My step-father died shortly after coming home, he was placed in this facility for "rehab" but he died instead. He was here during the COVID lock down so they would not let family in to visit and I was told he did not want to talk on the phone. PLEASE DO NOT leave your elderly loved one in this…read morefacility if they have had a big surgery like my 80 year old Mother had, and they require round the clock care. I put my Mother here, because this is what I thought she would get. Not mistreated and neglected! Some of the staff and CNA staff is uncaring if at best and very unprofessional. There is no DR on staff that I am aware of, just RN's and LAZY CNA's. Most of them do not belong in this field of work, they are lazy, lack empathy and just here to collect a paycheck. None of the CNA's had on name tags, but you can bet I do remember faces. My Mother had a lower spine, 3 level laminectomy on 2/8/19 and was transferred to HP on 2/12/19 for PT. Starting on the very first day I had received repeated complaints from my Mother that some of the CNA staff were being mean to her, (she did have hearing issues and I made them aware of this) And she had a hard time getting them to take her to the bath room when she requested it. She was even found on the floor, because she needed to go to the bathroom and no one came to help her. (she could not walk, and all I heard from a few of the CNA's was is she is dead weight, we can't pick her up My mother was all of 5'0 and 135 lbs.) HELLO- I don't care if it takes 4 of you to lift her up, put on her back brace, in a her wheel chair or walker and get her to the bathroom, DO YOUR JOB! Her shower was always bone dry, they did not bathe her, but my Mom probably didn't want those GITM to bathe her anyway. They didn't help her with her expand a lung breathing machine, which is to help prevent pneumonia. Long story short, after 5 days of her not having anything done, but mistreatment, and especially having no LABS and BLOOD work, other than some brief PT/OT, she called me and begged me to come get her on 2/17/19. I raced as fast as I could from FL, where I was helping my brother with my father who was dying of cancer and went straight to HP to get my Mother. As I was making my second trip on the getting on the elevator to get my Mom's belongings, I heard one of the CNA's who was standing at the first floor nursing station, she was talking with another woman (that lady was kind and did come to the elevator and ask me if I needed any help), as she knew my Mom was being discharged yell out "Hallelujah, thank God." Lady, I don't know your name, but I do remember your face! Little did I know that 3 days later my precious Mama would pass away due to renal failure, from a UTI that was not caught and treated in time. So if you value your loved one and the type of care they receive for after surgery, this is definitely not the place.

    I would give this facility a minus rating if possible. I have encountered every possible situation…read morehere...from lying staff to refusing to administer required meds, to inadequate therapy.The food is downright comical (as in "what is it?") I have not seen anything fresh in the week I have been here. Germ control is frightening. I pray I don't end up with sepsis. Where is leadership? Post-discharge, when I called to speak to the social worker regarding my dad's home health referrals, the calls were frequently disconnected during the transfer process. Sometimes I had to call 4 times consecutively before the call finally transferred to her (or, more frequently, to her voicemail). When I asked for a direct line for her, I was told there wasn't one. Pruitt's Corporate Hotline, which I called on 12/1/23, did not get a response from the Evans facility either, or at least none that they relayed to me. Finally, after multiple unreturned voicemails and emails to the social worker, I finally alerted the employee who answered the phone at the Evans facility that I'd be filing a complaint with the state licensing board for the delay of care--because they had still not sent over the paperwork for home healthcare referrals, several days after my dad had been discharged. At this point, the social worker, Myesha, finally speaks to me on the phone. She then claims that the orders for home healthcare are "never signed," and implies that it's our family's fault for wanting to use Enhabit for home healthcare services because she'd "never had a problem with an unsigned order before and it's your fault for wanting to use this home healthcare agency." She added, "All I can do is ask the physician to sign it." After I tell her I'll hold for her to check if the physician signed it or not, she then says the physician has signed it and she's sending it over. By the time my dad finally got home healthcare services, about a week post-discharge, the home healthcare nurse commented how it was a shame that my dad had lost so much rehabilitation time as he was now behind in his recovery. She expressed shock that the paperwork hadn't been sent over to the home healthcare service as soon as Pruitt had learned of my dad's expected discharge date. Please don't let your loved ones stay here.
