Things broke down in my subsequent visit with her since her follow through with ordering physical therapy was delayed by several days. We agreed upon a treatment plan of only medication and PT during that visit. About two weeks after my visit with her I developed increased pain and reached out to her to adjust my medication. A gentleman called me the day after I left a message around 9:00am to get more information but when I had heard nothing by 4:00pm I called again. Surprisingly someone actually answered the phone which had never happened before during my several calls throughout my evaluation and diagnosis. But I digress. Apparently sometime during the day Ayumi responded that I needed to reach out to my primary care provider. I have no idea if I would have been called with this message the same day. I took this to mean that she had discharged me from her practice. I really would have appreciated it if she had told me that at our last visit because it is now 4:30pm on a Friday and my PCP does not work on Mondays. So I will sit here in pain until Tuesday. I cannot recommend this practice since they appeared to have abandoned me as their patient and I will be reaching out to the Board of Registered Nursing with my complaint.

However, if you're looking for a spine specialist who does everything possible to help you avoid surgery, Dr Heros is that guy. In 2010 I went to Dr Heros as I was contemplating lower back surgery. He told me not to go through with it, as there were too many issues with my back for one surgery to correct. I didn't listen and had the surgery from what I now know to be a quack neurosurgeon named Francisco Soldevilla. I am now permanently disabled as a result of that botched surgery. My wife and I continue to see Dr Heros. I respect his skill and and his recommendations.

The clinic environment is calm, welcoming, and thoughtfully designed to support the experience. The ceiling light show created a peaceful, immersive atmosphere, and pairing it with binaural beats made a huge difference for me. Each session felt safe and intentional. For me, it wasn't until the 6th infusion that I really turned a corner--but the difference in my symptoms has been significant. I canceled my appointment before I ever met with the provider because my experience with the office…read morestaff made me uncomfortable trusting this practice with my health. As a patient seeking help for pain, communication matters. Before my appointment, I tried to confirm the details but did not receive a clear response by email, text, or phone. When a medical office is difficult to reach before you are even seen, that raises serious concerns about how responsive they will be once you are actually under their care. After receiving no confirmation, I contacted the office. When I did not receive a confirmation of the cancellation, I called a second time and left a voicemail message canceling my appointment a second time. Despite that, I was later billed as a "no show", which is unacceptable. If the office fails to respond, fails to properly confirm an appointment, and then fails to process a cancellation, the patient should not be penalized for that breakdown. I cannot speak to the doctor's medical care because I never made it that far. But I can speak to the office experience, and it was poor enough that I chose not to proceed. For anyone dealing with pain, the last thing you need is added stress, unclear communication, and billing issues before care has even begun. This experience left me with the impression that the practice is more focused on collecting fees than making patients feel heard, supported, and properly cared for.

Dr. Khemlani met me for my first appointment, accommodating a scheduling conflict to make sure I was seen in a timely fashion. Dr. Kehmlani went to bat for me with my insurance to ensure I got an approved prior authorization. He truly had to go the extra mile. Further, the next day I was seen for the actual procedure. I was honestly terrified. He took the time to stop and speak calmly with me, reassuring me all would be fine. He took time. He showed compassion. He was above-and-beyond professional... but more importantly, he was kind. Providers like Dr. Khemlani are FEW are FAR BETWEEN. Providers that truly express empathy and grace are a rare find any more and I'm quite lucky to have been referred to him. Even better, he has a ridiculously good sense of humor and made the rather unpleasant procedure 100% tolerable... painless one would say. I can't speak highly enough of Columbia Pain Management and Dr. Khemlani. He and his staff were all spectacular and I appreciate everyone for their great care. I can honestly say, I am looking forward to returning for my follow-up appointments. Thanks, Dr. Khemlani. You're the best!

I had to have another…read moreinjection after two back surgeries and this office is outstanding at making you feel comfortable and calm. I highly recommend Dr Khemlani! I've been seen at the Milwaukie and Tualatin offices and either is great! I have no fear of my other scheduled injections.

The Dr treats pain. But, the office is not run well. You can never call and get someone to answer…read more There is 1 Dr, his Asst and a nurse. Yet, 2 receptionist and several more people in the back for ??? You text to get an appointment The Dr sees you then prescribes a treatment. If you get medicine, sometimes it takes weeks to get. You then see the Asst. from then onward. then the receptionist calls to give you another appt 5 weeks later so you're always out of meds a lot. They also make you fill out a daily 4 questionnaire w/o telling you they charge you for this. No one checks this information. In my case they would request from my insurance company for treatments i hadn't agreed to, they charged me for extended appointments when I saw the Asst for less than 10 minutes. I finally told him I was done coming, yet I was charged for 2 more months of visits. I had to file a claim with my insurance company. I finally got the amount I was supposed to pay from over $1,000 to $90. I would avoid this office. The receptionist is nice.

He would not listen, and someone in this field should be better behaved and show empathy, kindness, not an accusatory tone. He certainly thought a lot of himself, was gruff & despicable. His office staff was cordial and friendly, he does not belong there and needs to be fired.

My initial issue was I learned, with one individual there, and not the Institute. Am returning for more help from them and now can recommend them highly - in spite of the one staffer. Until yesterday's scheduled procedure (1/31/18) that is. Was informed after fasting and getting there at 8:15am by their anesthesiologist, that she was refusing to perform the procedure because "it was too difficult waking me up the last time." This was either my 4th or 5th procedure by them - there was nothing new going on with me - I still needed diazepam because of PTSD - and was denied service. I tried submitting verbal complaint so whoever's in charge would be made aware of how one staffer affected their business, but my calls have not been responded to. As my first review revealed the difficulty I had in getting my calls returned, so it continues. That particular staffer is either always out 'running errands' or just takes her time returning phone calls perioid. Left three messages over several days while awaiting a call-back. I'm done.

I was sent to him by the VA due to chronic neck and back pain with x-rays and MRIs to back it up, as well as a couple of sports-related injuries, including a poorly-healed broken elbow and a complete rupture of the gluteus minimus and medius (having surgery on that in a couple of weeks). He only cared about the neck, didn't want to hear about anything else. Zero eye contact, no facial expressions, clipped and monotone. I asked a couple of questions but had to repeat myself because he ignored me. When he was done fiddling with his computer, he did have a plan; come back for some nerve blocking (or whatever it's called) which should work for 4 HOURS. Then keep a pain journal so they know if they're in the right area. So... nothing today? "No." Why not? "We need authorization from the VA. Could take days, could take weeks. Don't know." Can I get something for the pain in the meantime?? (No history of abuse or addiction, all injuries well documented.) "No. Nothing. It was nice to meet you." Then he walked out. The office staff were nice, but you couldn't pay me to go back to that place. From their parking lot, I reached out to the VA to find a different provider - which took about 10 minutes total. They were eager to take my statement and apologized for Oleg's behavior. Oh, and the authorization I needed to wait for? They had it the entire time. Everything he wanted to do was included in the referral that had already been approved.

on Feb 19, 2025, I was with my daughter when she needed to go to the ER…read more First of all the time it took from the triage to actually have a meeting with a doctor was completely unacceptable. Arrived at 4:30pm-10:45pm, over 6 hours from the beginning to end. Yes is was fairly busy but I didn't think all that busy. Secondly, the restroom and the examination room were dirty, I mean gross. The underside of the toilet seat needed cleaned and the floor needed mopped. Worst of all, after the waiting and getting to the exam room, there was what we thought was fecal matter on the control panel of the bed. I cleaned the matter up using rubber gloves and wipes that were on the exam room. I am so happy I have different and better health ins.

Days to resolve a broken toe…read more My husband fell down the stairs in our home with our toddler. This happened on a Sunday. I was pretty sure he broke it, but he decided to wait until Monday morning to call the doctor to schedule an appointment. Monday morning rolls around and he makes the call at 8am. Dr. Goswami, not his usual general practitioner, saw him that same day. Great! She orders him to get an Xray done. After the Xray, we wait for her assistant to give us the results that it was, in fact, a broken toe at the joint area. My husband has been in a lot of pain hobbling around, so it was nice to hear that we would get this resolved immediately. We were told he would get a call from a referred orthopedic and someone would drop off a boot at our home for him to wear (at 10am Tuesday). Tuesday rolls around. We've been waiting the rest of the day Monday and Tuesday morning to get a call from this orthopedic. No call. My husband calls in to check up on the status and the assistant tells us that Dr. Goswami called in to the wrong referred orthopedic. The orthopedic was for a broken knee/hip when my husband needs one for his foot. So, essentially we could've waited forever if we didn't call in because it was the wrong doctor. My husband ended up spending 2 hours out of his work day to find the right orthopedic to call covered under our health insurance. 2 hours. That's 2 hours of figuring out who to talk to, talking on the phone, and then leaving a voicemail with that orthopedic. Meanwhile, at 10am, the lady comes to our home and has two types of prescribed boots/shoes. BOTH are wrong. They're for a broken ankle. One shoe my husband tried on has his toes exposed! So much for this immediate service. We're at the step of now going to the correct orthopedic on Wednesday morning and approaching day 3 of his injury/healing process. When someone has a fracture, it shouldn't take days to resolve. We're hoping the doctor doesn't have to re-break his toe to set it correctly on the 3rd day. We find it difficult to trust our bodies with medical professionals when they cannot provide an expedient, accurate service.
