Previously, she was in a different rehab facility. From what I can tell, Rockville is head & shoulders above the rest! Due to the virus, I have never been inside the facility. The exterior is well-maintained. From what I can see, the Reception area is modern and beautiful. My angel says the rooms are VERY nice! Given that I can't see my loved one, communication is so much more important. I want to full-throatedly commend the following people: Susan Welge (Administrator), Martine Samuels (Director of Nursing), Madonna (Nursing Supervisor), Nanci Waldman (Director of Admissions), Wendi Richards (Director of Social Services), Iana Casareno (MDS Coordinator), Amanda Fischetto (Executive Assistant), Debbie Bowden (Recreation Director), Kristi Barth (Director of Food Services) and Faith (Dietician), Pia Fucarino (Rehabilitation Director) and Dana (Speech & Swallow Therapist.) If I forgot anybody I am sorry. Also want to thank the various nurses and nursing aides. One in particular (Kathleen I believe) recently gave my angel her first shower in months. That is how long she was in and out of hospitals, er's and rehab facilities. Residents get sponge baths daily. Debbie (along with Kathleen) cut the knots out of her hair. It was knotted BEFORE Rockville. Rockville saw it and swiftly addressed it. Wish all companies were run like Rockville! The staff calls ME, instead of me having to call THEM! They are very proactive and always available. When she first arrived, I spoke with Nanci Waldman In Admissions. She explained the process. Then Susan Welge called, introducing herself. Wendy Richards was in touch soon after. Then Debbie Bowden. Debbie also set up a video chat so that I could see my sweetheart. That did us both a world of good. Thank you much Debbie! Get weekly calls from Susan and the staff. On a recent Sunday (her day off!), Susan went in to comfort my angel who was having a hard time dealing psychologically with her conditions. FYI, residents are tested for covid once a week, staff twice a week. Everybody is just a call or email away. They actually respond to you. They do not brush you or the resident's concerns off. Issues get communicated to management and resolved in a timely fashion. Everybody imho has been wonderful. Iana is great explaining medications and the discharge hospital's instructions and interfacing with doctors. Martine said if I have any issues to please have her paged. We have communicated many times by email and phone. Got several calls from Kristi and Dana about dietary restrictions and swallowing issues. Amanda (who will likely answer your call at the Front Desk) is also the Transportation Coordinator. You can hear the smile in her voice! She is very sweet and kind! She arranged an ambulette, driver and aide to take my angel to 2 important medical appointments. One is in Manhattan and one local. I'm amazed at how quickly she set them up! Doctors visit Rockville weekly. Susan even arranged for a company called Sight Rite to come and check her vision. They did. They are also able to dispense glasses. PT and OT have been fantastic. Goal is to get her walking like she did before her hospitalizations. They got her walking 20 steps in her room. That is a miracle! They also strengthen her legs and arms with resistance bands. She is making progress. Eventually they will get her to the on-site gym for more intensive PT. If I ever need a place like this, Rockville will be my first choice! Can't say enough good things. The communication is of utmost importance to me. Susan assured me my heart and soul will get plenty of TLC! Check them out at rockvillerehab.com. I was on a low sodium heart diet and all they gave me was ultra processed…read morefoods, canned foods loaded with sodium, and chemicals, artificial and genetically modified crap. The care was lacking in quality and quantity. I would ask for assistance for cleaning a wound i could not reach and all i got was " thats not my job" i was supposed to take 2 showers a week and had to beg for one. They couldnt even get my blood tests right to adjust my blood thinners. Terrible place .

To say the least, as my mom's care taker, I was nervous and unsure of how she would fare at a center that may not have gotten the best reviews. I am more than happy to report that my mom, having left the center a few weeks ago, is stronger than she has been in a long time and often mentions how much she misses EVERYONE at the center. She talks about her long talks with Joanne and how Nurse Terri's afternoon "stop ins" were moments she still wishes she could live today. The staff, including and not limited to those behind the scenes such as billing etc., helped us in every way they could. When I say us, I mean myself as well - they helped me navigate my mom's insurance situation with seamless ease. Sheryl would give me help beyond measure, calling me on my cell, allowing me to maximize what should be available to and for mom. When I had concerns regarding treatment, I was contacted personally by Dr. Ali. He would be keep me informed in the weeks thereafter making sure she was progressing, moving her closer to going home. The physical therapy crew made her laugh daily while she was "working out" and getting, as she likes to call it ,"IN SHAPE for beach season" :-) The smiles from minute one, were always a welcome experience for mom - she felt cared for not just on a physical level but an emotional one. Thanks to the beautiful staff at the center, mom is physically stronger and healthier. Emotionally, she misses them, speaks of them often and fondly. Overall, should she need TLC from a rehabilitation center again, there is NO question, we will chose Nassau Rehabilitation and Nursing Center.

I only now learned that during…read morethe entire time she was there, they gave her lettuce and tomato sandwiches for her meals. No protein, no beans, no pasta, no dressing, no hot dinner - just 2 slices of bread with a lettuce leaf and a slice of tomato. No certified clinical dietician would have approved that diet. I hope the person who decided to punish my sister for being a vegan gets a healthy dose of karma back at them.

Many of the nurses seemed unhappy to be there and acted like they hated actually having to do their jobs. The call button was essentially useless, with requests often ignored. Patients were frequently left sitting in wheelchairs all day doing nothing!, i witnessed 2 nurses tell a neighboring patient she couldn't go in bed, so she ended up sleeping, almost falling out of her wheelchair. She then woke up and even tried to get herself into bed, and she was nowhere near capable of doing that so i had to wait for the next round of nurses to get her the help she wanted; bc some shifts had the absolute worse nurses. Communication throughout the facility was very poor. Doctors were rarely available, and getting updates or answers was impossible. Dr Timpone is prescribed my mother another controlled drug on top of her nightly controlled drug for "pain". No one even told me about this new med until i found out at discharge and is the reason why my mother felt so poorly there, dizzy and had nightly panic attacks, but everyone was so dismissive of how she felt. The only positive aspect of this facility was the daily PT and OT. The therapists were caring, professional, and did a great job. Unfortunately, the therapy staff cannot make up for the lack of compassion, poor nursing care, and neglectful environment. Shout outs to nurse Maria, Fahad, CNA Josephine, PT Roma and OT Sharon. It is excellent! The therapists are caring and they help each other whenever needed . My PT was Chana and she was excellent and caring. Also kudos to Johny who stepped in when Chana wasn't available . For OT Sharon and Jackie were also excellent! The nurses and CNAs were largely competent and caring. Special kudos to Melissa, Laura, Arianna, Steve, Claudia, Monica and Maria. The one serious problem is the call bell often goes unanswered for a long time. The food is good and the recreation program is good. I must say that the social workers were useless and even harmful to my recovery. My condition is such that I will be wheelchair bound for a time after leaving Orzac and that was known from the start. I just realized that NICE ABLE RIDE is available to me through a friend who is in another facility and they advised him of ABLE RIDE and are helping him fill out the app. We just called the social worker at Orzac and she told us that it was my responsibility to inquire when I was at Orzac and that she could no longer help me. She knew that I wouldn't be able to walk when I left Orzac and should have quided me on what I needed upon discharge. Theapplication process is lengthy and it could be more than a month before it is approved, not to mention that I may have to leave the house a few times to complete the application which would mean hiring an ambulette!

    This rehab Center has a terrible, obnoxious, haughty and rude, nurse practitioner. He was in charge…read moreof prescribing and coordinating medicines for my wife during her recent stay there. He never consulted us or discussed pros and cons of the medicines and changed them without us knowing the side effects or interactions among them. I had to constantly go and ask him about them. One day he got extremely angry and yelled out "How many times do I have to tell you. Get out of here. I'm not taking care of your wife. Take your wife and go. Get out. Go." I was totally shaken and stunned. I told him he was not the owner. I wanted to talk to his boss, the medical director. He flippantly answered." Go ahead and talk to him. I am not his son." I complained to the Administrator and the medical director later. They both said they were reassign his area, but really didn't since he was at his nursing station whenever he worked,on our floor. They didn't fire him or even admonish him as I could tell. This kind of egregious professional misconduct should not be tolerated anywhere in rehab centers. These employees work there because of patients and rehab centers are open because of patients. There were times when medicines would be delayed, even after asking for them up to an hour. All in all most of the supervisors, nurses, aides, PT staff, admissions staff and receptionists were very nice people. The food is not good and we had to bring it from outside most of the time. I'm giving this place a one star because this nurse practitioner ruined everything for us. We were in the middle of rehabilitation, so didn't want to change Centers. I'm thinking of complaining about him to the New York State education department, National Health Care Associates and Medicare among others. Anybody considering putting their loved one here is cautioned about a similar experience to ours and should think about it very carefully.

My SLP definitely knew what she was doing and was able to get me on a modified diet each time. The quality of care was excellent. The office experience was also pretty good. The receptionists were overall kind and helpful. Then in 2025, I went back for outpatient wound care. The quality of care overall was pretty good. Although I saw the PAs more than I saw the doctor, and when I did see the doctor, she would only spend like five minutes examining the wound. Also, while the doctor was nice, they need a refresher about bedside manners. The office experience was good though. The receptionists were really nice and helpful. There were two issues I did have. First, the waiting room was too small. Many times when I was there, the waiting room would be packed and it was difficult to find a spot for my powerchair. The other issue was that many times I'd have to wait an hour or more to be seen by the doctor.

    IF I could give this place zero starts I would. II am extremely disappointed with the experience…read moremy parent had at Fulton Commons. From the moment we arrived, the condition of the room was alarming. The bedside nighttable had a white film on it that looked like dried milk. I ended up cleaning it myself with hand sanitizer and paper towels because I didn't want to place any personal items on a surface that looked so disgusting. The wall beside the bed was filthy, covered in dried blood and other stains (see attached picture). The staff was another major issue. The workers at the front desk were rude and acted like you were bothering them whenever you asked a question. If they even bothered to answer, it was with an attitude. On the positive side, the nurses, physical / speech and recreational therapists were nice and professional, but unfortunately, they were the exception. The private aid we hired to sit with my parent had her own disturbing observations. She said when she requested a diaper change for my parent, she was met with an attitude and the diaper wasn't changed for a shocking 7 hours--from 11 am to 6 pm. This is totally unacceptable, especially for someone who needs consistent care. After seeing all of this, it's hard not to conclude that this is a Medicare-draining facility more focused on cutting costs to maximize profit than providing decent care. A Cash Cow for the owners. If patients ask to be taken to the bathroom, they're told to just relieve themselves in their diapers. This is not only disrespectful but also likely contributes to unnecessary health problems like UTIs. If I had known how poorly run and filthy this place was, I would have never considered placing a loved one here. It's clear they're more interested in lining their pockets than actually providing quality care. I deeply regret this decision and would strongly advise others to look elsewhere. Would not recommend.
