    I was referred to physical therapy through Spine & Nerve Diagnostic Center and saw the in-office…read morephysical therapist. My first PT visit was fine... light pressure and stretching, no real improvement but no worsening either. At my second visit, the office was not open at my scheduled appointment time. There was no check-in process or intake paperwork, and another patient was turned away due to scheduling conflicts. This session was much more intense than the first, and I left feeling sore. Over the following days, that soreness turned into acute pain that I had not experienced since before treatment. When I contacted the office for guidance, I was told to schedule a follow-up with my doctor. At that visit, I requested copies of my PT treatment notes. Initially, I was told they could only be released to my attorney. Then I was told the PT was affiliated with another office and that I'd need to contact them -- but that office had no record of me since I'd never been seen there. A front desk staff member was dismissive and unprofessional during this process, even discussing my request with others in a way that I could overhear. After multiple attempts and delays, I finally received my records, but the process was unnecessarily difficult and raised concerns about accuracy of said records. Based on my experience with both the therapy and the administrative handling of my case, I will be seeking care elsewhere.
