The facility is wonderful, but Executive leadership has been a challenge! Within that venue, the food & activities have been a challenge! Rita Adams was a conservative & brilliant executive! She was just getting to the food & activity improvement when we were told that she found a new opportunity more suited to her desired future. We would not want to stand in her way, but she is SORELY MISSED! The food has been based on cheap & overused oil. Nothing can overcome the nasty oil being used! The protein is limited to mixer of meat, bone, & grizzle that ruins every dish. Occasionally there are suspect cuts of real meat with little relation to real meat. Most often the facility depends on slices of heavily processed meat rolls. The people cooking have no sense of how to season the food! There's far too much of chili powder & coriander, to the point that we can't consider eating the poorly seasoned compositions. A superior chef is sorely needed! The activities were exceptional with Jessica Trevino. Jessica was pushed to do the job of at least 2 people, which ruined activities. The new activity director has been given far more support from CNAs, but Jessica contributed wonderful food & activities above & beyond what is currently provided. read more

Very hard to get to talk to someone, the main line never answered -I was on hold for 45 mins just to hang up. I wanted answers about my family member, I just showed up and was not happy- I had an amazing CNA in the 200 hall named Maria R, she helped me find my mom, which she was currently working with, Maria had her room clean, as well as making my mother looking presentable. I tried to ask question from the nurses station and I was rudely ignored . this is an ongoing issue and if it wasn't for that one CNA, my mother would be out of there! read more

Sorrento skilled nursing facility on Babcock Road, San Antonio, Texas, was the only SNF in the San Antonio area that promised they could and would provide the administration and monitoring of the custom TPN I need to maintain life, along with the nursing care to monitor my daily medications. I have been here for almost 4 hours and have yet to be admitted or assessed! This is unfathomable to me. I've spoken to the floor CNA who told me that the RN "has been really busy "! Again, unbelievable that Sorrento accepted the Medicare assignment of benefits and as yet, has provided me absolutely no medical care. I will update this review as soon as possible. The nurses quality of care is subpar. I have seen several rehabs/nursing care centers with great care to their patients and their family. Here at Sorrento- they lack on patient and quality care. Each time we call to do their job- they walk in with attitude as if they have been interrupted from their freedom of doing whatever they were doing. They have no compassion for the patient/client. They are rude to the patient when the patient has dementia and not cooperating. There is zero kindness or compassion here. Stay away! read more

Don't allow your loved one to be sent here. My father was ignored and treated terribly. He was not listened to when explaining that his issues were getting worse, and they were getting worse. When he would ask for help at night, they told him NO. Forget trying to speak with a nurse. You are met with hostility when trying to check on your loved one or to ask questions. I tried to speak with someone there in person and stood at the nurses station for 10-15 minutes before anyone even strolled by the station. When I could finally speak to someone, I could hear another patient calling for help. They ignored him. I said, " doesn't someone need to check on him?" They said No. Calls to people in charge were eventually returned and all the promises in the world that things will get better- THEY DON'T. My father was in so much pain that I could hear him screaming when I was speaking to the nurse on the phone. She was at the nurses station and he was in his room. He was transferred out of there the next day on the recommendation of his PCP. Don't send your family member here. read more

I began looking for a skilled nursing facility because my wife wasn't able to do 3 hours of physical therapy. I checked out Sorrento's because it was a beautiful facility and I felt my wife would thrive here. We'll the facility is beautiful, and the people working here are for the most part incredible. The problem is they are so understaffed that proper care is not given to the patients. My wife was here for 24 days and developed bed sores. They should have been turning my wife every 2 hours and I should have noticed they weren't. My wife would use the nurses call and wouldn't get a response for up to an hour. She would call me and I would call the nurses station telling them my wife needs help. When the staff was with my wife they were great. The main problem I saw was they are severely understaffed. If you bring your loved one here or anywhere make sure you pay attention to their care. read more

Let me first start by saying this is the perfect place for your Worst Enemy. My mom was first admitted to this facility that is referred to as a Rehab Facility on 04-04-2023. Her last day of incarceration was 04-24-2023. I am so glad and thankful that she made it out without her spirit being completely broken and torn. Let's start with Monica Castaneda, MBA, LNFA Executive director who I never met the whole time my mom was there at this Facility. It is not part of her protocol to introduce herself to families let alone the patients themselves. Perhaps this is something that could be brought up at a Board Meeting. (Kudos to Juanita, Asst. Exec Director. She had a completely different stance. She thought it important that the patients be met). Moving on to the Nursing Staff or lack there of. Completely out of touch with patient care. I was appalled by their insensitivity and total disrespect to patient care. The one thing that they should value most. Especially when it comes to our elders. Our founders. Our loved ones. I could tell you a daily story about the distasteful treatment and total disregard of the staff. But I won't due to time. I emphatically implore you NOT to consider this "place" if you care about someone close to you. If I could give less than a 1 star rating, I would. I promised to update my initial review, so here I am. After Mom was diagnosed with the onset of dementia she became a long term care resident at this facility. I have grown to love Sorrento dearly as Mom has been living here for more than 19 months now. Mom herself has been giving Sorrento a PERFECT rating whenever asked for her assessment. I have nothing but praise for Sorrento's staff. Maria has been Mom's amazing main CNA since the beginning but I would like to also give a shout out to Julia who has recently been likewise entrusted by Mom to give her her showers. Shout out to boss LVN Marissa, LVNs Dana, Arvin, Dorris, Alex, Triston and Gloria who just recently replaced Lloyd (amazing guy as well) as weekday swing shift nurse. Shout out as well to CNAs Stephanie x2 (there's two of them), Frank who always visits Mom though he has other responsibilities now. Shout to Mom's medical aides Ivy, Lisa & previous ones ie. Heather etc. Shout out to Joanne, Mom's main housekeeping aide who makes Mom's room in perfect order all the time. God bless y'all. However, my personal experience as a resident's son evidences why I thank God each day for gifting her with Sorrento. Mom goes to Communion service at Sorrento's chapel every Thursday afternoon. I try my best to accompany her each time. Thanks and blessings to Irena who volunteers as a eucharistic minister from St. Brigid Church nearby. I wish that Mom availed of all the amazing activities offered by Sorrento to residents such as Bingo, crafts, mini concerts, cake and popcorn get togethers, community meetings, and even outings using the free Sorrento van, all thanks to Mike, Sorrento's Life & Activities director. I always have peace of mind that Sorrento is there to take good care of Mom especially when I am at work and travelling for a few days. I am sad that I have seen a few bad reviews for Sorrento. I wish that all who have experienced Sorrento's blessings would post their kind reviews as well so that we all could drown out the bad ones. Sorrento has had hundreds or even thousands of patients just over the couple of years it has been in existence. I'm sure that lots of previous residents and their family and friends could share their amazing experience with Yelp. I will update y'all again in a few months. Worst experience ever. Do not take your loved one there. Absolutely no care provided. Ignored. Finding a nurse, impossible. Had to asked 5 times for my mom's medication. Got into a disagreement with her nurse, Luis, over getting her medication. He raised his hand to me, dismissed me, said he wouldn't talk to me anymore. Got the DON involved and mom got her meds. Just crazy. With all this, I do need to mention that the Caseworker, Allison, was wonderful. She was the only one who would listen to me and try to help. Can't praise her enough. Also, the central supply guy, J J, nicest guy. Went and got a pillow for my mom, after I had asked at the nurses station several times for one. Trust me, this is not the place to go to. read more

The entire place has a odor of neglect. Sorrento administration is a Labyrinthine of Hierarchy. Its impossible to tell who is accountable for any given order on day to day operations. for weeks now we are running out of briefs every night. The staff executive manager was unavailable to speak to me. The don is currently on vacation. the manager Melissa just returned from vacation. When they give out the diapers/briefs it is just one at a time as if they were gold or something precious. I suffer after chemo and radiation and was treated for rectal cancer. I would rather wear one than clean bm off my clothing with a brush.The social worker is only part time. During the day time we are short of help always but have the same problem getting necessary supplies like wet wipes and size L gloves. I want to live here , already I have been here a year but things seem worse especialy the low grade of food. When I speak up the nurses & cna will make me suffeer. Advocating is also discouraged. I would like to see this resident abuse discontinued and it become the paradise that it appears to be from outside and the garden area. Very disappointed in this facility. First it was very difficult getting ahold of the nurses station. Our Father was transferred from the Hospital and they never informed us he arrived. A kind Nurse from the Hospital even attempted to call several times for us as to put us on a 3 way call and they never answered. My father was made to get up to the BR but on this occasion he told them he didn't feel well and felt his 02 was low. He was told he needed to get up anyway. He had a syncope episode that landed him back in the hospital. We would be on the phone with him while he pushed the call button and it would take 20 to 30 minutes for them to come help him. He told us he felt like everything he asked or said went on deaf ears. He was discharged home in a very weak state. He is getting stronger at home. Would not recommend this facility read more

1 star Is even too good for them. My mother was there less than a week before they put her in the ICU and she died. I was on the phone with her and heard her crying for help over 15 Minutes. We called the front desk and still no one came. Her Foley was disgusting when she went to the ICU and she had a UTI and a massive infections in her lungs. She told me you would have to be a strong person to survive this place. Since she no longer has a voice, I'll be her voice..... please don't send your loved one here. I had a very good stay at Sorrento's Rehab Hospital. The nursing care was great except for one nurse. I talked to management about the poor care given by her & she was not assigned to me again. I was in the Babcock location Rehab Center, San Antonio, Texas, March 10th - April 5th. The physical therapist & occupational therapist were excellent & I also had good fun with them. The food was above average. But there could have been a little larger portions. The room was very nice & housekeeping team kept it very clean. I did like having a microwave oven & small refrigerator in my room. I've read the reviews & they are varied. No place is perfect. I've been in rehab four times in the last ten years. Sorrento's has been the best place of the four: The Nix, Remington, NE Baptist Hospital & Sorrento's, all in San Antonio. Overall I was very pleased with my stay there. I would recommend this place if there is a need for short term rehab. read more

Shame on them, while in their care my mother got worse. There are a few good PCAs that actually care and a few good nurses. All the others did the very least they could and still did so begrudgly. She was there to rehab instead after a week they made excuses that had no plan from the Dr. Very dissatisfied and disappointed with them. They also let a nurse leave with the medicine key one night and the patients had to wait 30 minutes to get their pain meds....take your loved ones elsewhere. read more

My family's experience here was beyond awful. I realize that all healthcare systems are stressed at this time. However the incompetence and utter lack of human compassion at this place is a nightmare. I will say that most of the CNA workers and some of the nurses were helpful and compassionate. Beyond that, there was no communication, no social or case worker to help us navigate needs and a plan. I sent emails and left text messages and no one bothered to even respond. The folks we did manage to speak with face to face were evasive and hostile. We were finally able to get my father out of there but not without a fight. They refused to release him making excuse after excuse. They claimed we would be putting his life in danger. My conversation with a nurse just the day before indicated no such thing. On top of that they called Adult Protective Services on us for "endangering" his life. The only endangerment was from the neglectful hell hole he was in. We hired an ambulance and moved him to a facility that has shown nothing but competence and compassion. We are so relieved to be done with Sorrento. read more

This our favorite place to go to for our 88 yr old father to rehabilitate, this year is our 4th year coming back to Sorrento. Together ee can do it, from the wonderful staff to me and my Dads caregivers make the upmost effort to keep dad motivated and Thank God each time has been a miracle of taking Dad home walking once again. The atmosphere and meals here are wonderful as well. The whole place is kept so nice and clean its a welcome smell when you walk in. read more

I took an unannounced tour of the place a few days before she was transferred here and found the place was immaculate and any staff I came across was extremely courteous and helpful. When I asked I was given an impromptu tour of the place. My mother has been treated very from arrival to date and is improving daily. My mother had the misfortune of having to end up here. It has taken our family over a year to get her back to a resemblance of what she was before she went in there. To say this place is a hell for these patients is an understatement. Let's start with the nurses, they are highly unprofessional and lack any kind of compassion what so ever. The doctor that is supposed to be watching her care and administering check ups was never to be found so we could speak to her. My mother had a huge ulcer on her foot and from hitting her on her infected foot, forgetting to feed her, stealing her food, forgetting to schedule her bus for Dialysis, to giving her too much pain meds, and finally my mother was having a cardiac incident after a dialysis session one time and the nurses told my sister it was normal for her to be unresponsive. Thank God for my sister's tenacity she lifted her into a wheelchair and pushed her next door to Santa Rosa. The professionals there were surprised with the state of affairs of this place. My mother was out for a few days and the "director" of the place Denise, rudest most heartless individual I have ever encountered, threw all my mother's belongings in a trash bag and advised she needed the bed and for us to come pick up her stuff. These are just a few of the ordeals we had to deal with. I could write a book on the incompetence of the care manager. He was a joke a young maybe twenty something young man whom clearly didn't know what he was doing. When we finally spoke, after hounding him for like 2 weeks, he had the wrong file didn't know which patient my mother was, ridiculous. I hope they close this place down. Please for the love of God do not send anyone here unless you want them to suffer. read more

We have read several of the reviews about Sorrento and wanted to make sure there was a balance of feedback. My mother-in-law was transferred to Sorrento after a terrible experience at another care facility and we cannot say enough good things about this community. We have been impressed ever since the CEO of the hospital came to see her and they shared ideas, thoughts etc even though she is not up to such a long conversation. Let me share our perception and it has been three weeks as well as many of the other reviews have shared: 1. CARE: very positive, very engaging and very much aware of all aspects of her needs. The nurses are always responsive, they know her needs and most of all her idiosyncratic behaviors. John is just the best and he is aware, engaged and always attentive. 2. FOOD AND STAFF: This is huge for us as she is a vegetarian and she has been raving about the detail the kitchen does for her. One girl I see almost every day is so sweet, caring and always makes sure she has what she needs 3. HYGIENE: Always attentive and always asking about her needs. The CNAs are so good and so much a part of the overall care and they will always respond to her 4. PT/OT: This is by far the best care and best regimen. I have asked for a treatment plan from other facilities and they share the same thing: We are evaluating her, like for two weeks! At Sorrento they dig in, know her physical needs and are so attentive and positive, it amazes us daily. I am not sure why the other reviews are so hostile, as we see at all times of the day how caring and engaged the staff is. Read this please as it is imperative all see there are always two sides to an evaluation of services. Thank you all, especially the kitchen and PT/OT folks, this has been a God send! This place is yet another one of those places where you don't want a family member to end up at. The vast majority of the staff are rude, cold hearted, mean, and unprofessional people. I don't have any problem with the color of people's skin, but the majority of the staff are loud, obnoxious, ghetto street trash. Their cell phones blaring rap music ringers every few seconds, yelling and laughing at each other down halls, etc. Most of these people are only there because they would be a cashier elsewhere and this pays a few more bucks an hour. The nurses don't care about the patients and lie when you ask how they are doing. One nurse says oh he is walking around and took a shower. (He isn't mobile......) One nurse says one thing, the other says something completely opposite. You can listen to the blaring beeping from the call button being pressed in one of the rooms. They just ignore them. I go in and check on my family member and they forgot to give him dinner. Then they finally bring him solid food and he is supposed to be on a soft diet. I had to catch that. The rooms were not cleaned. Patients not being bathed. One morning he spilled some oatmeal on his shirt. The next night, he still has that shirt on....... I asked the nurses over and over to periodically just pop in and ask if he needs to use the restroom and help him get to the bathroom. They refused. They just put diapers on and then never checked to see if he was clean. They let him sit soiled all day. He tried to go to the bathroom and was not strong enough to go. When the nurses found him on the floor they berated and yelled at him. They then punished him by forcing him to sit in a wheelchair in the hallway for hours. He gets confused because the therapists tell him to move around, sit in a chair, anything but lay in the bed all day. The nurses get mad and tell him he better stay in bed. He is not to get out of bed.... This place is a living hell for people that cannot do or speak for themselves. I hope these cold hearted trash employees get the same treatment or worse when they are become sick or injured. read more

I agree with the posts about Sorrento looking beautiful, however, the customer service for the patients and their families leaves a lot to be desired. My hubby was there for rehab after a bout with pneumonia. He also has Altzheimers. He fell overnight, and was transported by EMS to University Hospital at 6:00 a.m. A message was left on our son's cell phone about the transport. A few concerns: Our son's # was NOT the main contact number. It took them 4 hours to initially 'admit' him from hospital #1. He had a yellow Fall Risk band on his arm from hoispital #1. No one could tell me what happened or when. Who knows how long he laid there? He was septic, had pneumonia, and unconscious for almost 30 hours after the fall. No one called with a follow up or even questioned why I pulled him out of there. No paperwork from there went with him to University. He went there with only his wrist band from his previous hospital stay at another hospital. Oh, I see his paperwork here, I guess I should get it over to University Hospital was the response with the phone call I received 8 hours later. I did get a return phone call 8 hours after I left an urgent message. The message had been left in her mailbox. My hubby currently in a different rehab, recooping from the fall. read more

My father in law was at Sorrento for rehab. When it was time for him to go home, he didn't want to leave. He loved the food, the staff & especially therapy. He enjoyed the amenities of having his own private room, restroom, kitchenette, microwave, & TV. When he finally left home, he told the staff he would be back. If you are thinking of admitting someone with a more serious health problem and the hospitals try to release them here, and you love your Mom, Dad, or whomever, please go somewhere else. This place looks beautiful, but the care is horrible. If you are just rehabilitating from an injury, you might live and be okay, but for those that need a higher level of care where the nurses actually DO something, you are taking your life in your own hands. I had a parent admitted here and when showed signs of having a stroke, the nurse took over 20 minutes to respond. When I stood in the middle of the hall at the nurse's station that they take action, they STILL took their time. When I tried to override the obvious lack of care and tried calling 911 myself, they raised all sorts of cane and were going to refuse him to be released until I began yelling. I'm a calm person, but this was an absolute circus. NEVER NEVER NEVER go there. My father never recovered from that night and had complications from it until he passed away a few weeks later. My 73 year old father ended up at Sorrento for physical therapy after suffering a fall that fractured his pelvis. To make matters worse he has esophageal cancer for which he was receiving radiation therapy daily before the fall. Because of the cancer he is unable to swallow properly and therefore had a tracheostomy and a peg tube for eating. My father was not going to be an easy patient that could be checked in on every 2 hours and get up for physical and occupational therapy a couple times a day. Long story short I am writing this review because the reviews I read about this place scared the heck out of me and added extra stress that turned out to be unwarranted for the most part. The complaints I have are not specific to this this facility but to the industry and individuals. Not every nurse has the same experience or professionalism and that is why places like this have lots of oversight and procedures. While at Sorrento we had some wonderful nurses and physical therapist. My father is back home well on the road to healing and able to continue radiation therapy because of the care he received at this facility. read more

I wish there was a way to put a negative rating, my grandmother was here for 3 weeks and at first it seemed ok, nice facility, pretty clean, and her first nurse was very nice. Then things started to slip, SHE HAS DEMENTIA AND WAS GIVEN CONSENT FORMS TO SIGN, nurses were speaking to her as if she would remember simple task like no one read her file, and then for three weeks her laundry was not cleaned. We signed up to have the laundry washed and when we brought it to her nurse's attention one night she said that they were under the impression the family was doing it even though in her chart it stated for the facility to do it. The nurse was aware my grandmother did not have any clean clothes and then three days later when we go to help her pack since she was getting discharged the LAUNDRY WAS STILL NOT DONE... AFTER LETTING THE NURSE KNOW THAT IT NEEDED TO BE DONE, HER CLOTHES CONTINUED TO SIT IN THE HAMPER. This place needs more staff, it is so obvious, my grandmother needed help using the restroom and twice when I was there visiting we called for a nurse and waited 45 minutes one time and then 30 minutes the other time. The reason for the wait was because her nurse was on lunch.... is there no one to cover when someone goes to lunch or do people need to just sit in their own waste until someone comes along. Hopefully no one was an emergency while someone is on lunch because it could be hours before someone arrives. Her sheets were always dirty, one time a nurse came in to help and told her the previous nurse did not clean her properly and she was still dirty. I found a catheter on the floor, and there was always trash laying around as if things were thrown and not picked up. One time her dirty linens were in the trash and the bathroom smelled of fecal matter. Maybe if there was more staff people wouldn't have to rush while taking care of someone's family member. IF YOU HAVE FAMILY HERE REMOVE THEM AS SOON AS YOU CAN, IF YOU DO NOT HAVE FAMILY HERE NEVER BRING THEM HERE. read more

This place would get no stars from me . Looks can be deceiving . My grandmother passed 3 days after being there . The nurses were crying historically because she had passed, maybe felt some sort of guilt . The was sound asleep when we saw her the night before , the nurse went and gave her insulin and she didn't flinch we should've asked more questions and not let him inject her. I think they over Medicaide's her . She was suppose to start rehab and for what ever reason kept on getting pushed back. They said she suffered a stroke in her sleep I don't believe it. Also when we asked how she passed or who she saw last a nurse said she woke up in the middle of the night to tell her she was ok . Lies because they couldn't even show the cameras . I don't think she was checked on and maybe that is why the nurse was crying. Please don't send your loved ones here. It is a horrible place . They do not pay any attention to the residents . read more
