This is the worst medical practice I've ever encountered. Avoid this doctor at all costs…read more She and her office are extremely unprofessional and incompetent. They forgot to conduct tests and lab exams and when I pointed it out they made me come again and ordered another lab exam. First they said that it was their error and I would not have to pay anything extra but when I had two lab bills they ignored me and refused to reimburse me. It was frustrating and I will never go here again and do not recommend this office.

The second warning sign should've been that the front entry to the building was in disrepair and I fell and injured my knee. The final straw SHOULD'VE been when a process server was attempting to serve the doctor while I sat in an empty waiting room -- but I was desperate. I went to Anna Lavotshkin's offices for a simple ailment in November of 2014. All I needed was a quick antibiotic and to be sent on my way. My GP couldn't accommodate me in a pinch so I got stuck going here. Right off the bat, Anna Lavotshkin was pushing to give me a physical. Ummmm no. I told her I had a GP and had had a recent physical, but he couldn't accommodate me. Luckily, after an uncomfortable conversation I was ready to get out of there. The worst of this was related to billing. I did not receive a bill for her services until MONTHS after the fact. (7 or 8 months, for the record) They do not accept cards at all, so I cut a check for the balance IN FULL through my bank account online and had it mailed to their offices. The check was cashed by her immediately. A few weeks later, I received a bill in the mail again. I called the office and they had no record of the check - they asked me to provide a copy of it. So while my statement is showing that the check cleared, I now have to pay a $10 to send them a copy of the cashed check. What a joke.

My mother was a longtime patient of Dr. Salazar for many years. After a complicated spine surgery…read moresequence left her fragile, we brought her recent CT/MRI, lab results (GFR 17) and hospital notes to him because we trusted his reputation. He initially told us he would help and said she needed an EMG -- then, under apparent pressure from the hospital, he reversed course, stopped advocating, and ultimately dropped her from his practice while she was hospitalized and unable to find a replacement. During that time she repeatedly received dangerous IV diuretics and treatment decisions that worsened her kidney function. She was hospitalized repeatedly, spent 45 days in the hospital, and ultimately her kidneys were destroyed -- she's now on dialysis three times a week. She can no longer manage her insulin pump and her quality of life is destroyed. She was 82 -- this should never have happened. He said he would sue the hospital for what they were doing -- then he fell in line. From where I sit, patients aren't protected when doctors bow to institutional pressure. I'm posting this so anyone who trusts their doctors to speak up will know to document everything, demand second opinions, and be extremely cautious if a long-standing physician suddenly "drops" a seriously ill patient. (This is our family's experience -- we have all documents to support what we're saying.)

I would recommend her to anyone, and even got my mom to switch primaries with her. One problem, the women that work in her office in the billing department are the worst people I have ever had to speak with in my life. I had several issues with billing and they were out right rude to me. My mother had similar issues and they were so rude to her in fact, that both myself, my husband, and my mother are no longer going to this doctors office. It is a shame, because Dr. Gussak is an amazing doctor and the people who work in her office ruin it for everyone. Because of them, specifically the manager in the billing department, I would have to not recommend her services for anyone.

Here we go again and this is the reason why Holy Name Hospital has such a low rating!…read more Back in August 2025 my son responded there after being in a motor vehicle accident and bumping his head just to get checked out to make sure he was fully OK The waiting area for the emergency trauma section was not terribly busy but he had waited nearly an hour to be seen. After being brought into the nurses station area they took his pulse and his blood pressure and were taking his temperature when the nurse walked away and did not come back for over 15 minutes with the thermometer in his mouth. When she did she went to do something else from what he says and didn't respond back to him for nearly 20 minutes at which time he got up and left because in his terms it was unreasonable to be treated in that manner especially after hitting his head and something could've been seriously wrong with him. After a month and a half we received a notice from the hospital billing that they were not paid for a $600 bill so at that time we submitted the bill to our insurance company GEICO and we began to have issues because Holy Name refused to submit the appropriate bill with itemized services so that GEICO could appropriately pay the bill. This went on two more times with the HOSPITAL refusing to submit the proper bill. In early December 2025 for the second time I contacted my auto insurance company GEICO who took holy names phone number in personally contacted them and my adjuster Joelle assured me that the bill would get paid because she had spoken to holy name. Fast-forward to April seventh 2026 and now my son is getting bills from a legal team collections stating that holy name was never paid and they are collecting for them well you have to contacting GEICO they advise me that after two person the person called phone calls the hospital still has not submitted an itemized bill for them to pay otherwise they would've paid it promptly. The insurance company explained to me that many times these bills are created in fraud and need to be verified and sent with the proper treatments with their codes so that the insurance company can pay them and holy name has been refusing so far to comply with the normal standards yet they're doing everything to avoid paying the bill in the appropriate manner meanwhile screwing my sons credit record up.
