From the outset, the facility had a pervasive odor of urine and uncleanliness, comparable to a soiled litter box. This was not an isolated incident; I visited daily and consistently encountered this issue, which I have not experienced at other facilities. Due to my father's insurance and financial limitations, our options were restricted. In hindsight, I regret not choosing the alternative facility. Communication with staff was extremely difficult. Phone calls often went unanswered, and when they were, staff were unable to provide basic information. Despite multiple requests, I was never able to speak with a physician regarding my father's care. I was repeatedly told they were "too busy." The social services team was disorganized and unresponsive. They frequently confused patients and failed to follow up on critical matters. On one occasion, despite confirming with three staff members that my father would be ready for a medical appointment, I arrived to find him heavily sedated and unprepared. According to the facility's own records, my father was not bathed for the entire month of January. This was discovered by his state caseworker. I recommend that all families request and review their loved one's records. Many of my father's labeled clothing items went missing. He was often left in soiled clothing for extended periods--once for three consecutive days. He developed unexplained bruises, and staff admitted he had not been checked on for hours prior to reported falls. He required supervision during meals, yet I frequently found him alone, staring at his food. Staff were often seen using their phones and socializing while call lights went unanswered. The facility experienced repeated COVID outbreaks, yet cleanliness and ventilation appeared neglected. On the day of his discharge to an adult family home, my father was so heavily sedated he could not speak. As his Power of Attorney, I had explicitly instructed that no medications be administered without my consent. This directive was ignored, and staff were unable to tell me what medications had been given. Upon arrival at the new facility, staff immediately noted hygiene concerns, including multiple sores likely caused by improper care. This facility requires a thorough investigation by state authorities--particularly regarding hygiene, staffing, and medical oversight. I am relieved my father is now in a safe, respectful environment where he is treated with dignity. To the management of Life Care Center: I am not interested in a generic response. I have attempted to raise these concerns through proper channels without resolution.

He was recently accepted into a school that specializes in teaching Twice-Exceptional (2E) students. For families considering support for a child with learning differences, here are three things I wish I had known earlier: 1) Dr. Cindy Dupuy is truly the best. She changed the trajectory of our son's life. Before working with her, he was struggling, behind grade level, and had to repeat 2nd grade. Now, he's thriving academically and emotionally, working above grade level, and attending a gifted school tailored for kids like him. 2) Don't wait. Dr. Dupuy has a long waitlist, so get on it as soon as possible. 3) Be careful about settling. We made the mistake of choosing other specialists simply because they were available or covered by insurance. It cost us time, money, and progress. If your child needs immediate help, that's understandable--but still get on Dr. Dupuy's waitlist. She is worth the wait. She will ABSOLUTELY change your child's life for the better.

Our daughter had three half days of testing and we received an incredibly thorough report on our daughters' learning profile including her strengths and weaknesses. Cindy met with the teachers about accommodations that would increase our daughter's confidence and help her succeed. Our daughter attends Explanations one day a week and works with Chelsea to refine her executive function skills. We are thrilled to see how much progress our daughter has made and how she is overcoming many of the challenges thrown her way by having a diagnosis of ADHD. Cindy is upbeat, encouraging, and she has put a positive spin on an ADHD diagnosis; in other words, she helps our daughter feel good about herself. I can't say enough good things about Cindy Dupuy and our experience with her and her staff. She has had a tremendous positive impact on our lives. Although the wait to see Cindy is long, and although the cost is very high, this is definitely an example of getting what you pay for. I wish that every child with ADHD, dyslexia, or other learning challenge, was fortunate enough to have the opportunity to work with Dr. Cindy Dupuy.

In fact, we even got pushed out later as we were told there would be a therapist who would have flexibility in their schedules. The day of the appointment I received a call from Julia in the morning to prepared us for the appointment. An hour before our visit I got another call from Julia saying our insurance was inactive. I immediately got on the phone and called my sons insurance and made sure to get things all squared away. His insurance was completely active. I am mediately called Julia after confirming my sons insurance was active, and no response from her. We arrive for appointment on time only to be denied care. The last few months was a complete waste of time as we waited for an appointment slot. I did everything in my power to correct the situation with such short notice. Only to have my son and I turned away in person. A phone call would've sufficed. Or even a response to my call. I find this program to be extremely unprofessional as well as Julia's lack of respect and communication for us as a potential patient. If you are looking for therapy for your child, Julia, is not the one to assist you properly.

This facility was completely unprepared to release my 91-year-old mother an entire three (3) days…read morefollowing medicare notice of non-coverage; which was issued 3 weeks after her hip replacement surgery. Because they stated the doctor "left for vacation to Mexico," they could not provide me her medications, a list of her medications, and none of her prescriptions had been called into the pharmacy. Further, they did not have a cheap aluminum walker to send her home with. Because the head nurse had expressed she was confident she would be kept on after appeal, she left me scrambling to fly from out of state the following day after rejection. This all took place on a FRIDAY evening, with no options but to go the weekend without meds or a walker. I managed to arrive the following day, but the couldn't handle a basic discharge that is part of what they do every single day as a business. They SHOULD have kept my mother over the weekend at no charge to her to make up for their MAJOR blunder (instead they estimated about $500 per night out of pocket). When I found my mother waiting for me, they had a quarantine sign on her door but my mother wasn't sick. The nurse told me the sign had to do with her roommate who had left a couple days prior, so my other said she was not even being attended to. I would NOT recommend this facility if you're looking for a place your loved one will be treated like an actual human being, worthy of respect, and who will help with any sort of transition after discharge.

I have been going here for six years and I hope I never have to go anywhere else. I have very complex health needs and I always know I will be treated kindly and with respect by everybody here -- doctors, nurses, staff, lab technicians, everybody. I feel like my entire care team here and at the rest of UW are connected and can help in treating me holistically, not just looking at numbers and stats.

I came in for my wellness check and…read moreto establish care and it was very basic which is fine, they do only what is the on the list for your age and gender but the reason for the rating is a couple of things. Do NOT ever let them record you for their notes because those notes and imformation can be seen by the billing department so not sure how they can violate Hepa but everyone does. Also, if the doctor asks you a question or notices something they just randomly mark it down as a diagnosis and then they bill you more as complex. For example, if you have acne but you came in for another reason and the doctor asks if you have acne and you say - I don't want to be treated for it - literally you say that, they can then write down a diagnosis of acne in your chart and charge you more for the visit. I don't think if the doctor notices something like that which you don't want to be treated for that they have the right to diagnose you without telling you. Make sure if you get asked to tell them that you don't want a "diagnosis". I feel like they are looking for diagnoses to make you complex case. I do not take any medications and have no chronic conditions but somehow UW Med says I am "complex" but no other doctor's practice has ever said that. Be very aware of what you say and what they write down is my recommendation.

In short Other than in-patient acute rehab, this is…read moredefinitely the best way around here to make big gains when you've had a physical setback. In long 6 months ago, my lovely bride had a massive stroke. She's lucky to be here, but her mobility is still heavily impaired. She spent a month in IPR, which is available at only 5 places in the state. Coincidentally enough, there's a brand-new one a mile from our house, and she made a bunch of progress there. After that, they referred her for home health PT, OT, and speech therapy. She went from 15 sessions a week to about 4, and her progress slowed down. We found out about Swedish's program and signed her up. It took a couple months to get in, but she's off and running now. They help her a lot! She gets stretched some days in PT, some days she does strength and balance exercises. In OT, she does coordination exercises and works on getting control back over her left arm. They push her to the point of exhaustion and frustration regularly, but she keeps coming back for more. It's easier now than it was before the incident to frustrate and tire her, anyway, and she needs tens of thousands of reps to get her arm and leg back to being more functional. She has worked with different therapists depending on when they have appointments available. Everybody is smart and professional and motivating. Nobody can match the positivity and encouragement of the PTs and OTs she had in IPR, but those places will tend to draw in good talent. We do have a copay here, and home health was fully covered. It's worth the cost. Scheduling has to be done manually over the phone. Half of the desk staff is awesome, positive, and proactive. The other half is a mixed bag--y'know, the kind of people who think they're surrounded by idiots and never get enough credit. But on their good days, even they can be super helpful and even kinda pleasant. I'm glad we got in here, even if I'm not glad we needed it.

Dr. Cahn saw me sitting in the waiting room and insisted I sit with him. In fact, I found myself spewing all that was going - for over 45 mins. I left that day with some referrals, potential answers, a link to an article and a feeling that I may just figure out what is going on. Dr Cahn's priority is his patients. He's got the kind of heart that you can only hope for in a family doc. And his PA and NP - the same. Both are incredible. His staff, family, even Lucy (the amazingly gentle and therapeutic office golden doodle) make an appt with him a welcomed visit to get better. I couldn't say enough about the best doctor I've ever had.

And I add to that: He is easy to talk with; he listens and is interested to hear about and consider what I, as a patient, feel would work for me and my body. Dr. Cahn's office staff is also friendly, helpful and genuine. I am overjoyed that such a wonderful family doctor works six blocks away from me.
