I completed the documentation online but the office says that it was never received due to them changing to a new phone system, etc. I did not know that the documents were not received until I received a call the day of my appointment at 9am to confirm my appointment at 3pm. I was told that there was a cancellation at 10am and asked could i come in earlier and I could not because I was on my way to work. She stated that was fine and that she would see me at 2:30pm. I told her that I cannot come at 2:30pm because I am not scheduled to leave work until a certain time. She told me that I needed to come in to complete the new patient paperwork but I told her that I had already completed it. I toldher that I could not leave work early to get to the appointment at 2:30pm but that I would get there by my appointment time. If there were issues with the email or phone that prevented the office from sending or receiving my paperwork then I could have stopped into the office in the days leading up to my appointment or we could have discussed an alternative method to obtain the documents since I live less than 2 miles from the office. My appointment was at 3pm. I arrived at the office at 2:55pm and obtained the clipboard with the requested information and presented my copy of ID and insurance. I was called back and began my pre-surgical service. The dental assistance was amazing. The procedure took 35-40 minutes. Following the procedure, I was given instructions for my next appointment which was scheduled for April 25th. I went to pick up my purse and my medical chart was on the computer screen and at the bottom of the chart was a note in red stating that "Patient is very rude and overtalks you". I was offended. I asked to speak to the office manager who was not available. The dental assistant stated that they office manager would call me the next day. The phone call came 3 days later and I was told that there were still some ongoing phone issues. During the conversation, I explained to her that at no point was I rude to the office staff but that I simply responded to her request to come in early by indicating that I could not. I was told that maybe it was my tone and again I reiterated that I did not use any tone with her. I literally stated that I scheduled my appointment at 3pm because that's the time that works for me and that I cannot come at 2:30pm but I would get there as soon as possible. I completely disagree that my response was rude and I do not appreciate this being entered into my medical record as it pre-judges me and anyone who views my chart will meet me with preconceived notions. She told me that she would speak to her staff then call me back the following Tuesday. She did not call instead on April 15, 2024, office staff called me to confirm my appointment for April 25 and I told her that I wanted to speak with the office manager first. Today, the office manager called me and said that the clerk told her that is how she perceived our conversation and I told her that just because that is how she perceived it does not make it accurate. I asked that the note be removed from my medical record and she stated that only office staff can see it and that was still not ok with me. She states it could be filtered. I stated that I refuse to receive medical procedures from an office that treats their patients in this manner. I also stated that I was going to contact HIPPA. According to HIPPA, the following should not go into a person's chart: Subjective opinions, Speculations, Blame of others or self-doubt By requesting and not having this inaccurate information placed in my chart is a violation of my HIPPA rights. I requested cancellation of my appointment and called my primary dentist for a new referral. I refuse to spend my money for any service especially a medical/dental procedure where patients are not treated with respect and dignity. My character and integrity were called into question and that is unprofessional. It's unfortunate because the Periodontist seemed great and I was looking forward to working with him.
