This streamlining for me is coupled with the extremely generous amount of time that doctors spend during appointments. I feel that the concierge practice model has provided care far exceeding what my old local medical practice had become after losing a few key physicians through retirement. I'm not sure if they are accepting new patients, but if Lifetime Internal Medicine is, I highly advise trying this 'small' team concierge solution as a return to times when medicine was practiced by Doctors instead of Doctors applying a one-size-fits-all template strategy for the codified roll-out of big-med:-)

But nobody, not even the good ones, can touch the level of care that Shor provides. He is off-the-charts smart, and he's dedicated to highly individualized care. He is open-minded but conservative, and his instincts are to be relied on without hesitation. He is also an exceptionally kind and decent man. When my daughter was quite ill and running low on options, Shor stepped in and saved the day. Since then, he's provided her with first-rate care as her general practitioner. However, I later received an unexpected separate office visit charge in addition to the preventive exam billing. My concern was not the care itself, but the lack of transparency regarding billing. During the wellness visit, the provider briefly reviewed outside lab results I had brought with me, and I was later billed for a separate office visit associated with that discussion. I was not informed during the appointment that this could result in an additional charge. After reviewing my visit documentation, I also felt portions of the note did not fully reflect my understanding of the visit, particularly regarding the extent of evaluation performed beyond the wellness exam. After raising concerns and providing supporting documentation, the office acknowledged overlap between the preventive visit and additional billing and indicated the matter would be reviewed. However, I did not receive a final follow-up or resolution regarding the coding review or reimbursement request. I would encourage the practice to improve communication around preventive visit billing and provide more consistent follow-up during billing disputes.

I was recommended to come see Dr. Thathagari at the NOVA Medical Services Center from a co-worker of mine who too can relate to my nonsense hypochondriac concerns. I thought I had meningitis (turns out it was more or less just a bad headache) but Dr. Thathagari answered my countless questions and concerns with a high level of professionalism but also with a slight sense of good natured humour. One thing that is comforting is if you call the office after hours if you have a question or suddenly get sick, Dr. Thathagari always checks the office voicemail and sometimes calls are forwarded to her cell phone. That is SERVICE if I've ever seen it!! I've never had a doctor do that before. So awesome. It's nice to know that doctors have that much concern for their patients. As for finding appointments-- I was able to get a weekend appointment with no problem at all. Their hours are pretty flexible and the office is open, at the latest, around 6 pm on weekdays. So if need be, you can schedule an appointment and not have to take time off from work-- a win-win situation. One thing to be wary about is that this doctor's office is shared with a pediatrician's office-- I rolled up to the office and I got some odd looks-- people must've thought I was lost! Rest assured though, you're definitely in the right place.

Everyone from the admin. staff to the nurses, interns, students, residents and ... show such generosity of time, care, consideration, and love to all my parents' requests and questions with kindness and smiles. Today, I walked in to the office with my two elderly parents for a follow-up visit and were received with such patience and care that it immediately lifted our spirits and made the uncomfortable medical environment so comfortable because the interactions with those professionals were imbued with kindness and consideration. Thank you, thank you!!! We are grateful because we know this kind of treatment is very rare in this industry. We highly recommend Dr. Milani and his staff!!! One of the reasons I rarely had to see a specialist as a young woman was that he was so creative and aware of what was at the cutting edge of medicine in many different fields. He really was great at taking the initiative and not letting.patients hide from their health issues. My tendency was always to want to use an Rx as my proverbial bandaid for a problem and move on. Now, too often I feel like medicine is prescribed piecemeal to treat chronic symptoms, rather than putting the pieces together and treating the cause. Just my 2 cents! But Dr. M really does continue to go above and beyond for his patients in terms of communications and compassion. He's a good guy, through and through.

No one, from the PAs to the doctors, seemed to have a clue who I was or what my intake paperwork/lab results said until repeatedly prompted to look. Why bother taking time to write out what prescriptions I'm taking or my medical history if no one was going to read or even mention it? When the doctor came in, he skimmed the latter and told me everything was fine, even though several tests had come back VERY abnormal. Upon being called out on this, he just hemmed and hawed, and looked very unpleasantly surprised that I had known to access my results online through LabCorp. On a side note, the patients I saw in the waiting room were overwhelmingly elderly and/or immigrants who struggled with English. 2. My BP was ridiculously high and I wanted to discuss it at the appointment, as it had been that way for a while, despite no clear reason for it to be that way. I'm on the young side, not overweight, and actually struggle with LOW sodium. The PA retook it 3 times trying to get a number he liked better, commenting all the while that I must be nervous about doctors, and if not that, should cut down on salt intake. But...my very low sodium was there in the lab work that had just come back. He finally said the doctor would probably need to put me on meds. Well, I never found out, because the doctor was in such a rush to get out, (after I had waited for a full hour) that he was in and out in under 10 mins. For the first bit of the appt, he chose to hover in the doorway-- to the extent that he was actually blocking his own scribe from advancing into the room. I spent the next 3 days in bed with a monster headache, feeling like my eyes were about to pop out of my head, and being indecisive about going to the ER *spoiler alert: I should have, high BP is no joke. I could go on about my incredulous attempts to get him to engage with me; or at LEAST stop trying to escape the room, but I think you get the picture. Weirdly, on top of his practice not having a patient portal of its own, I was asked (using my previous doctor's portal) to personally download my medical records and test data, print it all out, and then bring it in to the office for the admin team. Why they couldn't contact my previous doctor themselves to transfer my medical records directly is really still not clear to me. Moot point now, as I certainly won't be returning. So to summarize? Dr. Mehra and his team presided over the single WORST experience I've ever had in the entire medical system, and I've had some pretty crazy experiences over the past decade as a chronic illness sufferer. Please, do yourself a favor and go somewhere, anywhere, else. He may be a good choice for something like workman's comp--based on signs posted on the walls it seemed like his office was geared towards this already. But even if you're restricted by Medicaid, find a different doctor. Literally ANYONE would do a better job than this mockery of the Hippocratic Oath come to life. Avoid.
