We placed my mother, who has dementia, in traditional assisted living in April, 2020, during Covid. Before welcoming her with open arms, they had her room all set up with her personal belongings so she would feel at home when she arrived. Our deep gratitude goes to Nikki Vawter for this. She is an absolute Angel! The nursing and caregiver staff are absolutely amazing, warm and giving! During Covid she was kept safe and continually tested. Also during Covid, Nikki was able to FaceTime with mom's children as we were not able to visit at the time. Mom is so happy and content. She is a life long faithful Catholic and a social butterfly! She is able to go to the beautiful church any time she wants, as well as socialize with the other residents. And she loves the numerous activities! We very highly recommend this facility for any senior needs, from independent living to nursing home care!

In striking up conversations with them, I heard plenty of unsolicited positives--lots of on-site activities and off-campus trips for residents, a beautiful wellness center with a pool, restaurant-quality food, well-maintained grounds and flowers, good employee benefits and great co-workers, and very helpful staff that put residents' care first. If I were looking for independent or assisted living, I'd sell my home move there in a heartbeat! Kudos to St. Leonard's entire team for making everyone feel so welcomed.

    Do NOT Recommend Bethany if you are looking for actual Rehab and Recovery for a Senior…read more My Elderly Mother went there after a horrific fall and excellent treatment at MVH. Very disappointed in the total lack of organization and management or the occupational and physical therapy. She was supposed to have two sessions of Therapy a day. They usually did not happen and we don't believe a single one was for the proper time. The therapist would literally walk her half way down the hall for 5 minutes and return her to her room. It's almost like they want the senior to NOT recover so that they could get them as a full time permanent resident customer. No organization. No management. The therapy manager never returned my calls. They insist you declare on their markerboard if the senior has outside medical appointments so that they could schedule Therapy around it. We did days in advance. And just as I am wheeling my Mom OUT of the room for her declared medical appointment, BOTH the Occupational and the Physical Therapist show up and feign surprise that they won't be able to do any therapy that day. Pitiful. Stay away from Bethany for Rehab!!! Unless you don't want actual rehab for your patient. My Mom actually fully recovered ONLY because I checked her out early from Bethany and arranged for In Home Rehab thru Miami Valley Hospital. Also - My Mom's room was very worn and filthy. I asked multiple times if they could clean up the "accident" stains from the side chair. It only got better when a week later I cleaned it up myself. Pitiful.

Unless you hate your relative, make certain that you don't send them to this hole in the wall. The marketing brochures that they give to prospective residents make this place look like a five star. Go to Medicare.gov and you'll see that the facility is a one star, which is the lowest rating available, If I could give it a rating, it would be in the negative numbers, I.e. -5

She was wheelchair bound and too many times had to wait over an hour for bathroom assistance. When she messed herself as a result she was treated very bad because the aids had to clean her. The state law or rule (not sure which) states "every 2 hours check and change". This was never done. I've reported them to the ombudsman's office more than once. My wife has recently passed. When I collected her belongings today her laptop was missing. In the past she has had her clothing stolen, her jewelry stolen and of course her money (what litte she had). If you have someone that needs Shilled Nursing please avoid this place for their sake. Over the years my wife was in four different homes for various reasons and this was by far the absolute worst. I wish the star ratings went into the minus category. I almost feel like I abused her putting her in there.

    If we could have a do-over... We would have contacted Care Patrol, a free matchmaker service that…read morehelps you find a good long term care placement for your loved one. Instead, in crisis, we took a friend's advice and went to Elmcroft. Buyer Beware! If they ask you to write a "deposit" $2,000 check which they say they will never cash, well, actually, they will cash it. It is an entrance *fee.* And, if they say they'll change your loved one's bedding nightly if needed, prepare to be the person changing that bedding and doing that laundry yourself, despite paying close to $5,000 per month. Every day, my honey slept in a foul smelling bed unless I got there and checked his bedding. They had promised from the beginning that no matter how frequently he had incontinence, they would check his bedding and would make sure he didn't sleep in a bad-smelling bed. With his brain injuries, the ammonia smell of urine set him off and made him have mania. And then they treated him like he was a Bad Boy. They wore down his self esteem by not letting him participate in activities because of his incontinence. They were not equipped to deal with his problems and made me feel nobody else would want him either. They could have referred me to Care Patrol or to another facility, but instead kept telling me what a 'favor' they were doing me by 'letting' him continue to live at Elmcroft, since *nobody* else would take him. At mealtime, Elmcroft likes to "park" the elderly crowd at tables, then leave them there until they get around to delivering the food. Every day, my honey waited 20-30 minutes to be fed. After sleeping in a putrid bed and waiting for long periods to be fed (while sitting with residents who were not very interactive), it got so - at each meal - he would have a fit. He was a Bad Bad Boy. (He said the Emperor had no clothes!) (But they did not tell me he was having fits 3x per day until they had decided to boot him out. Talk about poor communication. They sometimes said, "It (his outbursts) happens 'all the time,' " but then did not mention it the next day or for another week, so I thought it got better and worse and better - and did not know there was a persistent, ongoing problem to solve, nor did I know they were not working to solve the problem. They kept doing the same thing - bad sheets, long waits for food, etc. - and expected him to change???) Do they treat their workers poorly? Why do they have such high turnover? And why were workers standing around in the hallways chatting, again and again, while I was laundering my honey's bedding? Care Patrol (which cost us nothing) - specifically Deana Barone - helped us find Symphony, five minutes away, a much newer facility which costs somewhat more but looks like a much improved situation for us. More attentive care. More accommodations. (Care Patrol's Deana Barone may be reached at 1-513-515-7315. No contract. No fee.) Elmcroft broke our hearts. It felt like home, except for the part about my honey feeling like he was a Bad Boy so much of the time. :-(

    This place is TERRIBLE! I don't even know where to begin. They are not a 5 star please do not…read morebelieve that. My grandfather was there for 6 months and it was the most horrific experience My family has ever been through. They are EXTREMELY short staffed. They had 2 aides to 30+ plus residents many times, they use agency staff which are always different aides and nurses , and they have no clue what's going on. We had to BEG for my grandpa to get showered. I called the D.O.N on a weekly basis to ask when he would get his shower and nothing ever changed. My mother started giving him his bathes along with all his personal care. My family was with him 12 hours a day and he still did not receive proper care . He would be left in soiled diapers, we would put on call light and would wait 20+ min and finally just did it ourselves. This place is terrible. My grandpa fell one night they never called us we arrived at 6 am and he was sitting in the hall naked bleeding profusely like it was no big deal. He needed stitches and had pneumonia when he arrived at hospital . Residents are neglected here. If you plan on putting your loved one here and you can't monitor their care 24/7 then don't expect a long term stay. There was one nurse Angie 2 aides one very short hair and the other black hair with tattoos that my papa loved and one dietary aide we really loved so much, the rest of the staff is horrible. My grandfather past away in the middle of the night a couple weeks ago which has been traumatizing for our family. We saw him at 7 pm and at 3 am they called and said he passed away. Only explanation we got was he had been throwing up, again no phone call to report the illness. The only thing we have come to terms of is that they must of had him laying flat on his back and he aspirated when vomiting. Since he passed we did not receive one phone call from anyone there not the administrator, D.O.N, no one to say sorry for your loss or check on us, goes to show these people are there for money. They could care less about the residents .. there were so many errors. We caught a nurse red handed giving him whole pills when there was an order for crushed only, she admitted to have been doing it for days and he had pneumonia that week he had the fall which could have easily been from aspirating the pills .. Same nurse was back in his room next day with an attitude . My mother then would not allow agency in his room at all due to all the errors. Please just do you research . Yes the building is clean and remodeled but everything else is terrible. I know things can sometimes be unbearable or hard to keep sick loved ones at home, trust me This was the hardest decision our family ever had to make and my mother stayed with him 8 hours every single day he was there, but please research thoroughly before making your choice if you have to go this route and check on your loved ones
