The current management needs to be trained on how to run a facility, a complete overhaul. Manager needs to be Fired immediately!!!!! The place is dirty, filthy especially the showers. The food is not fit to be eaten, especially by SICK PEOPLE. The physical therapy sucks, of poor quality and NO attention to detail. This is long but, but needed to adequately detail my experience. I had an operation on my right leg and my doctor recommended an in-house rehabilitation center for about 10 days. I had been a patient at another facility on two separate occasions where I received excellent care. I was hoping to return there. They could not accommodate me. I was given three choices. I choose the Lakeside Pavilion. Big mistake. My brother dropped me of. We entered the lobby. I immediately realized the magnitude of my error. The first thing that hit me was the building itself. It appeared to be a converted motel. The smell of the place hit me next. An overall odor of decay. My brother tried to convince me to leave right then. I decided to stay at least the night. So, here is what happened in the 16 hours I spent at the Lakeside pavilion. It was 4:00 PM, before I was released from the hospital to go the rehab center. My brother and I entered the not very clean lobby and told the receptionist who I was and why I was there. She seemed to not know I was coming. There was no, "Welcome to the Lakeside, we will take great care of you. We will help you to heal." This is the norm. Not even a "Mr. Ryan, Ms --- will show you to your room." What happened is the receptionist motioned to someone and said, "Take him to room 97." I was silently wheeled to the room and left to get into the bed. No one came by let me know the routine. When the PT and OT sessions where etc. A nurse (LPN?) came by and took my vitals. She was nice, but not very helpful. She told me a doctor would be by in the morning to explain things. The bed was old and very uncomfortable. Saggy mattress. The sheets were very thin and a bit dirty. Thin blanket. I made my way to the bathroom, like the rest of the Lakeside, dirty and smelly. The toilet was chipped. I put my shaving kit on the sink and went back to my room. At about 6:00 a tray was brought in. Some sort of beef in congealed gravy, mashed potatoes, beans and some kind of fruit cup. The appearance, smell and presentation took any appetite I may have had. I took one bit of the fruit cup. It may have been apple, but uneatable nonetheless. A sip of what I think was some sort of cranberry juice. I covered the plate. I then called my brother and told him to pick me up in the morning. After an uncomfortable night I called my doctor's and spoke to his fantastic aide. She said to come in right away. I packed my bags and prepared to leave. When they asked where I was going and why I taking my clothes, I lied and told them I had a doctor's appointment and was taking my dirty hospital clothes. At the doctor's office, I told the doctor what a bad place the Lakeside was and had to get out. He gave me a letter saying I should be released to home and receive in-home PT and OT. With the letter we returned to the Lakeside and informed them that I was leaving. This is when the senior staff, so to speak, went into a full court press. Three or four professional dressed Lakeside personnel. I believe one was a nurse (RN?), a couple of director-types etc., and the center's part-time doctor. I was told I could not leave, despite my doctor's letter. I was now under the care of the Lakeside doctor and only he could release me. I said I am leaving. I was told I may be liable for all the charges incurred since Medicare would not pay if I left early. I was made to sign a letter to that effect. We spent some time detailing our impressions of the Lakeside Pavilion. All of those were disputed. In talking to the part-time doctor, my brother asked him if he would allow his mother to be treated at the Lakeside Pavilion; his response was "Yes, under my care." It is telling that he added "...under my care." Meaning that he would not want is mother treated at the Lakeside Pavilion if he was not personally treating her. My brother remarked that the dogs at the greyhound track were treated better. I whole heartedly agree. I must also add that in my haste to escape the place, I forgot my very nice leather kit bag and phone charger. Repeated attempts to retrieve it proved fruitless. After I left, my brother did a little more research on the Lakeside Pavilion. It is owned by a large conglomerate specializing in treating Medicare patients. They have facilities nation-wide. It was in and out of bankruptcy in 200 /2001. It pays its employees poorly. OVERALL : The Lakeside Pavilion is a horrible place; dilapidated, dirty, smelly, and an uncaring staff. Since I was not there long enough, I am unable to comment on the reason I was there in the first place, physical and occupational therapy. Based on what I saw in my admittedly short 16 hours, I do not have any expectations that therapists are any different than the rest of the staff. Please, do not under any circumstances go to the Lakeside Pavilion. If you love anyone whose care you are responsible for, do not allow them to go the Lakeside Pavilion. read more

My father was put in here for rehab after being in hospital we had 2 Choices due to him being a Veteran. We first walked in no receptionist a random nurse came over to us after 5 min standing there waiting for someone to come to desk. We explained my father was being transported here and then finally someone in charge helped us... it Def seemed like a very run down place and smelled just like a nursing home which it seems like it is rehab/nursing home. Anyway it's dirty and could use an upgrade!!! Beds were old and floors were dirty. Doctors come around 2 or 3 days a week not everyday apparently. Some nurses were excellent but I had to be firm with them and explain this is what we want and what he wants, so you have to be on top of them!! It's a shame because the place has potential. Food was not bad except soup my father never ate that once lol but you get options for all 3 meals what you want. We are having problems now after 3 weeks getting his records from this place. Called several times to get his records and we signed a waiver everything to have them because it is his right so please make sure you get records before you leave. We told them We would be picking him up at 10am and we didn't leave till 1:30 pm because of all the paperwork and waiting for doctor so please be prepared for that and say you want the person to be released at 9am and be prepared for 2 hours later to leave. I'm Thankful we found a place for my father but was worried for him because he was not happy there and just seemed like a depressing place to be due to the looks and smell of the place! read more

This is a very mixed review for Lakeside Pavillion in Naples. My father was there very briefly in the summer of 2013 with Hospice care, and he passed away there after one week. My review is based on that experience. The good: some of the nurses were excellent, compassionate and skillful professionals who treated my family and my father with respect and good humor. The administration knows just what to say to anxious family members. The bad: for a facility that specializes in end of life care, they were remarkably hamfisted and unprepared to deal with a Hospice situation. The rooms are small, cramped and uncomfortable, with no place for family to sit and spend time with their loved ones. The director of nursing was all for removing the catheter and "just providing good incontinence care" but the aides who provided that incontinence care used wet paper towels to roughly clean my father, leaving him with shredded bits of wet paper clinging to his privates. When we complained to the director of nursing about this, she said "that tells me they don't have the right supplies." Which is true - but for the love of pete WHY is a nursing home lacking the proper supplies for incontinence care?? Why did the aides not get the proper supplies before cleaning up my dad? Why were they so rough with him when he was vulnerable and helpless as a newborn? They were late with pain relief meds, resulting in an emergency trip to the hospital, a move my dying father did not need and could not tolerate very well. Like the nurses, some of the aides were lovely, competent and caring people, and some were rude and cruel to both my father and his wife. I was afraid to leave him there alone. The day before he died, my father's wife received a notice of a plan-of-care meeting that was scheduled to take place in 2 weeks time. It was obvious when he got there that his stay would likely be short and why did they delay this meeting? It seemed like insult upon injury when my stepmother got this letter the day before my dad passed away. I have to say, if you have the family resources to have somebody stay with your loved one 24/7, you might get consistently good care from Lakeside Pavillion - if you monitor every single thing yourself and communicate with the administration whenever things go awry. You'll need to be a constant and fierce advocate for your loved one here. read more

My dad was brought to Lakewood over a year ago. He was in a horrible nursing home in Sarasota where he was drugged and ignored. By the time I got him to Lakewood, he arrived in a fetal position with wounds all over his body, unable to talk. He now talks up a storm, is moving well, and is thriving here. The staff is amazing, caring, helpful. The place is clean. And they are dedicated to providing my dad with the care he needs without having him drugged into oblivion. I love this place! This facility is not very sanitary. The kitchen has roaches and employee rarely wear gloves when preparing food. Sometimes there are food shortages due to budgets.The facility often has plumbing problems and kitchen appliances outdated and often needs repairs. My Uncle was here for a week. He sat in his urine for 4 hours the first 2 nights. Then he got a bed sore that the staff tried to deny. Then he went lethargic and I pleaded with staff to do something and they refused. Long story short he ended up in the hospital sepsis and his blood pressure in the tank, his heart rate through the roof, and his temp sky high. 24 more hours and he would have died the ER nurse said. That was after the Dr. supposedly saw him that day. The only thing that saved his life was that I got the staff to see his bladder was ready to rupture from distention. So they finally sent him to the hospital when they couldn't get a catheter in him. read more

I will not recommend this place. My father was admitted to this pave through NCH so we didn't have a choice. But after a couple of days stay we opted to take him out and continue elsewhere. The Place is not clean; majority of nurses are unkind and not caring. It looked like they are doing is a favor answering our questions! Nurses aids are somewhat better. Food is not good. Medication were not given on time. As a matter of fact they neglected to give him his antibiotics for three days!!! His blanket had holes in it. Oh my lord .No one wants to put a parent in a nursing home. So we are all a bit hyper sensitive . Janet the director of admissions bent over backward to help me get my mom in. The staff are wonder and caring. Jennifer the charge nurse is amazing. Janet the director came in to fix my mothers tv. Who does that.? This place is exceptional. I love my mom. I trust them totally.
