My mother, who has been living in an assisted living apartment at Terracina Grand for the past 6…read moreyears moved to The Gardens at Terracina Health and Rehabilitation after she had a stroke. We spent a long time looking for the right place for her and were happy to have found this one. My Mom is 91 years old and has had frequent hospitalizations in the past 10 years and we are so grateful for the care and patience with which she has been treated for this extended period of time. She has several physical therapists and occupational therapists in both the Gardens as well as her apartment at Terracina Grand and they always work with her to build up her strength and get back to a normal way of life. She has been in a wheelchair ever since her stroke but there are always enough CNAs and Nurses who monitor her medication and get her to and from the bathroom, as well as work with her when she has problems swallowing. The food is very good particularly at the Gardens and even though it sometimes must be softened so that she can easily eat it, she is usually able to rehabilitate herself with the help of the wonderful people who attend her there. We couldn't be happier than to have a place such as The Gardens for Mom to recuperate in until she is strong enough to go back to a more independent lifestyle which is available at Terracina Grand.

At almost $8000 a month a room shouldn't have an inch of dust on the shelves. Simple nursing 101 care being neglected. Residents personal item walking away from their room. ongoing issues with supplies provided for a resident seemingly disappearing faster than a resident could use. The activities program is wonderful and there's live music almost daily. This is the only thing that is truly "Platinum Service" at Villa! My concerns were with the assessment skills of the nursing staff and the uncaring, rude demeanor of several caregivers. They remain employed despite numerous complaints from family members! Nursing Administration was mostly defensive when issues were reported. The dietary director was initially reluctant to accommodate my husband's changing needs and then when changes were made, they were inconsistent. We, as advocates, trust that our loved ones are being cared for by knowledgeable people who will recognize ACUTE changes in a resident's physical and/or mental status and take proper action in a timely fashion. The facility is beautiful but don't base a decision on this alone. Also, private duty care is almost a requirement at Villa as your loved one's disease progresses.

Due to COVID, we were not allowed into the facility or to see her for her two week quarantine-totally understandable. We stayed in touch with her by her iPhone. I think I spoke with the women at the desk every day while they patiently answered my questions. Once quarantine was over, she was assigned an occupational therapist and a physical therapist. She started going to the gym and joined in a sort of group social therapy. Before she came home, the therapist, a social worker, & the Home Nurse Association reached out to me to arrange for any extra care she might need after care. All the way home she spoke happily of the people she met, who helped her, what they did. It was an unexpected experience made manageable by the people at ManorCare.

Unjustified Retention of Refundable Deposit We paid a fully refundable security deposit of $X as part of the initial application and holding process. Due to unforeseen changes, we withdrew our application well within the contractual period that guaranteed a full refund, as stipulated in the initial agreement signed with the facility. Despite documented contractual obligations and our formal, written request for the refund, the management has unilaterally and without legal justification refused to process the return of these funds. This is not only a breach of contract but demonstrates a profound lack of financial transparency and ethical business practice. Consistent and Unacceptable Non-Responsiveness The process of attempting to resolve this matter has been severely hampered by the management's persistent and deliberate non-responsiveness. Over the past six weeks, we have made more than ten documented attempts to contact the administrative office via phone, direct email, and certified mail. The attempts to reach the Director or any relevant financial officer have been met with silence, unanswered calls, and unreturned emails. The complete lack of communication on a serious financial matter is unacceptable. This pattern of evasion regarding a simple, contractual refund raises serious concerns about how this management team would handle critical, time-sensitive issues concerning the health and well-being of residents. In the context of long-term care, where responsive and reliable communication is paramount, this administrative failure is a severe red flag. Conclusion and Recommendation Our experience with the management has been deeply disappointing and highly unprofessional. they killed my mother-in-law, we found the place under her insurance plan thinking it was a good…read moreplace but little did we know no one knew what they were doing there. we would visit her everyday we get there she is so dirty food all over. we would asked why is shy in the kind of care most of the nurses did not speak english didn/t understand they would always say hu hu . well we would have to start to shower her smelled the dirtyness odor. was aweful they never had soap or shampoo and no kind of lotion we provited everthing she needed. they also never had clean towels. they wouldnt feed her she could not see there was so much neglect that when we left all we could do is pray!!!!!!!!!!!! forgot to say the facility was full of coacroaches disgusting all over the rooms beds everywhere. glad it finally got shut down and the nurses should have had there licences revocked. please hope you had sympathy for the ones that passed away at heritage health care facility. the lopez family

There isn't a no star option…read more This is a medicaid convalescent center and nothing more. The treatment I received was "below standard" according to my Home Health professional. There were limited rehab beds available upon my discharge from the hospital and I was placed here by default. Post surgery admittance with large wound care necessary. I arrived with an order for it to be changed 2xday. They chose to change the bandage once a day, and one time didn't do it for 28 hours. One night I got up and had fluids running down my leg. They puddled on the floor. An aide came in, saw the puddle, and walked out. I mopped it up with a towel. My arrival was around 7:30 in the evening. Other than my IV antibiotic, I was told my meds would not be available until the morning. The bed was an air bed that didn't inflate. They said they'd fix it in the morning. They did end up moving me to another bed that was not much better. The bathroom was filthy, even a dirty toilet. Under the soap dispenser, the wall had extensive (old) streaks from soap and water. I asked the aid for a commode but she didn't understand. Once it came, I had already soiled my shorts. The commode had rusty legs that shedding rust on the floor. The call buttons are vintage. They sit on your bed. I rolled on it and an aide appeared. I apologized for the mistake. When I did it a second time, she came in and threw the call button, tethered with the bed control, on the floor and left. The next morning I told my wound nurse I was uncomfortable there. She said she understood and they "do the best with what we've got". She had an administrator come see me who agreed they would move me to another room and asked to give them a few days. They really made an effort to clean the new room. The housekeeping director even inspected the work and called the housekeeper back in and pointed out areas needing attention. I question how things can get so dirty and wonder what would have happened otherwise. The new room overlooked a nice courtyard I expected I'd visit. Shortly after being moved in, a parade of wheelchair, walker, and cane based residents showed up and started smoking cigarettes. It was a smoking lounge! I found out later they had designated smoking hours. At a rehab! I was being treated for infection. The place is truly filthy. Some nurses didn't sterilize my picc line before injections. And didn't flush it afterward. One night without my call being answered, waiting for a picc line flush, I walked the whole first floor calling "anyone here? Any nurses?" The WHOLE floor, including going in open offices, the kitchen, and employee lounge. Nothing until returning to my room and seeing 2 staff pop out of a door and laugh at me. I asked them if this way funny and they returned behind closed doors. The nurse then appeared from a door at the other end of the hall with an aid in tow. He said he was doing rounds. All the walking caused my foot brace to come apart. It's to be worn all the time and they had no tools or ability to repair it. Their doctor came to see me Sunday for the first time since arriving Tuesday. He knew I wanted to leave and said he'd sign discharge papers. They weren't able to arrange home services until Tuesday, and I went home. They didn't let Home Health know I had antibiotics being delivered for a 2:00 pm injection so Home Health didn't come until Wednesday, a visit the day after is standard. Fortunately, I had already administered my own IV and was able to do it myself. Not that food matters much to me, the menu and what was served only matched once, and the quality is not good. They had no cream for coffee 3 times during my 7 day stay. They ran out of milk. My wound was worse when I left than when I arrived. I changed my own dressing upon arriving home. After home health care and my own efforts, it's improved substantially. Now I'm anxiously awaiting placement in a suitable rehab. After my first home visit, kudos to Family Home Health Services for taking the initiative to place me in a suitable facility. Rosie and Samantha are truly my life savers. There are some staff worth praising. Weekend day nurse Andrea and wound care Victoria - when she did the dressings. Eboni got me out! This place is truly "bad". DON'T DO IT.

Genny has been communicative not only with my parents, but with me. Recently, she went above and beyond before, during and after Hurricane Ian. Genny was truly one of my lifelines throughout the devastating event. During the long power outage and lack of cell service in the area, Genny managed a way to remain in contact with me. She always responded to my texts/calls--both night and day. She continued to reach out with sincere concern and compassion for my parents and their situation(even though she herself was trying to navigate the aftermath of the hurricane on behalf of her own family and business). I am forever grateful for my parents' safety. Additionally, I am grateful for people like Genny who remained connected with me during an extremely stressful and frightening time. Thankfully, Genny, her family and her employees were/are safe as well. :) To say "thank you" is an understatement... ;)

Manorcare lely palms…read more 10/3/18 Upon arrival at 6pm, room had a little gift welcoming package, her name on the door and I thought all was nice. 10/4/18 pt's RX list was on her bedroom table. I liked that! Pt. complains of no service when you press the nurse button. 10/5/18 - pt tells me there is a Haitian woman there that is not very nice to mom. Very uncaring. Mom uncomfortable with this. PT and OT is very good. Observance of not all patients are treated equal. Haitians Health Aides nice to others while not nice to the other patients. 10/6 - pt talks about the one not nice Haitian lady. They also don't come when you press the "nurse" button for toileting. Observed PT and thought was very good. Pt is stressed out about the mean Haitian lady. Pt has to #2 and no one comes. 10/7/18 - 2am-ish, slight heart attack feeling.. press nurse button, after a while, a woman walks in, then walks out. Later on pt yells for help someone comes in stands there and walks out. Quickly a man arrives and they put a machine on pt. According to front desk admin, pt went by ER around 7am-ish. So my mother in heart attack mode for 5 hrs. front desk lady looked it up and it was Yvanette the aid that walked in/out of room and did nothing. minor complaint - food not good. ****admitted to NCH baker hospital downtown and not to return to this place again****. 10/8 I talked with Maribel Vazquez (social worker) I told her of issue, she talked with me on phone at length. Concerned. She said the person that I should speak with is Duane (admin of bldg.) or Ian (clinical dr of nursing) # 239-330-3108. She said she would forward my information to him and have him call me back. 10/11/18 - received a letter from the Admissions Director Cristina Cortina saying that we did not sign the admissions agreement. Very nice letter asking me to fill it out. Boy, when it comes to billing, all of a sudden you get attention!! 10/13/18 11:22am I emailed to Maribel Vazquez. Per our conversation 10/8, you were going to have Duane or Ian contact me. To date, have not heard from anyone. Really think this is more for them to reach out to me, then myself to them. Kind of insensitive. They just do not care. I hope that Yvanette was reprimanded and that this didn't go unnoticed for the next senior citizen in her care. T/Maribel Vazquez/lmsgr/still did not hear from Duane or Ian. As of this writing 10/18/18 Thursday no response from anyone. I feel it is only right that they contact me, rather me contact them. I learned from an outside source to complain, contact Ombudsman State Central Advocate for Client Complaint FL Longterm care Ombudsman Volunteer line 1-888-31-0404.

My mother went into the rehab after getting minor surgery. She ended up staying because of…read morecontinual health issues. She got transferred into what eventually became hospice for her. The in-between care was absolutely horrible. They kept her in the bed pan for hours, they were nasty and abrupt with her and treated her with no tenderness what-so-ever when she was becoming incapacitated. To add insult with injury, after she died with them giving her no dignity; her phone and $200 were missing. I will always regret sending her there.
