her quickly. Yes, there are a few unfavorable reviews we saw on the web but we checked the DPH licensing web and checked on things like service or health violations as part of our decision to move mom to BG. They bent a rule and let us move her on a holiday weekend since we had to get her in sooner when the NJ facility gave us two days notice of a discharge date of Friday before labor day. As my mother has improved, she has come to consider this her home with new friends. The staff have been kind and considerate of her recovery and care. They give her choices in her wardrobe selection and activities since she is wheelchair bound at this point. She loves her meals and feels happy, much happier as time passes, AJ, the ED has answered each and every question that we come up with, is very responsive and the receptionists are so helpful. We decided at the 11th hour of closing up her house to bring up her kitty after the first month who is also elderly and the staff has been wonderful with Timmy. I hope in considering choosing BG you will visit and check all sources, talk to staff and tour the facility. You will see for yourself that many factors go into a big decision with what is best for your parent or family member. We have found all the staff to be courteous and considerate and right down to the Director of Facilities, who responds quickly to complaints if any. The fact that Mom is close to myself, my two other siblings and we can see she is happy is a bonus. That and the friends I have made with other resident's family members. For our family it is a win win all the way around. And, then....let me tell you all about Bear........
