They tried to send a mobile doctor out whose car broke down, which is not her fault however when coordinating my mom's discharge recently from sepsis inpatient treatment, they texted me yesterday, saying to coordinate when the nurse can come out then all of a sudden today they tell Kaiser that they can no longer service my mom as a patient and give us no reason other than there's no nurse in the area, which is not true. We're 15 minutes from Glendale and there's definitely providers that could come give my mom care this completely mismanaged my mom's discharge plan and has left me at a loss. I am 132 pounds. I cannot lift my mother alone let alone Help her take a shower or do the things she needs to do. They should've been more transparent about not being able to accept my mom so l would've been coordinating other healthcare the last seven days this is declining my mom's health. I hope you guys will reconsider how you treat patients in the future and not leave them in the lurch this really really really hurt my feelings because I thought you were a top-tier service but now I think you guys just dropped the ball and lied to me. I spoke with the owner the other day. My mom saw her doctor on Sept 7, and for some strange…read morereason, Affiliated says they got the referral on Sept 22. I find this difficult to believe because her doctor is very thorough. That being said, once the referral was received, I got a call from Russell, at Omni Physical Therapy. He left a message that a case manager and physical therapist would be calling to start PT. I never received a call from the case manager, but the PT ( Josh) called on Sept 23rd. I promptly returned his call and left a message that we would like to be scheduled for therapy. He never returned my call, nor did he respond to 2 text messages. All the while Russell (who stated that he did not have a manager for me to speak to, that he in fact was the manager in charge of assigning a case manager to my mother). I told him that my mom still had not been scheduled to start PT and no case manager. He told me that "I did my job, and it is not my fault". I admit that I disconnected the call because I can't stand people that make up dumb excuses. I did get a call from a case manager within 30 minutes because Russell figured out that I was upset and he actually did drop the ball. Jane the case manager told me that she did try to call me multiple times, but I called her out on her lies. There were never any call logs on my phone, which was the point of contact number. I asked Jane when she was assigned to my mom's case and she told me Sept 23rd. That day of our conversation was Oct 3. Lies. I was only looking for 2 things- physical therapy for my mom and an apology for dropping the ball. The lies and excuses made me request a different Physical Therapy company. Affiliated needs to contract with actual health care providers that are in the service field of providing necessary treatment for those in need.

The Intake Nurse who showed up at my home had NO nursing credentials, albeit taking my temperature and blood pressure while fumbling through it. She explained that she was still in Nursing School and had been contracted by Affiliated Home Health Services to report to my home. She stated she had no knowledge of AHHS personally. This company has NO physical location, per se, as they are a "mobile" service who contracts their staff, i.e., physical therapists, etc., from outside/independent sources, such as from the 'school' referenced above. When I attempted to contact the owner, my phone call was answered by a "representative," but days later identified himself as the "owner" when I called back. I immediately DISMISSED their services and WOULD NOT RECOMMEND them.
