My mother previously received care from 24/7 Care At Home. However, when she changed her primary care physician, we lost access to 24/7's home health services. At the time, I did not fully understand the relationship among primary care physicians, hospitals, and home health providers. I simply followed the discharge instructions we received, trusting that all healthcare professionals involved were working toward the patient's well-being.
More recently, I became dissatisfied with the primary care arrangements for my elderly parents. I had believed their physicians were the only option for coordinating care at home. One Saturday, Tam Tran, Patient Support Manager, called and asked whether I would consider giving 24/7 Care At Home another opportunity to care for my mother. I was very pleased to do so because I remembered the kindness the staff had shown my mother in the past. On one occasion, when I was unable to be with her because of work, members of the 24/7 staff visited and celebrated her birthday. That thoughtful gesture was deeply meaningful to me because it demonstrated genuine compassion for an elderly and vulnerable patient. It also gave me confidence that this level of care extends to other members of the community. Over the years, 24/7 staff occasionally contacted me to ask whether they could assist my mother. However, obtaining a new order from her primary care physician each time became difficult, so I eventually stopped pursuing those services.
Ms. Tam Tran is exceptionally hardworking and responsive. I have been able to contact her for assistance with matters ranging from doctors' appointments and pharmacy locations to medical supplies, including on weekends.
Ms. Michelle Phuong Nguyen, our parents' case manager, has also been remarkably kind, dedicated, and professional. One Sunday, after my father was discharged from Orange Coast Memorial, he experienced a rapid heartbeat from early morning through midday. My stepmother called me because she was unsure whom to contact for assistance. I reviewed the information Ms. Tam Tran had previously sent me and called Michelle. Michelle answered promptly on a Sunday and said she would call me back within five minutes after reviewing my father's medical history. She called back within three minutes, provided guidance to my stepmother regarding the medication my father should take at that time, and said she would follow up again in two hours. My father's heartbeat improved after the medication and had returned to normal by the time Michelle called to follow up. I want 24/7 management to know how professionally and attentively its staff members carry out their responsibilities.
Ms. Hanh Nguet Le, Patient Support Specialist, is another example of the excellent support our family has received from 24/7. She called to welcome our family and asked whether there was anything she could do to assist us. I mentioned that my father-in-law had previously requested a medical bed through his former primary care physician and had since changed doctors. I asked whether she could help, and Ms. Hanh Le immediately offered her assistance. About a week later, she called to confirm whether we had received the equipment. When I told her we had not, she apologized and promised to expedite the request. Within a few days, the bed was delivered and set up. Ms. Hanh Le then followed up again to make sure the bed was functioning properly.
Ms. Thao Tran is one of the finest nurses our family has encountered. She cares for all four of our elderly family members: my mother, my father, my stepmother, and my father-in-law. She travels throughout Westminster and Santa Ana to provide their care. Knowing that my father had recently been discharged from the hospital, she spoke with him with patience and compassion, organized his medication for the week, and made it clear that his well-being mattered to her. My stepmother has serious diabetes, and Ms. Thao Tran checks on her frequently. On another occasion, she needed to leave promptly to check on my mother after a fall had caused a wound on her back. Both my father and stepmother speak very highly of Ms. Thao Tran and describe her as the best nurse they have had.
Based on my own interactions with Ms. Tam Tran and Ms. Hanh Le, as well as feedback from my mother's caregiver, my father, my stepmother, and my father-in-law, I can confidently say that our family considers 24/7 Care At Home to be an outstanding home health provider in Orange County. Its staff members have consistently demonstrated dedication, professionalism, and compassion in caring for our elderly family members.
