When you call to schedule an appt, you get someone in a foreign country who only knows a script. Frequently the accent is so strong and cannot be understood. They have a policy that they are not required to provide you with your own test results. I thought this was illegal, but there is some loophole. I can only guess the more info you have of your own health could put them at some disadvantage. They are run by business people, not doctors. I have recently been threatened with a collection agency to get a (paid) $15 from a year ago. The patient portal is totally unsatisfactory. You cannot just look up when your last doctor's appt was. You have to know when it was, which defeats the purpose. You have to pay for parking at Ridge Hill, by a system on a cell phone requiring a great deal of info including credit card info. It is time consuming, then there is a substantial walk to the facility. If I could rate it zero stars I would. rate it zero stars I would.

My insurance company said they were an in-network provider so by law they had to pay Forme directly. Not only did I pay the full cost for my visit, but Forme was also paid my by insurance company. Forme has refused to reimbursement me for my payment even though my insurance company has also paid them as well for my visit. I would not recommend this place.

People are always fighting at the front desk over fees. The place is filthy, especially the bathrooms. My doctor who used to work here just walked out and was replaced by an unprofessional incompetent unqualified rude three foot tall nurse with some kind of bizarre god complex, who doesn't understand English and badmouthed my former doctor. She doesn't even know how to write a prescription properly, and it was already in the system. If you value your health and your life, STAY AWAY FROM FORME´MEDICAL CENTER IN WHITE PLAINS. I never write bad reviews but if I could give negative 5 stars, I would. This is about your well-being. Go see a real doctor, not these quacks. I wouldn't trust them to take my temperature.

    On Monday night at around 11:30 pm I was transported by ambulance to the ER with severe shortness…read moreof breath and dizziness. I spent the next 12 hours in the ER while waiting for a room. They gave me a battery of tests, blood labs, chest Ex Ray , EKG. When they determined that my Hemoglobin was severely and critically low they were expecting to find some kind of internal bleeding. Their thoroughness found out that I had a urinary tract infection coupled with my diabetes, my anemia and some of my diabetic meds were adding to my severe anemia condition. They immediately put me on antibiotics and started to withhold some of my other meds because I needed to stabilize my blood. They gave me 1 blood transfusion that did not improve anything. After 2 more transfusions my blood improved tremendously. I would be remiss if I did not praise the staff. Mark the technician in the E R was great. The nurse and ER doctor were very attentive, methodical and reassuring. When I got to a room later in the day I was amazed at the nurses and techs and how they were a team and exemplified caring for patients. At this stage of my life I have been in hospital situations over the years and White Plains Hospital should get ten stars. Thank you Mark, Megan, Michelle, Rachel, Pam, Julianna and Dr Alon the attending physician. Thank you ER staff. It was determined that I had a UTI that sent my blood to go too low. I gave 3 stars because of them!! However, the transport tech and other lower echelon staff,in addition to "SECURITY," need professional development and etiquette training. To put it mildly, they are simply GHETTO and unprofessional. They escalate situations when they should be trained to de-escalate!! The area is also DIRTY!!

    March 3, 2021: One year, 7 physical therapists, and 12 doctors later, I have been diagnosed with…read morecongenital hip dysplasia, cam impingement, and they also found labral separation. It is believed that my legs were pulled in the position they had me during the botox procedure causing further hip pain. It is unfortunate that I have been treating my symptoms in the wrong order as many of my symptoms did not manifest until the day I had my procedure at Maze Clinic. I am still having moderate to significant pain in my groin, lower abdomen, lower back, and have consistent vulvodynia and pudendal neuralgia symptoms as well SI joint inflammation consistent with sacroiliitis/degenerative change. All of these symptoms began immediately after having the botox procedure here with the aggressive dilation. May 2, 2022:  I recently had stage IV endometriosis (adhesions, lesions, and cysts) excised in addition to having two PAOs where my hip dysplasia, impingement, and chondral labral separation, and SI joint inflammation were also causing significant pain. The endometriosis was everywhere, specifically behind the rectovaginal area, and most likely present when I had this procedure as well because I had debilitating dysmenorrhea that was overlooked. Endometriosis can cause deep dyspareunia, superficial dyspareunia, vulvodynia symptoms, hypertonic pelvic floor and more. It's just unfortunate that none of these were diagnosed or appeared earlier prior to doing this procedure. This is a common problem as many people face dismissal and are misdiagnosed over and over. I was told by many physicians and physical therapists including this place before this treatment that I just needed the "extra push." My vaginismus/hypertonicity is just a symptom compared to everything else. I also most likely have congenital neuroproliferative vestibulodynia - overgrowth of nerve endings at the vulvar vestibule - which is often mistaken for vaginismus for many patients. There is a tremendous lack of knowledge/literature in the relationship between hip abnormalities and pelvic floor dysfunction. Hip dysplasia and/or impingement can lead to hypertonic pelvic floor and labral tears which can cause pelvic pain, SI joint inflammation, and symptoms that mimic pudendal neuralgia/vulvodynia and bladder urgency. Most pelvic floor PT exercises STRETCH and RELAX the muscles surrounding the hip, which is actually the OPPOSITE of what you need to do with hip abnormalities (dysplasia and impingement) since it can cause further damage and exacerbate pain symptoms. The bottom line is that I needed real surgical interventions - 3 SURGERIES with leading surgeons before I could even come close to treating the sexual dysfunction and hypertonicity. No amount of dilation or botox would have helped any of this. My hips, buttocks, and lower back have little to no pain anymore and strengthening has had the best benefit for me. To both patients and providers, attend an ISSWSH annual meeting or course to learn more about sexual health and stay up to date with the recent literature. Pelvic floor dysfunction can have many causes and misdiagnoses are common. If you are someone with a long history of pelvic pain, tightness, and the pain exists not just internally there may be something else at play. Also, if pelvic floor PT doesn't help or makes things worse, please go to another specialist and/or have a sacrum, pelvis, lumbar spine, and hip MRI/CT scan first. Please consider hip abnormalities, endometriosis, SI joint dysfunction, and vestibulodynia, and screen your patients for lower back, lower abdomen, and hip pain too.

She also ended the call abruptly when it went over the time limit even though I was in the middle of explaining something. She was also not helpful in providing any direct information about what could be going on with me even if it was an assumption based on what I told her.

I talked to doctors 2 times and a social worker once and on all 3 occasions they either were thoroughly confused or they just did not care about providing care and instead just lied to me. Of particular note was a Dr. Asmin or Asmen (I might be a little off on his name). He called me to report on the condition of my 80 year old mother after a painful fall. It's wonderful that he called, and if anything he said was honest I would be writing a positive review here. Instead he swore up and down that my mother must been out of her senses when she complained about the medical staff at night moved her injured legs with complete disregard for her immense pain. The doctor promised me that not only was my mother not in pain but that she was given morphine twice. He stated the exact times the medication was given as if he was double-checking to be safe. When I arrived at the hospital I double checked with the nurses, they confirmed that no pain medications were given. I called the doctor back with the nurse at my side, and the doctor said, "honestly, your mother isn't even my patient." Mind you, he had called me, unsolicited, to inform me of how well she was being treated, when the exact opposite was true. Your hospital is pretty, but I would never trust it in a real emergency. If you have family that ends up here, know that it's on you to verify the honesty of everything the staff claims. Be safe and verify everything. 3 lies in a row from doctors isn't an error, it's a pattern showing lack of care, sloppliness, and true incompetence.
