Beware Richens Eye Center BILLING overcharge practice:…read more MODERATE ISSUE: Richens initially tried to get me to pay more than double what my Medicare Advantage insurance would have approved & paid for an eye exam. When I called them out on that, I found they hadn't even submitted a claim to my insurance). Their billing representative ("Dar") dismissed it as trivial, "We all make mistakes." Well OK, but after that, it gets worse... SEVERE ISSUE: When my insurance processed the claim, I discovered Richens "UP-CODED" the claim... 1. ...Billing for a SPECIALIST exam (code 99204) as a MEDICAL claim. instead of... 2. ...Billing for a ROUTINE eye exam (coded 92004 or S0620) as a VISION claim. In so doing, they OVER-CHARGED my Medicare Advantage, and left me with a $20 co-pay. This is contrary to what I made abundantly clear both when making my appointment, and again on arrival - I came for the "ROUTINE exam with $0 co-pay." I had no eye problems - only the need for a new lenses & frames. When I again confronted Richens, this time "Dar" justified the up-coding by falsely and repeatedly (6 times) characterizing both the Richens Sunset office, and Dr. Jed Lister, as an "OPHTHALMOLOGIST" and a "SPECIALIST." Richens own website shows Dr. Lister is an OPTOMETRIST (which is a primary care doc for vision) - NOT an MD, NOT an "OPHTHALMOLOGIST" or a "SPECIALIST." In fact, the front desk told me that Dr. Lister is the only doc in that Sunset office - no OPHTHALMOLOGISTS there. After this, in a final desperate attempt to justify the up-coding, "Dar" tried to make a big deal out of Dr. Lister noticing I had mild cataracts. Well, of course. That's one of the many things an OPTOMETRIST is SUPPOSED to look for during a ROUTINE exam (along with glaucoma and macular degeneration). But there was no referral to an OPHTHALMOLOGIST for treatment. Likewise, all 4 of my other OPTOMETRISTS in the past 11 years observed the same cataracts, but none tried to get "extra credit" for noticing it, and all billed insurance honestly, using the correct code, resulting in $0 copay. "Dar" refused to correct the billing, ruling out the possibility of another "honest mistake," and went on to confess, "WE BILL LIKE THIS EVERY SINGLE DAY." Wow, so it's not a "one-off," but something that gets past a lot of patients who aren't experts on billing codes. She then threatened collection action, and encouraged me to "go ahead" (and leave bad reviews). So, here you go. I really don't blame "Dar," since she was certainly trained to do exactly as she has done, and say what she has said. End note: Of course, if Richens and Dr. Lister want to insist he performed a more advanced exam (99204) instead of, or in addition to, what I came in for (92004 or S0620), then he did so WITHOUT MY INFORMED CONSENT. Now that's a more serious matter.

I TRIED. Richen's on Sunset is "IN NETWORK" for my MEDICARE ADVANTAGE plan. I COULDN'T HAVE MADE IT MORE CLEAR TO THEM THAT ALL I NEEDED WAS THE ANNUAL "ROUTINE EYE EXAM" which shows "$0 COPAY" IN MY "EVIDENCE OF COVERAGE," and that is exactly how it has been billed with 4 other eye care providers in my 11 years in Medicare Advantage. I made this clear... 1) on the phone when I made the appointment, 2) also when I arrived, and identified myself, and 3) again after the exam when I left. All was OK. Nothing due. I didn't notice anything different or unusual about this exam, and wasn't told it was any different than a "routine exam." BUT STILL... RICHEN'S "UP-CODED" ME AFTER THE FACT, for a COMPLETELY DIFFERENT, MORE EXPENSIVE EXAM - one I WAS NEVER INFORMED OF, AND WAS GIVEN NO OPPORTUNITY TO DECLINE - even though they had three opportunities (listed above) to warn me and get my consent. This would be a huge problem on its own - NO CONSENT. So then, I get a $375 bill, and had to call billing. I was initially told they never even submitted it to my insurance, saying, "gee, we're only human." Well, $375 is MUCH, MUCH MORE than they ended up getting from my insurance. "Human" nature to try for more, I guess. But even when they submitted it, they billed as though it was for a MEDICAL SPECIALIST (code 99204), which IS SIGNIFICANTLY MORE MONEY FOR THEM, AND has a copay FOR ME. This was INSTEAD OF the "routine VISION exam" I scheduled with them, I and came for. This was supposed to have billing code 92004 or S0620, and $0 copay, as billed by my 4 previous optometrists. I had to investigate and uncover all of this myself. When I called billing again, they tried to justify the higher charge by characterizing my ROUTINE exam as being a "SPECIALIST" exam (NOT TRUE), performed in an OPHTHAMOLOGY office (NOT TRUE) by an OPHTHAMOLOGIST (NOT TRUE). Dr. JEDEDIAH B. LISTER (an OPTOMETRIST) is the only Doc in Richens Sunset office. In summary, Dr Lister either: 1. Didn't do anything outside a "routine" exam, yet up-coded it as though it was a medical exam, 2. Or, he performed an exam I did not ask for, was not told about, and did not approve. Which is it? He noticed I had mild cataracts. (No treatment, or advice). So what? A routine exam is for the purpose of noticing many things including cataracts. My previous 4 eye doctors, over 11 years, noticed the same cataracts, but NONE of them tried to charge me extra for just noticing it. The billing dept. refused to correct this bill, and as others have said, threatened collection plus 33% if I didn't pay up. This rolled off her tongue as though it were a script she recites every day, and in fact, she admitted that they BILL THIS WAY EVERY DAY, which tells me this is NOT an honest mistake. She also claimed that my Medicare Advantage always has a co-pay for vision exams (COMPLETELY FALSE). But if that were true, then they should have warned me in advance the exam would not be $0 copay, as I specified to them. She also told me to go ahead and post bad reviews and complaints, which sounds like this is not at all unusual for Richen's. When I asked to speak to the office manager, I was told this is the office manager's last day. I wonder why.

The people there were phenomenal and got me set up with 2 pairs of new glasses in 2 hrs! It would've been faster, but they had to turn one set from puck into lenses. The doctor finally came in and I voiced my frustration of having to wait such a long time. He didn't seem very sincere when he apologized with empty words to appease me. I think this is just part of their practice and they think they are so busy and making so much money that they can get away with their disregard for their patients. Well, I'm taking my business elsewhere. StG may be a small town, but there are other ophthalmologists who will be happy to have my business. I'll make sure my primary care physician knows so he can stop referring them to patients.
