He thoroughly arranged the necessary tests, including MRI, biopsy, and PSMA PET/CT, which led to a clear and timely diagnosis. Because I was concerned about the risk of infection, I specifically requested a transperineal (TP) biopsy, and Dr. Shinghal was very receptive and performed it without issue. I truly appreciated how he listened to my concerns and accommodated my request. After the cancer was confirmed, he took the time to clearly explain the available treatment options, discussing both the benefits and potential concerns of each. This helped me make an informed and confident decision about my treatment. His recommendation of Dr. Yao for surgery was excellent and made a meaningful difference in my overall care and outcome. I was admitted with a kidney stone that left me literally kneeling by the hospital bed in agony when he first came in to see me. From the moment he walked in, his kind and calm demeanor made a huge difference. He didn't just rush through -- he took the time to explain what was happening, what my options were, and how we could relieve the pain and get me on the path to healing. He treated me like a person, not just a patient in crisis. His expertise was clear, but what stood out most was his compassion. He reassured me in a moment when I felt completely overwhelmed, and he guided me through the procedure with such steady confidence that I felt safe placing my trust in him. Thanks to his care, I am able to move from a place of unbearable pain to healing and stability. Dr. Rosenberg gets 2 stars because he didn't keep me waiting too long past my appointment time, and…read morehe did spend more than 10 minutes with me. Unfortunately, it didn't do any good. He treated me like a moron, didn't listen, and didn't solve my problem. So he only gets 2 stars. I was born with my right ureter through a muscle, which caused many kidney infections as a child. The local doctor misdiagnosed this as backwash. Luckily, my mom persevered and got a correct diagnosis and I was successfully operated on when I was 6. Last year (at 28), I had 3 kidney infections within 6 months. I thought this was a problem, so I starting the slow path through the insurance and doctoral hoops until I finally got to a urologist, who had a test done, which revealed that (surprise surprise) my right kidney had reduced functioning. He said this was likely due to scar tissue from the childhood operation, and could be fixed easily enough. Unfortunately, I moved to SF before we could get on with the fixing. The old urologist recommended Dr. Rosenberg here in SF, and I immediately made an appointment in hopes that we could start out where I had left off with other urologist. I brought with me my chart from the other urologist, including the results of the kidney function test. I wrote down my whole history in their "new patient" forms, and then told it all to Dr. Rosenberg. He pretended to listen, and then proceeded with the generally uncomfortable process of manually exploring my urinary tract. Below is a small excerpt of the frustrating conversation that ensued: Dr: It's probably backwash. Me: Actually, that is what I was misdiagnosed with as a child. Dr: Hmm. So, where's your scar? Me: On my lower abdomen. (Dr. examines scar on lower abdomen) Dr: Well, that is not where the scar should be, if the operation was what you said. It should be up here on your side. (Dr. proceeds to physically turn me and search for a scar on my side. Like there really might be a scar there that I had not noticed in the past quarter century. Like maybe I got a long thin scar on my lower abdomen through some other childhood scrape, like, I dunno, falling off a bike, and that distracted me from the REAL surgery scar on my side, unnoticed by me until the astute Dr. pointed it out.) Me: Um, no, I am sure I don't have a scar on my side. I don't know exactly how they did the operation, but I know it was my ureter through a muscle. Dr. (giving up on searching for the elusive side scar, but with no explanation as to the actual scar on my lower abdomen): Couldn't have been. It's probably backwash. If you have backwash, go to Dr. Rosenberg. For any other problem and/or for an actual doagnosis, go elsewhere. I am.

Warning: Long review about Male infertility, IVF, Testosterone, Urologist…read more When my wife and I faced fertility challenges, her doctor recommended a semen analysis for me. The results were shocking: I had azoospermia--zero sperm in my ejaculate. I was devastated. Those were the darkest, most painful days of my life, and even now, the memory is haunting. I began searching for a urologist and I am immensely grateful to have found Dr. Karpman. As some reviews mention, he can be very straightforward and less emotionally soft at first--but to me, that was irrelevant. What truly matters is expertise, and that is where Dr. Karpman shines. Some reviews also note long wait times, but in my opinion, that is minor compared to finding a doctor who truly knows what they are doing. Dr. Karpman conducted another semen analysis, blood tests, and confirmed my azoospermia. He diagnosed it as non-obstructive based on a physical examination alone. His expertise became evident when he suggested an MRI of my pituitary gland after reviewing my hormone levels. The scan revealed an abnormality that might have been overlooked elsewhere. Unlike some doctors who may have immediately suggested surgical sperm retrieval, Dr. Karpman recommended medications to stimulate my pituitary gland. After three months on the medication, my semen analysis showed a few thousand non-motile sperm. While modest, it was encouraging--there was hope. Six months later, my sperm count had improved enough for IVF. I continued the treatment for several years as my wife and I navigated the challenging IVF and ICSI process. Throughout, I consistently had sufficient sperm for every egg retrieved. After multiple IVF attempts, chemical pregnancies, and even miscarriage, we finally achieved our dream. Today, I am holding my 6.5-month-old beautiful baby, and words cannot express my gratitude to Dr. Karpman. I still remember the day I feared I would never become a father. Never say never. I am not a religious person, but Dr. Karpman's treatment feels like a divine blessing. I sincerely wish him all the best and hope that anyone reading this who faces similar struggles will consider consulting him.
