The Executive Director and Director of Health Services and their staff were fully aware of the on going problems which never was resolved. I hope this is a warning for families that are thinking of putting their loved ones in Clarendale of Algonquin. We finally had enough of their in adequate facility and moved my mom out.

BEWARE!!! As long as "assisted living" is minimal time involved they are more than happy to take…read more$$$$ - as soon as you need 3 extra minutes (no exaggeration) they say they can no longer "care" for you. Director even told me assisted living isn't what they can do for you but how much staff time it takes. Then they unceremoniously tell you to leave. While you find somewhere, they are happy to raise your rate while cutting your service. Horrible management! Example: they claimed loved one was a 2 person + assist and aides were instructed to leave her in her room for all meals - in her lift recliner (where they also left her to sleep and go to the bathroom!) IT'S A LIFT RECLINER!!! I came in BY MYSELF and transferred her to her wheelchair (giving her time to get her feet under her and turn) to go to the dining room. I filmed it and the entire transfer BY MYSELF took 3 minutes and 30 seconds! this is what they claimed 2 or more of their staff couldn't do!!! PS - loved one is doing everything at the new facility that Clarendale said she couldn't do simply because they couldn't be bothered to give her literally a few extra minutes. They won't tell you this up front and have no qualms totally upending an elderly persons life for their convenience. Additionally, they screwed up submitting 2 months of insurance claims and after basically kicking her out, have not responded to requests for a refund on the entire months rent and care that was paid - we are out about $24,000 until everything can be resolved.

I have to say that my time there was not great but it wasn't terrible the people that work there…read moreespecially for one CNA Mariah she really cared for all her patients she deserves a raise. when i first got there she really noticed that i was in major pain and tried to make me as confortable as possible. When she told me about options about the shower schedule she had to do a bed bath on me and treated me with dignity and made me feel confortable. From that day on i knew that while she was there i was in good hands with a caring person. There was a few more people there that always made me confortable and these people are the reeptionist i always called her georgeous and she called me hansome she made me feel at home i didn't get here name and i am sorry. there was two more people that really kicked my butt in therapy and they were Agnes and Ross they really worked me because they knew how bad i wanted to go home and they did not take it easy on me i appreciated that there were days when i got back to my room and just exhausted. This is the reason that this review is not five stars and it comes down to privacy. This is more for the second shift when the door is closed to the room it is only proper to knock and wait to be answered before coming in to the room. They had a habit of just walking in the room without knocking. There was a couple of times where i was in the bathroom and they just barged in and opened the door. Even though it is a kind of a hospital vibe when the door to the room is closed it should be considered being private and the courtesy is to knock not just walk in the door. if i ever have to go back for rehab i would go back to avondale

When we were looking for a place for…read moremom to live, after it was evident she could no longer live alone, we looked for a facility where she could stay even as she needed more and more help. Heritage has been amazing. Managing her meds, being available as much as she needs them. When she was in the hospital they even came to visit and talk with her. I so appreciate all of their help during these very difficult times. And the best part about it... she likes it there. The food is good. The staff is helpful and friendly. They honestly take care of 100% of her needs. Thank you Heritage!

The front desk girl who does the appointment scheduling didn't allow me to. My last appointment the same front desk girl did my PT when it should have been a licensed PT. I was discharged way too soon. I don't recommend his place for PT. I highly recommend going to Northsore or Northwestern for PT. ATI released me to get paid instead of helping me get back to 100% better.

Very down to earth, honest, friendly,compassionate and caring. These visits were before surgery. I felt and was able to tolerate my bad knee during treatment but eventually needed a knee scope especially due to a torn meniscus. After surgery (Brian moved on,sorry to see him go) THANK YOU BRIAN. I had Therapist Rebecca S for a few weeks, she was also compassionate, friendly, always smiled likes her work, cares for her patients and was honest and answered any questions I had with dignity. I was sorry to see her move to a different ATI about 1700 miles away, so it would be difficult to continue my treatment with her, I will miss her. Good Luck to you at your new location Rebecca S. So my 3rd and final Therapist /Director was Reena T. Third times a charm right? YES! Reena is phenomenal! Very passionate with her work, enjoys her patients, compassionate and is friendly, always smiles, is funny, answered all my questions and always asked how I was doing. Reena is energetic, knows her stuff and is eager to expose her talent and share with her patients. Although I did not have any therapy with PTA Leslie M, I have had interactions with her and she is wonderful. Wrapping it up (in a polkadot bow) , the staff here at ATI in Grayslake is the "BEST ONE COULD ASK FOR" no matter which therapist you have, they will "WORK U OVER"- (NO PUN INTENDED) to your recovery! Despite being noticeably understaffed, she moved from resident to resident with remarkable patience and professionalism, feeding those who needed assistance, checking in with others, and maintaining a warm and positive attitude throughout. Her compassion and work ethic clearly reflect someone who takes pride in what she does. While I'm not an expert in this field, I do work in a very similar profession and deeply understand how much compassion, commitment, and emotional strength it takes to provide care for others. Nicole embodies all of those qualities. If the facility had more staff like her, or even just more staff support overall, each resident would receive the level of care they truly deserve. Due to the staffing ratio of approximately one CNA for every sixteen residents (based on timeframe of observation). In memory care, where residents often rely heavily on guided assistance, this level of staffing makes it difficult to ensure everyone's nutritional and emotional needs are met. It's evident that there have been positive strides at Thrive, and with continued improvements, particularly in staffing, the facility could deliver an even higher standard of care. I have visited a few care facilities…read morein the last year, and this facility, long and short term living and nursing care, is new, clean and well maintained. The decor is a bit minimal and basic, although I was impressed with the therapy gyms, and the courtyard was nice. Rooms are spare and small, although bathrooms are wide and have roll-in showers. The message here is is take care of your health, people. If you are lucky enough to have healthcare, use it and don't neglect your checkups. Save for retirement and beyond. Rant over.

They have helped her to become more independent while keeping a positive mindset when she is struggling. I think what's most impressive is the entire facility works together to give residents a feeling of belonging and a sense of control whenever they can. The facility makes an effort to celebrate anything and everything which gives the residents something to look forward to. Today they took a group of residence out to see a movie. What a huge lift, but they made it happen. I have a lot of experience with long-term care facilities and this one really knocks it out of the park. In both cases, she was professional and friendly. While her physical therapy plans were demanding at times, she always had my back, literally, as she stayed close to me during some routines. At times, my performance was frustrating, yet Meggie offered encouragement. She provided materials to take home so I could continue the physical therapy plan she had developed. I have had other physical therapists. Meggie is the best I have had.

I really resented being passed off among physical therapists. I like to get to know a PT and stay…read morewith one person. Moving among PTs within a short 45 minute session seems ridiculous. I did not feel like the PTs cared about me and just wanted to log their time and process. I decided to quit and just do the exercises at home and I'm fine. Very expensive as well. Disappointing experience.
