Do not bring your loved ones to this facility. My parents have been here for a year and a half and…read morehere is what they have experienced: * The quote for services was written on a blank piece of paper. These were the services agreed to prior to moving in. After moving in, these prices changed. * Services not asked for were added without my parents' knowledge, leading the facility to charge more than $18,000 extra over the space of 4 months. This was only resolved through months of communication with the Brookdale management. To this day, the billing is not standardized, itemized, or consistent. * The food that is served is substandard. Think about high school cafeteria food. Some meals were not delivered, causing my father to have to order out. A large percentage of the breakfast meals were delivered without toast. Missing items per the order submitted is a regular occurrence. * The cleaning of the apartment is supposed to take place weekly. Sometimes weeks have gone by without a cleaner coming to the apartment. Cockroaches have been seen inside the apartment. * The care of my mother who is bedridden is supposed to be completed by 2 caregivers as per the contract. This has rarely been the case. This is my personal observation by my weekly visits on Sunday mornings. 90% of the time, there is only one caregiver, forcing my father who can barely stand to help. This puts both of my parents in jeopardy. As a side note, the caregivers that come to my parents' apartment are great with them, giving them the best care they can though in many instances are untrained and have had to be shown how to care for my mother by my sisters. By the words of the caregivers themselves, they are constantly shorthanded. Look elsewhere!

I am in my early 50s and I stayed at this place from March for a whole month until April and honest…read moreto God I was terrified, but it was the BEST experience! I WOULD RECOMMEND PHX Mountain to anyone! The care is TOP NOTCH! I'm on the younger side (For once) to be in a Skilled nursing home! I took care of my father and mother before they passed so I know nursing homes ALL OVER PHOENIX AREA including Tempe, Chandler, Mesa, P.V., I know how scary and terrifying some of these places are! That is not the case for Phoenix Mountain on Tatum. The nurses like nurse Barry and nurse. Tristan are amazing. I can't say enough great things. The rooms were clean. The food was fantastic. OK there was a couple days a couple meals are weird, however, 90% of the time the food was phenomenal! When I was there, they had a fantastic chef! The physical therapy team outstanding, the nurses aids or the CNA's down to the housekeeping crew we're incredible. They kept that place immaculate. Although the nurses are overworked, they need a raise and they need to have a one nurse per hallway!!!!! then it will run so much smoother for the nurses. The nurses worked their buns off! and I am so appreciative of that and I am so grateful! I cannot tell you how pleasantly surprised I was of how great this place was!, the people that lived there and that were in my same situation were awesome. My room was lovely. I overlooked a fountain and had lots of sunlight and I had my own bathroom. My roommate was incredible Patricia. She's a 93-year-old blind woman who is the most interesting person. She was awesome, again the place was fantastically clean! It looks a little old older, but it makes up for everything of how great the nurses are and the care is, it was great. The food was great. They even have an area outside where they had a big barbecue and we had hamburgers and hotdog for everyone and they have tons and tons of activities. If that's what your people are into, they had games they had workouts. It was incredible. I can't say enough good things and I'm only in my 50s and I was so surprised but so grateful! I also did have a loved one who stayed there and she had a good experience there too!, so take it from me, I was just there the last day of February 2026 for an entire month until April and I cannot say enough good things about this place! Hit me up if you need more info. Hats off to Phoenix Mountain post acute on Tatum and Thunderbird fantastic! Kind Regards Rosy Slater 714-910-1943

I worked at Phoenix Mountain Post Acute as a CNA, and unfortunately my experience there was very…read morediscouraging. The facility is constantly short staffed, which makes it extremely difficult to provide residents with the quality of care they deserve. With so few CNAs on the floor, call lights would often go unanswered, residents were left waiting for long periods, and basic care like hygiene and toileting was sometimes delayed. Many of us CNAs voiced our concerns, filed complaints, and even reported the building to the state in hopes of bringing positive change. Instead of addressing the issues, several staff members who spoke up were terminated. It felt like management, corporate, and even some of the nursing staff were more focused on protecting themselves than supporting the caregivers or prioritizing resident safety. There are good caregivers who truly care about the residents, but the system in place does not support them. The turnover rate is high, new hires often don't receive proper training, and the overall culture makes it hard to provide the compassionate care that families expect and residents need. Until major improvements are made in staffing, management, and overall accountability, I cannot recommend this facility as a safe workplace or as a place for loved ones to receive care.

The initial call I received from Sara at Centerwell to inform me of the in-home services was great!…read moreSara was very thorough and knowledgeable with the information she provided. I received a call from the Nurse (Joyce) to schedule my first appointment. Joyce showed up a little later than scheduled. I did not hear back from Joyce for a second appointment, however another nurse (Janet) contacted me, but I no longer felt I needed to see a nurse during week #2 since I was already seeing my primary doctor. Karla, the in-home OT contacted me in week #2. I saw her once, because we both decided I no longer needed OT. My PT Stasia visited three times. She was very prompt when scheduling appointments, and always showed up minutes before the scheduled time. Stasia is the perfect in-home PT for any patient as she is very knowledgeable and caring. I am grateful for having Stasia for my PT, and I highly recommend if you get the opportunity to use Centerwell Home Health for your in-home services, ask for PT Stasia- you won't regret it. I rated 4/5 mainly because of the awesome service I received from Stasia. Kudos to her, and thank you Centerwell Home Health.

The only time we can get help is when Jesus is available, otherwise they either ignore us completely, or respond telling us to deal with it, basically. Would not recommend. Exceptionally poor customer service and they do not preform the job they are paid to do.
