I would recommend verifying credentials carefully before choosing this clinic for USCIS immigration medical exams. At the time of my visit (March 2026), I was unable to confirm Dr. Neema's authorization status as a USCIS civil surgeon, which is required to perform exams and sign Form I-693. My family of three was charged $2,400, and the process took about three weeks. Unfortunately, the paperwork was not usable for our purposes, and we now need to complete the medical exam again with another provider. I also requested a refund but have not received one to date. Sharing this so others can do their due diligence with USCIS beforehand and avoid potential delays in their application process. read more

I rarely write reviews, but this one deserves it. My visit to Dr. Neema Malhotra's USCIS Civil Surgeon clinic was the most frustrating and overpriced medical experience I've ever had. I went in for a simple USCIS immigration medical exam, and ended up paying $1,925 for two people -- far higher than any other clinic I contacted (most charge under $300 per person for everything). The staff used vague language and pushed me to pay for multiple "convenience" tests upfront, saying I could get a refund later if I brought records. That refund process turned out to be nearly impossible. During my first visit (Sept, 2025), I waited more than an hour before seeing anyone. Most of the interaction was with the front desk, not the doctor. My follow-up appointments were just as confusing -- I was asked to clarify signatures and even form barcodes while still not receiving the sealed I-693 forms required for immigration. After multiple visits and a month of back-and-forth, we left exhausted, out of money, and without the completed paperwork. If you're doing your immigration medical exam, please call other civil surgeons first. Ask for full pricing, turnaround time, and exactly what's included. You'll save yourself a lot of stress and hundreds of dollars. read more

Update : changed 1 star to 3 star as I got my refund after this review! I had a visit to Dr. Neema Malhotra and was charged $200 upfront for the consultation. I was assured that this fee would be refunded once my insurance coverage was verified. However, despite repeated attempts to contact the office and numerous assurances that the refund was being processed, I have yet to receive the money after 4 months. Additionally, to my surprise, there was another new doctor present in the room during my consultation. I was not informed about this beforehand, and it made me feel quite uncomfortable. Discussing personal medical concerns requires privacy and trust, and having an unfamiliar person in the room without prior notice was a clear breach of that trust. I found it embarrassing to answer sensitive questions with someone else listening in, whom I had not consented to having there. Also during breast cancer screening another male doctor walked in to the room without asking which was really embarrassing!!!!! I hope the practice will address this issue and ensure that future patients are informed of any additional personnel being present during a consultation. Until then, I would hesitate to recommend this doctor to others. read more

I'm so disappointed with this Clinic. I paid for $695 with talking with Dr.Neema about 10 mins. She just told me to follow the steps she made. I made appointment with Quest Diagnostics as her recommended but the Clinic didn't give a a doctor's order to the Quest. So l was kicked out from Quest and I wasted my whole week. So I asked for refund, the clinic said They will keep $195, they will refund me only $500 which is they don't refund me yet. And they said they are so busy to transfer money back to me.!! I went to Instant Urgent Care yesterday. I paid only $199, I had an exam from the Doctor and got 2 vaccines; Flu and T-dap from the nurse. After that I went to Labcorp to get my blood draw which is I paid only $90. Will you pay $695 for Dr.Neema and you have to do everything by yourself. Or pay for $289 to Instant Urgent Care and Lapcorp then you will get almost one stop service. You can make your own decision!! read more

I have to update this review, since I am allowed only one. First experience: Sep 12th 2021. I really want to thank Dr Neema Malhotra and her staff for their help with the I693 forms. The whole process was really simple. To start off, the website has very detailed information about the whole process. The initial appointment was for the physical exam and also to go through our medical records to figure out the tests and vaccinations needed and the follow up appointment to fill up the actual forms. While filling the forms, Dr Malhotra patiently explained what the fields in the form meant and how they translated to our case, so we could understand. I am also really appreciative of them for rescheduling my second appointment at the very last minute and accommodate us the following day, despite their very hectic schedule. Thank you Dr Malhotra! Second experience: July 12 2024: I want to thank Dr Malhotra and her entire staff (special mention: Mayukha and Vinod) for helping me again with the 693 forms. I had to get this done again, since my previous 693 expired. The experience this time was exactly as my previous one, except better. The process works like a well tuned machine. The first visit is to access the needs of the patient for the 693 - compare it against a well documented baseline and process to identify the tests that need to be done and the timelines for them. After this, Dr Malhotra's staff provides multiple options to get these tests as soon as possible. The second visit is after the tests results are in, to transfer the data from the tests and vaccination records into the official 693 forms. Dr Malhotra does this very methodically and also while patiently explaining to us what each field means and how this maps to our results/records, so that we understand. A special mention for Mayukha, who handled the case before meeting the doctor and after as well. She made sure that all the documents were in order before the meeting and once the forms were filled, checked and double checked the data in the forms to make sure everything was done right. I am glad that I got an appointment at this clinic when all other nearby clinics were all booked. However things are little unmanaged at this clinic. I went there for USCIS medical examination. On my first visit the nurse confirmed that she will book my next appointment on Fri 2:00 pm. Luckily I called on Wed to confirm my appointment time. Apparently she never booked the appointment!! (I kinda panicked because I was a bit on the clock because I had a deadline to submit the report.) I then booked another appointment for next week Tue. Then on Friday I got a call from clinic saying they can't do it on Tue and then again rescheduled. ( I had to move a couple of meetings at work because of all this) I still made a peace with all this shuffling. Finally, on my second visit, my appointment was at 4:40 and I reached there at 4:30. After waiting for more than half an hour, Dr Malhotra came and said she will look for our papers that we had submitted on our first visit. I waited more sitting outside patiently. A couple came after that and the doctor called them in and attended them first(and they took another 20-25min). That's when I lost my temper!! I don't understand this process. Shouldn't they attend on first-come -first-serve basis?! Even if that couple had an appointment before me, they were late and I was already in line and was supposed to be done first!! I went there with my husband and my husband stopped me from saying anything to the receptionist because that would be impolite. Finally we were called in at 5:30!! We waited outside there for one hour!! Just to get inside. That's also after we had appointment. What's the point of getting an appointment and being there on time?! I had a class at 6:00 which I missed because of this unexpected delay. They need to respect and consider other people's time. read more

Omg, I thought this review was finished back in 2018!! but apparently, it was still hanging strong as a draft..lol Anywho, I enjoy and respect every doctor and nurse, and admin that worked in this establishment and that help my sons. Every time I came it was professional and clean and I was attended to fast and but not in a hurry. The best decision I ever made and luckily I'm like 8min. from the Milpitas clinic if you live far make the effort you wont regret it :) First of all when I first met Dr. Malhotra I feel at ease like she's been my doctor for a long time. And you can't go wrong with this clinic they are very professional in what they do. All questions are answered, everything is provided all information is given and they have a process of doing things. and you will be guided throughout the whole process. For lab results they are proactive as soon as the results came in they will contact you. The same with follow ups it's easy to get in touch with them. If not for Labcorp mishandling of my blood sample I could have gotten my I-693 medical report in just 4 working days. Highly recommended for USCIS medical process. Thank you again! Doc and staff! I literally fell in-love with your staff :) Once approved I'll be back I'll bring some donuts! :) btw. to help others I got my covid,flu and Tdap from walmart for $327 look or ask for coupons and If you will take all the 3 dose all at same time like what i did, be ready its kinda heavy to absorb I was bedridden for like a day and a half. Polio shot I got it from walgreens for $71 luckily they have 20% off sale again ask or look for sales or coupons. I am ecstatic to have found Dr. Ben. My family is full of health care professionals (physicians, surgeons, health care researchers, etc.) so I was raised discussing health care at the dinner table. I also have an undergraduate degree in Human Biology from Stanford. I continue to read scientific papers and watch videos by doctors and scientists relating to health. So I'm reasonably knowledgeable about how to optimize my health (good diet, exercise, certain vitamins/supplements, periodic diagnostic procedures, etc.) to live a happy and healthy life to a ripe old age. Dr. Ben was a true wealth of wonderful information. I came in with a write-up of highlights from my medical history as well as information about what my parents passed away from which he loved. He is extremely warm, respectful, and very communicative as I like to understand why something is recommended. He explained why blood work might be advised at this time in my life and what blood tests he advised for what reason. There are a couple other diagnostic procedures I let him know I hadn't had in a while and he agreed it was time to do those again. I am not a young adult but rather middle age so feel very blessed he took me on as a patient. I've seen a couple dozen physicians over my lifetime. All have been excellent. But I can honestly say that I have never had such a positive experience with any physician as I had with Dr. Ben. It was a delight to me he really listened to my concerns and desires for my remaining healthy. The doctor spent 5min to "have a chat" with us and tell us "you may Google for PCP" then charged us $200 for that "consultation". She even didn't look at the patients (my parents) nor did she admit them to the private examination room (she only "stopped by" the waiting room to have a chat about our case). It is NOT acceptable to see patients in a waiting area when there is no privacy, nor is it acceptable to charge anyone for a 5min chat, let alone $200. Encouraged by the ratings, we reached out for an appointment. Dr. Malhotra responded personally and referred me to her online system for booking the appointment. The appointment process was seamless. We were contacted by the administrative assistant who explained exactly what we needed, what the day of the appointment would be, and the costs involved. We went in and had an initial examination with Dr. Ben Kachouei, who was super-friendly, detailed, and practical regarding how we could complete all our documentation. Within a couple of weeks, we completed all the necessary vaccinations and tests, with the ever-approachable Dr. Kachouei guiding and advising us actively on next steps. Once we had everything we needed, we walked in and out of the clinic in about an hour. Dr. Neema Malhotra was very sweet and respectful, and particularly patient with our very chatty toddler son. Having observed such unprofessional behavior as a degree of rudeness, inability to establish trusting relationships with patients - it's questionable whether these people are professionals in their field. Not recommended. If you need your immigration papers done and you think this is just a formality you might tend to choose only by the price. Think twice - you might end up paying more, being offended and treated as crooks. Choose a respectable clinic and don't be afraid to pay more - you'll get less problems, believe me. read more
