I worked at this property many years ago when it was under different ownership and hadn't visited the community since. I have always loved this property as it is a smaller community which always felt like a bed and breakfast. I can first hand say how compassionate the staff is at the community. From the moment you enter, from the concierge, to the dining staff, the housekeepers and maintenance team. Everyone is so happy and cheerful. always greets you with a smile on their face and it's a very welcoming community. The caregivers are prompt and accommodating with compassion as well. Harbor Chase took care of my Mother for the past year. The staff is excellent, from the Administrator's, Nursing, Nutrition and everyone else. I would say names, but I don't want to leave anyone out! Beautiful facility with so many activities to do and keep you family members occupied and happy. I would live there and I feel that says a lot. We have the same mother as Lois R. below and had quite a different experience. In fact Lois R.'s mother-in-law was at the same facility previously and they spoke highly of Harbor Chase from that experience. We were there 3-4 times every week. Lois R. visited once every other month. Our experience with Harbor Chase was excellent. The management and the staff were very responsive to our every need and request for mom. They were kind enough to allow mom to keep her 2 beloved cats with her so we were there to clean up and feed them every few days. We were very friendly with the staff and they knew us by name. They checked on mom every hour and they were cheerful and helpful. If any staff was not up to par they did not last long. Because it is Memory Care other residents would forget where their room was and wander into mom's room because they were lost. This was always dealt with quickly by the staff and the resident was helped to their own room. After meals they would gather in a large common area to watch TV. Some would fall asleep, some would visit with each other. It was a very organized and quiet time. If one staff member was visible there were always others right near bye. Mom loved her own room with her cats and her own cable TV. She had her hair done at the Salon, could go out on excursions if she wanted to, enjoyed Bingo and live music. Mom passed away naturally in her sleep at age 92 after over 2 years at Harbor Chase. We were so impressed that some staff even came in on their day off just to check on mom and left us loving notes of caring for her. We highly recommend Harbor Chase for their professionalism, cleanliness, beautiful decor, and the responsiveness of both management and staff! read more

This week I had the opportunity to visit with the residents of this wonderful place. I witnessed the dedicated and compassionate staff interact with them in both memory care and assisted living. Both my parents lived at HarborChase - my father in Memory Care and my mother in Assisted Living. Here are a few of the problems they encountered: (1) The family was not notified of a scabies outbreak in Memory Care. My father and other residents were treated without our knowledge or consent. (2) The Memory Care unit was sometimes unstaffed - residents were left completely unattended. On one such occasion, my father was injured by another resident. (3) My father's medications were not given according to doctor's orders. Management could not locate his medication log when we requested a copy for his doctor. (4) The air conditioner in my mother's room stopped working because the filter was completely clogged with dirt. A persistent cough, which began shortly after she moved in, went away after the air filter was changed. (5) There were live roaches in my mother's room. I requested a family meeting with the Executive Director to voice our concerns. Instead of addressing the care and safety issues, she gave us 45 days to "find another facility" for my parents. 22 days later, they moved to a new home. HarborChase kept their $3,500.00 deposit and the pre-paid rent - approximately $15,000.00. If I had checked with the State of Florida for complaints against HarborChase and taken the time to verify credentials of people who worked there, I would have avoided a terrible experience. But I didn't. I believed what I saw and heard during the tour - not realizing that a facility can be staged to look clean, well-staffed, and safe - with troublesome patients kept out of sight. HarborChase is a horrendous memory for us, but fortunately, my parents are safe and happy in their new home. read more

My mother passed away Sunday and has lived in Harbor Chase Memory Care for 3 years. I feel It is under staffed and the staff is in my experience very hatful to the Residents. Patents are left alone for long periods of time and family are told they are cared for and watched with care. More than once I found a male patient in my mothers room trying to help her out of bed or just standing and watching her. When I complained I was told nothing could be done to stop this man from wandering into someone's room, he was harmless. the care givers are rude and do not monitor patients with the level of care I expected. Memory care residents who do not understand what they are there for or why they are not getting help were often left to sit in front of a TV in groups with only one or two workers sitting and watching them. My mother was locked out of her room so she would have to participate in activities and was found by me slouched on the floor trying to get in her room to go lay in her bed and go to the bathroom. This tells me no one noticed she wandered away from the TV room. When confronted the staff always has some excuse. She fell and was not found until I am not sure how long. Sad and sick about her care because I am not The family member who gets to making her arrangements, and was not listened to. She is in a better place now. Do your homework before you choose this place. If it was an option I would give them no stars. read more

My father has recently passed away at 93+ He had been a resident in the memory care unit for 18 months and our family could not have more please about the care and attention of every single staff person from the director to the wonderful care staff. The facility is cheerful and the staff keeps the residents engaged. In the memory care unit there are daily activities through out the day to keep the residents as active as they are capable. There is a wonderful young woman, Kathy, who is always cleaning the facility and keeping it spotless. The director, Michael, jumps right in and gives pedicures and never asks anyone on the staff to anything that she is not willing to do herself. I never met an employee that was not smiling and helpful and willing to help. I do not believe my father would have received better care anywhere else. The head of patient care, Jackie, is very professional and gives attention to detail and is on top of care. The nurses are quite capable and equally as vigilant. The chef, Robert Taylor was Chef of the Year (but then he had been Sous Chef at the Naples Ritz Carlton prior to coming to Jacksonville) Candy in the business office is always smiling and a delight with whom to do business and Janann Holt, the director is as pastoral and caring as they come. (BTW she selected "Director of the Year in 2016) All in all, the facility is nice and is maintained well, the staff is capable and caring and I would not hesitate to place someone else in my family here if the need were to arise.
