    Dr. Tariq is both knowledgeable and very kind. He listens to all my concerns and has been very accommodating, can't recommend him enough

He is a consummate professional and has dedicated his life to not only Veterans but all his patients. He served the VA Orlando as the Chief of Neurology for over 14 years and has now gone into private practice but not without ensuring he is still contracted with the VA as a community care provider. He is also a professor at UCF and works closes with the RosKamp Institute in Sarasota ensuring Veterans exposed in military service get the help they need. He believes in patient care FIRST as he spends a significant amount of time getting to know his patients, examining and evaluating them for ongoing care. So many times in the medical community you are made to feel terrible about whatever is going on or you are gaslit by the doctors you turn to for help. Dr Hoffmann provides individualized care, treatment, and compassion in all that he does. He is truly a HERO among those who seek out his help. He advocates and goes the extra mile for his patients to ensure they can live their best lives. While the VA throws pharmaceuticals at Veterans like myself, Dr Hoffman looks for alternative ways to manage physical symptoms, and pain. He is a wealth of knowledge and brilliant minded while remaining truly genuine and down to earth in wanting to help all who seek his care. In my lifetime I don't think I have met a more caring, compassionate human. His knowledge and education are impressive and his patient care comes before anything else. I specified medication I've taken for over thirty years for thyroid issues on the intake forms. I told the intake nurse about my thyroid medication, then told the Dr. about the medication. After a VERY brief interview he prescribed a medication which I looked up when I returned home. Warnings were prevalent on numerous web pages that the medication he prescribed should NOT be taken with my long term thyroid medicine. On his webpage is a contact e-mail called "helpdesk" I asked two questions 1. Why prescribe this medication 2. Can I have a lite beer once or twice a week (he had never mentioned the adverse effect it could have) I wrote them four times asking why and is another medication available. Never receiving an answer to my simple question I called twice they called back and refused to answer my question, telling me the Dr. only will talk in an office setting. So I was prescribed (for me) potentially dangerous medication. And he won't respond on the helpdesk. Obviously he never read or listened to the three advance warning that I can't take the medicine he quickly prescribed and won't take responsibility for what could occur if I didn't do an internet search.

    Update to previous post…read more 6/17/19 - Called today to report some concerns about the medications Dr. Sam has my son taking. The receptionist became irritated with me and claimed they have not appoints now this week. Also - they will stop taking Cigna on July 21st and haven't sent out any information about this change. We have been u see his care for over 3 years. Dr Sam is great. His staff, from desk personnel, reception, office managers have all been terrible. Why we have struggled to continue to see Dr Sam is our mistake.

First, communication from the office was extremely poor. I never received a confirmation call for my appointment. When I asked the receptionist about it, I was told they had called, yet I had no missed call and no voicemail. When I asked why a voicemail wasn't left, the receptionist was unable to provide an explanation. Additionally, I had personally called the office multiple times prior to my appointment and could not get anyone to answer the phone. When I arrived for my appointment, it became clear that Dr. Shahid had not reviewed my MRI or even had it available in my chart. As a patient seeking neurological care, I expected my physician to review my diagnostic imaging beforehand. Instead, I felt like I was starting from zero. During the appointment itself, I felt dismissed, unheard, and unsupported. Dr. Shahid did not take meaningful time to understand my symptoms or concerns. When I tried to explain what I've been experiencing, I was met with dismissiveness rather than curiosity or compassion. There was no effort to truly listen or understand what I'm going through as a patient. What was most shocking was being told that this was a "professional office" and that I could leave if I wasn't happy. Patients seek neurological care when they are vulnerable, scared, and looking for guidance. Being spoken to in this way was deeply unprofessional and lacking in empathy. I came to this appointment hoping to find a neurologist who would listen, review my records, and work collaboratively with me. Instead, I left feeling dismissed, disrespected, and completely unsupported. I cannot recommend this physician or office based on my experience.

It did take me a long time for someone to call me back for an appointment the first time. I went to the office maybe the almost end or middle of March and had to go back The beginning of April. I had some issues where I have certain medical diagnoses which restrict me from a normal MRI machine . The MRI service told me that due to those exceptions. My doctors office would need to do a prior authorization before they could do my MRI. I had never heard of this before so I made sure and asked. Do you want me to call the doctors office or the insurance? They said the doctors office. I've worked in pharmacy for almost 20 years and I know for a fact prior authorizations regardless if it's for medication for a medical supply or service, they are all done through the prescriber. I called the doctors office and for about six minutes I went back-and-forth with Teresa and she kept saying how they never do that. They need to schedule it for you. They don't know what they're talking about and after a while of back-and-forth she said please hold and let me see if I can find out. Almost 10 minutes later Evelyn answers the phone and she just says hello. This is Evelyn. I had not spoken to this person today yet. I said I'm holding for Teresa. She said I was told to answer the phone. How can I help you. The correct way to do that would be to say hello. My name is so-and-so. I was told by so-and-so to answer the phone to further assist you how may I help you? Makes sense? Good. Today is April 10 have not heard back anything regarding that check or verification that she was going to do . Yesterday afternoon I received a phone call at 4:33 PM. I missed it because I was working, I stepped out two minutes later and made the phone call and this. Message was saying that the office is closed and that they are now on service. Why would you ask me to call back now? If you are on service? I left a message with the on-call physician. Did not get a call back in the next morning. I had to call three times leave a message the first two on the third attempt someone finally answered. They transferred me to the administrator of the office Jamie . Like I said, I worked for a doctor's office before and worked very closely with my office administrator, and she behaved well above the minimum standards of common decency. This person stated that she was helping the office staff make calls and that is something that she never does so that's why she did not leave her name on the answering machine because if I called back, she would not be the one that I would reach out to and she had no answer as to why you would leave a voicemail asking somebody to call back when your phones are already on service. I will definitely be looking for a new Neurologist, because I am already depressed and anxious as it is that I have to go to a neurologist and live in St. Cloud and drive all the way to downtown Orlando to see these people. The doctor is fabulous, but if I have to deal with that office staff one more time I don't think I'll be able to hold off an anxiety attack. These people have me shaking.

I suffer from chronic migraines, which have been managed well over the last 4 months with medication, but have increased in frequency/severity over the last 2 weeks and were causing me to miss work and social activities. I needed to find a neurologist in Orlando after moving from Chicago a few months ago but many of the offices I called had very long waiting lists (some 5 months long!!). When I called Dr. Toshniwal's office they were able to get me in the very next day! The office itself is very tiny and outdated but cute. It lives in the Orlando Medical-Dental Center complex and is on the ground level in Suite 109. The waiting room is lined with chairs and wood paneling. Dr. Toshniwal took time to listen to all of my symptoms & concerns, asked a ton of questions, and did a thorough neuro exam. Overall the exam took about 20 minutes and she sent my new prescriptions electronically to the pharmacy. I'm very happy to have found her.

Dr. Toshniwal LISTENS, the others did not. She ANSWERED all of my questions, patiently. I never felt one bit rushed. She evaluated me thoroughly, including looking at my MRI and LP test results - which the others did NOT. She explained things on the MRI and LP that no one else explained to me. She talked to me AT LENGTH about the treatment options - including the newest ones, her opinions on them and asked me to do some research and make a decision on what I thought was the best choice for ME. I really appreciated that. I came back this week to talk about my conclusions - she answered some more questions and we decided on a treatment plan TOGETHER. She is also the only one who talked about managing the symptoms of the disease - offering medications (but not insisting on it) to make living with this disease more manageable. And lastly, she is VERY perceptive. She noticed that my one eye was a bit 'off'. I had not mentioned it but I thought I saw that in the mirror the day before, but then I thought I might be imagining it. It felt good to know that I found a doctor that is educated, observant, patient and competent. She is a professional, she is not going to coddle anyone. She does not like to be interrupted (I am really bad about interrupting people) and she will just go silent until you are done interrupting. It's a brilliant response, actually. I have decided I will use it myself when someone interrupts me! She gives plenty of time to talk and ask questions, so that one was on me. Oh, and it is in a weird building - park in parking lot on Evans Street, walk across Evans St into the middle of the building, turn left and it's right there.

    Dr Oppenheim was called in to do a consult on me at FL Hosp Altamonte, and I was warned by the…read moreattending that he was probably "going to be moody" and that he would "catch hell" for asking for a Saturday night consult. I caught hell instead. I was having such severe muscle spasms that I was on all 4's, head on the bed, screaming, when he finally got there. He came in the room and said "turn over" and because I physically could not, he announced after about a minute "Well then if you aren't going to comply I'm leaving" and walked out of the room. My bf and daughter had to CHASE HIM DOWN THE HALL & BEG for him to come back. When he finally did, the nurses were able to turn me on my side and cover my breasts (because I was so hot they removed all my clothes) and he literally took out a penlight, did a one-two pupillary check, said "She's lying" - and left. The next day the attending said "we know you are faking and jumping hospitals to get disability" because that was Dr Oppenheims exact words were to him. I went to a neurosurgeon recommended by a nurse a few months later as I got worse and worse, and found out I had a broken neck, VBI, CCI, AAI, traumatic Chiari, and high intracranial pressure...all things that could kill you instantly in and of themselves. I had brain decompression and cervical fusion with 2 giant screws to hold my neck together, as well as stem cell fusion grafts because my ligaments were torn C1 to C7 from a previous fall. I have fistulas now in my sinus/ears where CSF will pour out when my intracranial pressure is high. I would have been dead in weeks had I not found the problem when I did. I also have residual 'episodic cerebral ataxia' because the brain was damaged so severely by the time I was diagnosed. Dr Oppenheim should retire before he kills someone with his ineptness. He also flag deletes anything negative on other sites. He is a worthless, demented human being and I have never been closer to suicide than after this horrendous interaction. If it weren't for the nurses that took care of me that night, rubbing my back and apologizing for his shocking, sadistic-type behavior, I probably would have ended my life. The delay in treatment has left me with permanent brain damage. Most hellish experience I have ever had with ANY doctor in my life.
