My father is currently in the hospital after a fall last week. He is on blood thinners. He literally is a miracle as he survived a dissection of his aorta that tore down to his femoral arteries. He was treated with extremely poor care after he was moved from a one person room to a three person room. He was there less than a month. I would rather not give 1 star. My dad went in for rehab for his legs to get stronger . It was mostly CNA that work their . 3 men per room 1 public shower. The place was filthy. They had nothing to clean the rooms with . My dad was in the middle and he had no privacy curtain or TV . Food was horrible. Dementia patients on the same floor that screamed and threw things , they also would come in my dad's room. No security. read more

My mother was admitted for antibiotic treatment of a bone infection. She had no respiratory symptoms when she arrived. Over the next three weeks, she began to develop breathing issues. Despite this, staff repeatedly dismissed her symptoms, saying she was fine and "just anxious." Even the doctor insisted everything was okay, reassuring us: "She's getting her breathing treatments." [[HIGHLIGHT]]That very same morning, only because of a family member's persistence, she was finally transferred to Memorial.[[ENDHIGHLIGHT]] There, she was immediately admitted to the ICU, intubated, and diagnosed with respiratory failure. She passed away a couple of weeks later. read more

Not as advertised. Rehab has 3 crammed into 1 room. They don't separate the usual patients from the alert and oriented ones. Quality of care is like throwing your loved one in an old nursing home. Putting someone in a wheelchair all day just after foot surgery and a dialysis port is not right. Transferring that patient into chair and thus tearing the stitches on the port where blood is all over patient while they refuse to pay for guerney transport for dialysis is criminal. Patient had to go to ER after the nurses took time to decide to wheelchair transport or call an ambulance. This patient was in severe pain not only from his foot surgical wound but the torn stitches on dialysis port that needed to be replaced at hospital. Think hard before you choose this facility. read more

This was my second time at MPAC. Last November I was there for 3 weeks. I needed pt and ot as I was a fall risk. I had a fractured hip,recovering from my body crashing and being septic. [[HIGHLIGHT]]Everyone from the upper staff to housekeeping were kind and attentive.[[ENDHIGHLIGHT]] I shared a room with 2 other women, I was in the middle or B bed. I didn't have direct access to a TV but I had my tablet and earbuds. My room was clean and so was the bathroom. You do have to share as it connects to the room next to you. The food was eh, not great but considering all the people they care for, it's not going to be 5 star. It can take some time to get your call light answered. As you're not the only patient, Well this time it was different, I was there for 3 and a half weeks, the level of care not the same. I was admitted after back surgery. [[HIGHLIGHT]]They are understaffed, most of the CNA's are really nice and care about your comfort.[[ENDHIGHLIGHT]] I made some great connections, others seemed too preoccupied care. Some of the nurses in charge of handing out the meds would be slow in getting it done on time. I have to say the therapists ot and pt were awesome! Both times I stayed there,they were wonderful! Very patient, kind and encouraging if I could have I would have put one in my pocket and brought them home with me, only to rotate them out once a week lol. They helped me be strong and not quit. HORRIBLE.. the neglect they have on the patients is insanity in this place.. don't send your loved ones here unless you don't want them to come home.. please.. The nurses are WAY under staffed here specifically CNAs and the company knows it and continue to make no change. Patients are walking around with diapers to their ankles from being so full. Patients are walking around bandaged from continuous falls. [[HIGHLIGHT]]They shut the door on the patients so they dont have to hear or deal with patients.[[ENDHIGHLIGHT]] Mind you I have seen ALL of this first hand. I had to change the trashes and her sheets on her bed everyday because they always "didn't get to it". When she arrived there was feces smeared all over the bottom of the toilet and I had to clean it because no one knew "whos job that was" even the janitor that is there one day a week said he wouldn't do it. So if you need something done don't even waste your breath asking just do it yourself. [[HIGHLIGHT]]When they have their "care" meetings it's full of them defending themselves (no accountability) and nothing but waste of your time[[ENDHIGHLIGHT]]. When my loved one arrived to this facility until the day she left (passed) 3 months.. they never even had her records from previous hospital so they were giving her the wrong meds and wrong treatment the entire time which caused her passing. Due to their negligence on looking up past records. [[HIGHLIGHT]]Not sure how anyone has given this place a 5 star review.. they have to be fake.. because I was there religiously everyday for 3 months because I didn't trust the place.[[ENDHIGHLIGHT]] Before we had the chance to transfer her it was too late so please don't send your loved ones here. read more

I had a loved one that left this facility.From the day he arrived there were issues. We visited him and it took 45 min- to an hour before they were able to change him. He was literally writhing in pain and it took just as long to get him pain medication. Some of the nursing staff were very caring and really tried to help but others were rude. We had a family member come visit and they were given information on how he was doing. Never were they asked if they were allowed to have that information. When other people who were allowed to ask about such things, tried to ask they hit a road block. This inconsistency in the sharing of information is disturbing. The person who showed us around was very nice but newer to the facility. Our loved one was promised a semi private room but was sharing a room with 2 other people for most of the time he was there. The facility could use some work. There were gouges in the wall of the last room our loved one was in. (Picture attached) The most upsetting issue came the last night our loved one was there. I dropped by to see him and he was laying in his bed in a diaper. No blanket or sheet! It took 15-30 minutes to get clothes on him. Some of the nicest things were the live birds and how some of the staff went the extra mile and got to know my loved one. I wouldn't recommend this place to my worst enemy. read more

I would call this place Hell's Waiting Room. They treated my mother like trash, with the exception of a couple of compassionate on the ground staff. First, we have NO EVIDENCE that my mom ever saw a doctor there or received any kind of therapy. They just had her rotting in her bed. Then, THEN, something strange happened. We were suddenly notified that they were discharging my mom. This decision was driven by the business manager. I was told by my mom's medicare advocate that the file showed a conversation between the insurance nurse and the business manager that led to the discharge decision. Even the advocate thought this was strange. Of course we were not prepared for the discharge so we had to pay out of pocket to get ready for my mom. Our family was whirling from all of this activity with NO ASSISTANCE from the hospital. Don't get me started on the social worker who basically is a sidekick to the business manager. Important to note, my mom was in great pain in this facility. One nurse noticed it and called the dr who ordered x-rays. The business manager scheduled my mom for discharge before they did the x-rays, which is another reason she wasn't ready for discharge. We appealed the discharge decision and won and were reimbursed for our out of pocket expenses. It took the business manager weeks to return our money. After promising that she processed the reimbursement, I once again had to call the administrator to get our refund. The day I came to get my mom, I ended up waiting 2.5 hours to get the discharge documents. It wasn't until I asked to speak to the administrator did things start happening. Also on that day, they lost my mom's brand new bottom denture. I called the social worker when we arrived home, left a message and never heard back. Never got a call back...TERRIBLE. The facility was always hot and dirty. My brother took photos of my mom's filthy dirty bathroom with feces on the seat. Only when we complained about the heat, did they bring in a fan to move the hot air around. The AC only worked in the hallways and in areas frequented by staff. My mom got a new roommate. A woman with a cough who wasn't vaccinated. We overheard her tell one of her phone callers that she felt feverish. My brother expressed his concern about Covid to the nurse's station, who assured him that everything was fine. Only to find the next day my mom was moved and the whole room was sealed off because of Covid. We knew that woman was sick. I think you can tell that this is a very awful facility. My mom's other lovely roommate told us that she had a surgical wound that got infected in the facility. I cannot stress how terrible this place was for my mother. I plan on writing a letter to the state and joint commission to complain about this place. I would never, ever want anyone to experience what my family experienced. I have rescue dogs who have received better treatment than my mom and some of the patients in this facility. If you can avoid this place, don't put anyone you love here. read more

If you're a cna do not work here. They assign you so many residents (the ratio is unsafe), they don't provide appropriate ppe, the nurses talk bad about you in their language (extremely rude) and they don't provide medications on time to patients. If you care about your loved ones, don't send them Here at all. If you complain to the state they retaliate against you. 0 stars. Sabrina the supervisor is amazing, she gets a 5/5 stars. read more

My dad collapsed 6 times before being admitted and needed rehab to be able to walk on his own. He spent a week in the hospital and was sent straight here. He caught covid in the facilities and was isolated, which is understandable. He was the only patient who had it. We were told he would be isolated for 10 days. However, he was only there for 5 days and was released before receiving any actual care or rehabilitation. He collapsed at home his first night after being giving the okay to come home. read more

This place is not a good place they don't follow the instructions on how many times the bandage needs to be changed ,the food here is so bad and they give you such small portions like they are paying for it so you sit here and starve you ask for a something to eat and they bring half a peanut butter sandwich they give you the pain medicine on time so you sit there and suffer in pain,what is wronge with these people who are suppose to help you and take care of the patience while they are there. They don't give you the pain meds on time so you sit there and suffer. read more

The administration, nurses, and PT's were all wonderful and caring. It was VERY clean!!! Staff members were constantly keeping the place tidy, eliminating that urine smell. Everyone there was friendly and caring. I highly recommend Modesto Post Acute Center!!! read more

I was admitted to this facility in 2022. I was transferred from Memorial Hospital in Modesto. I had had fallen in my home, I causing a Brain Bleed. I am going to not go into details. I am going to just tell what happened. The admissions people were no where to be seen. I was rolled into a room that looked like something out of horror movies the entire area was dirty. I was totally scared. I had a roommate, that was using her cell phone the entire time I was there. night at about 10:00. PM I was found on the floor. The staff contacted my family, stating I refused to speak with them. my daughter told them to put me in an ambulance and take me to the hospital. I was told I had been left for on the floor approximately at least 4 hours. I then was taken to Doctors Hospital in Modesto. I had had a second brain bleed. Another Brain surgery was done. it's been going on 2 years. I will never advised any family to use this disgusting filthy facility . The staff was horrible. The charge Nurse was a bully. I had complained to her, and was yelled at. I am thinking about filling a law suite. I have told my family that I am writing this message to worn families to know what happened to me. read more

1 star because it doesn't allow 0! We had absolutely no choice but to have our family member placed here. He was placed on an Amtrak train to Utah. Unable.to walk, placed in a wheel chair, alone for a 12 hour trip. We were told AFTER he was on his way. I was shocked to find the condition he was in. No one wants to see their family member in this condition. He was soaked in urine as he wasn't able to get to a bathroom for 12 hours, he was a mess. He seemed really confused and never quite recovered from this ordeal. We had to put him in a hotel for a week, our house is not wheel chair friendly. The Dr here in Utah couldn't believe that this was allowed in California. The Dr office helped us find a place for him. They said we weren't leaving without getting him real help. We got him into a facility here where he passed a month later. HORRIFIC! read more

Terrible facility. The food was not edible. It looked like someone threw up on a plate. We brought cup of soups and instant oatmeal for my mother in law so she had something to eat. They washed her hair one time in 100 days after she threw up in it and we called to complain that she needed it washed. She was there for 100 days after heart surgery and not once did they have her getting up to walk to the restroom. Too much trouble for the CNA's to be called to get someone up to use the bathroom. Shameful. Aren't they supposed to be getting you ready to function at home ?? Some of her clothes went missing. Then they call us...son and daughter in law at 4pm on Thursday and tell us they are releasing her the following day. No time to organize anything. So many problems...too many to list. Would never recommend this facility. I would give it zero stars if possible. So sad. read more

My special needs aunt took up residency here in September after a several week hospital stay and her sister, my mom, has been wanting me to write this review for some time. From just about day one, my mom, who visits several hours a day, every day, has had almost nothing but praise for Modesto Post Acute. I also visit my aunt about 4 days a week and have come to know the staff. From the front reception desk to the nurses to the administrator, we have witnessed nothing but kindness and compassion. During my visits it is not uncommon to have several nurses swing by my aunts room just to say hello and check in on her. My aunt has Down Syndrome, and has more bad days than good and the staff is patient and kind and always treats her like her feelings and thoughts matter. If you or a loved one is in need of rehabilitative care, we would highly recommend Modesto Post Acute. read more

As skilled nursing goes this has been the best one, and I have been to three of them prior to this one. It is clean, it does not smell like a rest home. All staff are accessible and willing to help. They make you feel important. CNA's are wonderfully playful, they will make you laugh and tell jokes. They are so respectful, especially when they are helping you clean up. I can say it does take a while for your call light to be answered, but that's because they are helping others. If you come here for rehab you must embrace your rehab to get better, I can say at first I had some issues but just tell someone and they will help. They did for me. They made me feel fairly joyful and important, and that is usually hard at places like this. read more

If I could provide 1/64th of a star I would. Zero stars would be more appropriate. Do NOT send a loved one here if you desire to keep them a loved one. I was referred here because my mother was discharged to home from Memorial Hospital. My mother had extreme, dementia, exacerbated by a UTI. She could not stand, walk or care for herself in any capacity; yet, the hospital did their job of stabilizing vitals and recommended discharge. I argued that she could not return to her home and was initially referred to a place in Newman who could not obtain Medicare approval in a timely manner. I then selected this facility because it was in Modesto and I was hoping that my mother return to some level of functioning. I will forever make amends to my now deceased mother for this choice. The room had a massive stain on the floor located near the bed that was present and pronounced for 2 days until a family member complained. I was told that a dietician had placed my mother on a diet of pureed food. I never spoke to a dietician or saw medical orders that indicated the need for pureed food. Family fed mom various solid foods with no problems. I frequently had to seek out medical staff to find out when my mother's pain medication was administered. Residents were bathed every other day. I bathed and changed my mother's clothes and undergarments on the days that I was present (majority of the family lives in the Bay Area.) I never met with an MD during my many visits. Many of the visits I found my mother in clothes that did not belong to her and the clothes provided by family missing My sister was out of the country at the beginning of my mother's stay but the staff (specifically the business office) was provided with appropriate contact info upon her return. They continued to confuse the numbers and inquire about who's who; please note that contact information was two distinct area codes. The business office's main concern was number of days in the facility and who was going to pay. MediCare was notified of my mother's discharge date by the business office (which was later denied despite written documentation to the contrary.) My family was told that we failed to respond to a time sensitive document and the discharge was finalized as a result of our failure to reply. Two out of pocket payments were made to the facility which slated to be returned to the family after an appeal to the insurance; we are STILL waiting for a refund my mother was in the facility in mid May and out by June. I could write so much more but I believe this is enough of a cautionary tale read more

Best bet for any resolution here is to contact Ombudsman complaints in Stanislaus at (209)529-3784 My family member went there for OT and PT care after a 2 weeklong stay in DMC for a bad accident. They would leave him in pain for extended periods without meds even though there was an order in place. Would ignore him and leave him in own urine and BM on a daily then would yell at him for calling me to get him help. Then discharged him home with someone else's medication. Didn't setup his home health care in his area even after being told and with orders. Spoke to the Director of Nursing, Ass. Director, and the Administrator...Once I hang up the phone, nothing was done and haven't gotten back to me. read more

This place treated my loved one horribly. He told me he was abused. When I found him he had an IV hanging out of his arm. He was sent there without our permission and he was kept there overnight and he was abused he said and I know he would not make this up. He was told he could not have a walker or wheelchair which he had to have to get around. He was told to pee or poop in the bed. He was in pain and was given none of his medicines. He had a horrible wound on his foot and they called me for material to use on it and never did a thing for him. This is the worst place I have ever heard of and had a loved one experience. They, management know what is happening and I don't know who passes them with good grades but I would not trust them. I HATE THEM. THEY JUST WANT MONEY. Why anyone would allow them to stay in business you have to mistrust. The poor people living there must be having it that way to. What a horrible thing to do to put your loved one there. We never gave our permission to the hospital who placed him there. I rescued him out of there. BEWARE be alert. read more

I was in MPAC for almost a month and am still amazed at how well this facility is run. The nurses and CNA's quickly became as family and there were several I would gladly have taken home with me. Especially outstanding was the physical and occupational therapy department. I've had several physical therapy courses over the past several years and this one is by far the best. I progressed better under their care than any I've experienced before. Originally I was thinking of a care center closer to home (I live a 3-hour round trip drive from MPAC) because it was so much closer to my "support" group, but it never could have compared to MPAC as far as quality of care, professionalism, and downright friendliness. I hope I never need such care again, but if I should I would quickly choose MPAC. read more

My 92 year old great aunt spent her last 4 years of life here. I had good experiences at first and while she was eleigible for "rehab "she got attention and was treated well. The activities staff are very nice and helpful. The Finance office who maintained her Trust account (for small purchases she might want) was also very good. The care and communication, especially in the 2019up to her passing in December 2020 were iffy to down right awful. It took a complaint to their home office to get local staff to pay attention last Autumn when things got really out of hand. I'd buy soft pretty blankets for her and they would insist on "laundering" them in some dryer so hot it melts the polyester fibers; she was moved to new rooms frequently and in doing so all her belongings got lost including family pictures and the small picture of her and her husband. In the end she fell out of bed and they didn't even call me. After I found out they promised to keep me informed, When I called the next day all was "fine". Three days and three calls later they told me she had not eaten anything since the fall! I was allowed to see her the next day and she passed 4 days later. Advice - if your loved one is there VISIT OFTEN. Do their laundry for them. Don't trust that your voicemails left to Social worker will be heard or responded to. Same for emails. Review the med sheet and make sure they get what is perscribed and not thins that aren't. I was out of the area and had to rely upon the staff, many who don't speak English well on the phone and were almost unintelligible. I was nice but persistent. I brought treats and goodies to the staff hoping it would influence her care, and yet in the end I am very bitter at the treatment we recieved. Advocate for your loved one and best advice? Make sure you purchase long term care insurance to not have to be left to the "care" that passes for end of life in a "long term care" placement. read more

My sister was a resident at Modesto Post Acute Center for over 1-1/2 years. Every staff member treated her with love, kindness and compassion. The facility is not fancy, but it was always clean. The dietician catered to my sister's gluten free diet. Each department gave her their full attention and did everything they could to make her feel special. The residents were treated like staff was a guest in their home. Thank you from the bottom of my heart. If I find myself in need of long term care, I would want to be there. read more

I recently was under the care of this facility back in April until just recently May 12, 2018, I was badly injured in a car accident in March and never thought that I would walk again at least not so quickly upon my stay there I met the most wonderful physical therapist named Ramses Rivera. The first word that he ever said to me was it's going to be OK, you're going to walk before you know it, I have faith in you, just trust me. Honestly I thought he was crazy because I had very little faith in myself but I was very determined. Each day this man took each day very seriously and he was very consistent. He was kind, caring and most of all unique. He has a true gift that has promising results in his patients. You would be amazed with the skills that this man has acquired over the years of his practicing. Let's just say by the time i time my foot doctor said ok weight baring, he was more excited for me that I not only began to walk but looked forward to each day to work with him that 10 days after my foot doctor said I could bare weight I walked well enough to go home to my family! I also want to include the two young ladies that Where are my occupational therapist that also accompanied and assisted with the motor skills to help me go home one is Lashell and Kris please excuse me for not remember both there last names. Also Randeep the other physical therapist that helped too. But this facility worked magic with these four Therapist that I mentioned The facility would be crazy to lose or not have them as a part of their team. I would recommend this to everyone I know. Please forgive me if I didn't mention the nurses or other staff but these four Therapist really grabbed a hold of my heart and still meant and still mean the world to me for I have them to thank today for me walking again thanks a lot your friend Crystal Smith read more
