My aunt was here for a month. Opinions are mixed. Jeni from the Business Office and the PT/OT staff…read moreare wonderful. She made much more progress here than at the previous rehabilitation center. My concerns are around an overnight weekend male aide who left her on the commode for 4 hours. She would push the call button and then he would eventually poke his head in and say "OK" leave and never return. He also didn't want to deal with changing her bed or getting someone to help get her up so he would leave here there. My other issue is with the older woman who works the reception desk on weekends. She is very rude and likes to roll her eyes and sigh at everyone. She doesn't want to do her job of opening the door so she needs to do a different job.

I was considering this facility for my mother's care, but after my visit, I decided to look…read moreelsewhere. The interactions I had left me with little confidence in the professionalism or compassion of the staff. One staff member who I believe said he was the director of something, he came across as completely uninterested and indifferent. I felt like my concerns didn't matter and that I was just another number instead of a family trying to make an important decision for a loved one. I also observed another staff member who was passing out meal trays. She appeared unprofessional and gave the impression that helping residents or interacting respectfully with others wasn't a priority. The overall atmosphere felt cold, unwelcoming, and lacking the compassion I would expect from a place caring for vulnerable people. Choosing a nursing home is one of the most important decisions a family can make. I was looking for kindness, professionalism, and genuine concern for residents and their families. Unfortunately, my experience didn't reflect those qualities, so I chose to continue my search elsewhere.

My mom needed a place to go in between a hospital stay and home care so she spent nearly 2 weeks…read morehere receiving needed physical and occupational therapy. Although she didn't care for it much (loss of independence, etc.) I thought it was a very nice facility. Her room was large, there was a big tv, tables and chairs in the room for family and a nice large private bathroom. The lobby area had places to sit with vending machines, puzzles, etc. Family can choose to wash their loved ones clothes on site in the nearby laundry room. The staff seemed very attentive and caring. The meals were hot, healthy and smelled great. We especially liked the "healing garden" located in the center courtyard for a place to get some fresh air and sunshine. Although no one probably wants to be in a place like this, when needed I thought they were very good.

From personal articles coming up missing to two nurses in particular involved in an abuse situation that required an ombudsman and literally zero actions taken by the facility other than to cover their collective tails. I would take someone literally anywhere other than ambassador health. I am giving one star because of the two rock star nurses (they know who they are) and respiratory. The facility should be shut down. It is quite literally that bad.

They met and exceeded every expectation we had! Mom flourished with the socialization, the activities, and the monitored therapy!! She got stronger, more confident, and every day said "it was a great day!" The staff is excellent - very caring, very professional, very attentive! Lots of hugs every day! The Recreational Therapy Director Larry was absolutely fabulous! He has so many different things for the residents to do, and every day there's something organized morning, noon, and night! You can tell everyone there truly cares about the people entrusted to their care. The residents are so happy and friendly - we met them all, and nobody had a bad word to say about their care or their living situation. The rooms are all private, comfortable, quiet and clean. It was our first experience with a Skilled Nursing Facility, and we were all completely impressed and eternally grateful that Mom was able to spend time there!

And it is not just because they are friendly and engaging people (which they are), but because they also push each one of their clients to work their hardest and to get better as soon as possible. Sandra 123 Your Street…read moreYour City, ST 12345 (123) 456-7890 Welcome to AZRIAA HEALTHCARE AND NURSING HOMES. We're so glad to have you join the team! This guide is meant to provide team specific information to help you get off to a great and productive start. WHO WE ARE Our mission Our main goal is to provide privacy to a degree that you may not even know who we are. For example; we sometimes allow residents to wear their clothes for a couple, 3 days. It's their choice.. We won't rush in and help with teeth, or other activities of daily living. We feel that is just too nosy and forward. PRODUCT Our product is cheap care for your elderly loved ones. We find that paying staff more and hiring people who just 'care too much' only leads to favoritism. We must provide the same care to each resident. As little as possible. Meetings: We have weekly meetings but usually don't do much unless a client's family member attends. Our job is to reassure families that we are doing a laundry list of tasks we ( just don't do). ONBOARDING TASKLIST Week 1: In week 1 you will learn the most important lesson. Do as little as possible, we want you to always appear busy but loud talk and laughter is encouraged so families hear that we are having fun. Week 2: In Week 2, we want all of our nurses to understand and utilize chemical restraint. This allows aides and nurses to avoid spending extra time learning about de-escalation methods, deep breathing, and walking away. If you care about your loved one, just remember, we will know when it's time for them to leave this earthly home and we will help your loved one transcend. Run from Azria care. If you can't have a granny cam, think about what nursing homes are afraid you 'will' or 'won't see. They gave my mom an antipsychotic, rexulti which made her unresponsive; no eating, no drinking. No sustenance. My mom's POA had clearly stated we did not want my mom on that medication. On the medication she was unresponsive, She was a bit responsive for a couple of days after they took her off but she was dehydrated and without food. She then got covid. Or that's what her chart states. The state substantiated our case. Azria will be seeing a lot of fallout from this negligent and abusive homicide. Don't get me wrong, we are all glad mom is out of there and free from neglect and abuse. Similar situation? Call me at 8167259083. Sandra

I was very skeptical when I first walked into their office. I mean, I'd already done this how many times?! Now, three months down the road, I could not be more grateful to Jake at Horizon. He didn't just do some stretches, pop a TENS unit on me and send me out the door. He tried many different therapies and devices until we found what worked for my neck pain. When it came time to get a home unit for me to continue the work on my own, he worked his butt off to get everything squared away before the end of the year so that it would be completely covered by my insurance. I will never be able to put into words how grateful I am to have my normal life back without constant pain getting in the way. I will never go or recommend anywhere else again!
