We have had a great experience with the staff here. The activities are always the BEST!! There is entertainment weekly with themes, singers, musicians and Food. There are exercise classes daily, Bible study groups and the favorite is the Travel Guides. The residents get to explore areas of travel from all over the Globe The food has also been delicious. Both of my parents (early 90's) moved in to Sage Mountain in early 2025. They and I did not know what to expect since this was their first time living in a home they did not own. Due to the assistance they now required, Sage Mountain was a perfect fit. They now can socialize and participate in activities they were unable to do on their own. The staff is extremely friendly and supportive. They love the food. It seems whenever I come in (at least once a week if not more) there is always a new activity going on related to the time of year. My mom started out in the assisted living facility and after a while, she moved into the Memory Care. Shane is AMAZING and so caring and patient with the residents. He is a big reason why my mom is there. I have peace of mind knowing that my mom is well taken care of and is safe. This senior living is strictly for people who need "No Care". They do a prior assessment of incoming residents before move in. They advise what the extra level charges will be at that time. Then, they don't provide the care they agreed to give. Two weeks later, they come back with double rates due to a "change" in care. The caregivers had no idea how to care for my father. I had to show them how to provide the care they stated they were capable of giving and should have known how to give. The lack of care caused a horrendous UTI which caused my father pain, disorientation, and confusion. Sage Mountain called to say he had escaped from the facility. This was not common behavior and we advised them it was only because of his confusion due to the lack of care that was caused by Sage Mountain. UTIs in the elderly cause confusion. This was a direct result of inadequate care. In addition, our private in-home visiting care nurses were very concerned because the care required was not being done and my father was suffering because of it. Sage Mountain was advised numerous times by in home heath care professionals that they were not giving the proper care. We were advised by in home health to get them out quick. Sage Mountain suggested we put our parents in their memory ward. They said all these great things about it. They stated they work with the residents, provide activities, etc. I went up on 3 occasions and found my parents sitting in a corner alone and scared. The other residents were all in wheelchairs lined up in front of a TV. I never once saw any interaction. It was horrible. I would stay as far away as possible from this place. They pride themselves on sucking you in but then, once in, tell you everything is changed, more money, no care. At the advise of in home health professionals and Dr, we moved both our parents out after only 6 weeks there. Stay away, far away. We were told that they only have 4 caregivers for a very large facility with definitely over 100+ residents. There is no way adequate care can be provided. Current update: In addition, Sage Mountain wants to bill us for time and care "after" the date our parents moved out (which was due to their neglect). I am currently working with senior social services, senior advisors, and an attorney to alert of Sage Mountains neglect and illegal billing practice. This was a horrible experience dealing with Sage Mountain assisted living. If your love one needs any sort of assistance/care, you should avoid this senior living. My mom was in the Memory Care facility for a year and half. We loved the staff and were very aware that they have a tough job. After my mom fell twice in one week and broke her shoulder, we asked for more care since she was a fall risk. Before she was even released from the hospital and not even reevaluated, the higher ups at Sage Mountain decided to charge us double for her care..... over $14,000 a month! We moved my mom out within 48 hours to a board and care where she is receiving way more care and cheaper than what we were paying per month before the fall. Total money grab! read more

We looked at every single one almost & not feeling good about any in our budget I decided to look here anyway since its fairly new & real close to us, and this is the one we chose! I knew right away with seeing it, talking to staff, going a few times, and seeing the care & activities, food, atmosphere, we chose Sage & im still so happy everyday that i didn't go by reviews or he wouldn't be here. And it only gets better the longer we are here & see the care. I had to share in case others need real opinions and based on post COVID struggles which were all discussed & made perfect sense why they had kinks to work on due to lack of finding good employees during a time no one was choosing to work & they had just opened , and a chance to get they employees that were the right fit . With all new staff over the past several months it was obvious that they have completely turned it all around and it shows 100%!! I didn't give 5 Stars yet Only because we have not even been there 3 months & I want this to an honest 100 % Review. As they all should be so i will update in a few months! My parents have been at Sage for two months. Ian is definitely the best part of Sage. He has been so responsive to our parent's needs as well as ours. There were a few bumps in the road in the beginning and Ian helped resolve anything that came our way. My parents are adjusting which can be difficult, but the staff at Sage have really tried to make it easier for them. read more

I personally visited over 20 different communities in the area before selecting Sage Mountain for my mother. Even though there were some negative reviews I decided to visit. I knew right away that this would be the best fit for my mom. The energy in the common areas, the residents and staff conversations, the dogs visiting residents along with the location made it the right choice for us. Melissa, Nidia, Ian and Nicolle have gone out of their way to make my mom's transition as easy as possible. The community of residents is engaging, and the caregiving staff really try to get to know each resident. No place is perfect, but my mom is very happy with her care at Sage Mountain. read more

The female front desk (1/20/21) at approximately 11 am to 12 noon. I didn't catch her name but there was only 1 female front desk rep at the hour I mentioned. The attitude she gave in person and even through phone really got me irritated. Mind you, if you need to put someone on hold. Let them know. From my experience with this place, it seems there is NO LEVEL OF CARE here for those who need assistance with dressing, bathing, eating, or physical company. (I cannot comment on other needs or facility offerings). We recently had an extremely unfortunate experience at Saje and were unable to check on our loved one for 6 weeks (due to COVID restrictions) to comprehend the level of neglect he had faced. After removing our loved one from this facility, we hired private care - many of which told us they had quit their jobs at Saje due to its lack of commitment to full service care. It is HEARTBREAKING. We took our loved one out of the home as soon as we could, and discovered bed sores, stiff joints, and substantial weight loss. He has severe PTSD from being left unattended and has developed night terrors due do this. IF YOU LOVE YOUR FAMILY, I WOULD NOT EVER SEND THEM HERE. Do not waste your money read more

Absolutely disgusted with this place. after hearing from multiple nurses and having a feeling that my grandfather was not taken care of we took him out of the home only to find out that our fears were true. His back was covered in bed sores in my opinion from not being moved, not to mention the amount of pain he was in from being bed ridden. His heart is failing and was in the home for 24/7 care to assist him but it seems as if the nurses never came to him when he called. We've taken him out of this disgrace of a care home and at night in his sleep he screams out "nurse nurse nurse!!" We now have in home care workers that tell us they believe it's PTSD from being in the home and calling out nurse and no one ever coming to help him. My grandfather was in the home because he has a failing heart and according to a nurse the only food they fed him for breakfast lunch and dinner was eggs and bacon. If this is true it breaks my heart. He has lost over 30 lbs, and I still cannot wrap my head around the idea of giving a man with heart failure bacon. If you love your family do not ever send them here and waste the thousands of dollars on this despicable place. I cannot even begin to explain the amount of stress and trauma we have been through especially my grandfather. read more

I will never recommend Sage Mountain to anyone or to any of my doctors to recommend there friends, family or acquaintances. They had 4 Executive Director in less than 2 years. Memory Care residents are at the mercy of how the corporation wants to run that part of the the facility; the residents who cannot speak for themselves. Memory Care did not have a full time LVN for morning or evening. Caregivers, Med Tech's or Director of Activities in charge of this facility because they had less residents than assisted living. Only a brainless moron would apply that rule, those people need the most care. Maybe that is why there are frequent UNWITNESSED FALLS? rooms not always clean, poop on rugs dirty diapers on sink where teeth are brushed at times, clothes missing and clothes that are not theirs? Long Acrylic finger nails on the Caregivers that can cause injuries, Caregivers standing around the nurses station talking and laughing instead of watching the resident's closely in Memory Care. We could no longer go onto the facility when the COVID-19 started in March 2020. We noticed employees without masks and others like maintenance workers not wearing a mask at times. Caregivers without name tags, missing medication and caregivers not notifying family members immediately regarding falls and documentation not the same if given out to the family by 2 different staff members. We saw a caregiver go against hospice orders in how they treated our family member and what was to be done. They chose to do what they wanted to do. FAMILY member taken to E/R for bad falls. Information that should've been reported from the medical chart was not used and false information given to a hospital and to an outside agency for an assessment. The new Efecutive Director at Sage had no compassion for us, our loved was dying and never once did she contact us or ask if we needed anything. Orders from hospice were not followed and requests from us were answered 1 to 2 days later We were sent a plant a month later after his death. He did not have to die. Read all the reviews which are true. Many Caregiver's have left the company and other staff employees. If you post anything that disagrees with SAGE'S Facebook that goes against the new executive director you will blocked from making any comments even the LIKE button. If you really care about your loved one, you will do your homework and check out the facility where you are going to place them for care; or REGRET WILL BE WITH YOU THE REST OF YOUR LIFE AS GRIEVE FOR THAT PRECIOUS LOVED ONE THAT IS NO LONGER WITH YOU. SAGE IS BEAUTIFUL ON THE OUTSIDE, BUT I WOULD NOT RECOMMEND IT TO ANYONE "EVER AGAIN " IF THEY CAN'T GET TOGETHER IN 2 YEAR'S WHEN CAN THEY?" The last executive director was removed unexpectedly January 15th, which was a surprise to everyone as she was a very hard worker and very loved. The outbreak of COVID-19 was not reported exactly how bad it was within the facility, which included staff and residents who texted positive. If you look on your insurance billing Vikor was the testing agency; they will not release the results if positive or negative you must ask, SAGE MOUNTAIN FOR A COPY OF YOUR FAMILY MEMBERS TEST AND HOW MANY TIMES TESTED! I also complained to management that the Memory Care residents needed the most care because of the Dementia and Alheizmer, because they cannot speak for themselves; there family is there ONLY ADVOCATE and WATCHFUL EYE'S. Read the reviews about Sage, there Corporation and the latest COVID-19 disclosures that were not reported. The Acorn newspaper in Thousand Oaks, CA did a write up reported by a family member who recently died. They need to get management in that knows how to treat residents with Dementia and Alheizmer, ITS NOT ALL ABOUT PROFIT FOR THE CORPORATIONS WHO OWN THESE FACILITIES!! Med Tech's, Caregiver's and Activity Director's are not nurses, where are there credentials to administer medication and access those with Dementia and Alheizmer? Why aren't cameras in motion in Memory Care to alert staff if a resident's fall has happened IT SHOULD BE ON ALL TIMES WHEN SHORT STAFFED, AS THESE ARE MANY TIMES SO UNSTABLE!!. This facility was very short staffed starting in November and December as staff and residents were COVID-19 positive. A family member found the web site that posted the actual facts and we were denied the information and were shocked with the actual information. These facilities are a very lucrative business. The turn over in staff is unbelievable .Check the facts out for yourself, job openings at Sage on the internet; also , reviews about the Corporation that owns Sage Mountain and employee comments. We now regret the day he became a resident in the Memory Care Unit at Sage Mountain .IT Started out very good, but ended up very bad. Read all the reviews HOW MANY ARE GOOD, ONLY ONE OR TWO GOOD ONE. My elderly father-in-law has been living at Sage Mountain Senior Living for 6 months, and he loves it there. We are well pleased with the level of attentive service they provide to him, and the accommodations are top notch. The food is really good, and the common areas, dining room, and private rooms are kept very clean and sanitized. They are also good about reaching out by phone and/or text to the family when they need to communicate with us regarding his health, well-being, or anything he might need from outside the premises. We pick up his prescriptions and a few other necessities for him periodically since we are local, but they provide a service for that too! Many of the private residential rooms have scenic mountain views with beautiful sunsets to enjoy. They also provide monthly outings to the residents, if able, and have a recreation room and outdoor seating areas. We highly recommend Sage Mountain to any senior that needs extra care and wants to enjoy their later years with dignity. read more

My father was one of the first residents to move into the Memory Care area at Sage Mountain. Although the surroundings at Sage Mountain are lovely, it was a difficult and worrisome decision to move him from the home he had lived in for 35 years to assisted living memory care. He has now been there for two months. The Sage Mountain Memory Care Department exceeded my expectations regarding the care they provide to my father. First of all, the Director James Christensen is excellent and caring. He and his staff prepare programs that provide an environment where the "in the moment" atmosphere for the residents is interesting, respectful, and enthusiastic. We are all very happy with the care our father receives. Finally, one thing I must mention that I did not expect, was the effort that the staff provides to the families of the residents. Transitioning is almost as difficult for the family as for the resident, and I believe James and the other staff spend as much time communicating with the family about the residents' needs and also help the family to understand their loved ones' process of being comfortable at memory care and in a new environment.
