While there were some positive aspects, significant concerns about staffing, communication, and quality of care ultimately led us to move our mother-in-law to another facility. On the positive side, the food was very good. Residents were offered choices and could eat in a restaurant-style setting, which she genuinely enjoyed. Additionally, some staff members were exceptionally kind and went above and beyond in their care. However, there were consistent issues that impacted her well-being. Planned activities were frequently canceled due to staffing shortages. Because she is not mobile and requires assistance to attend activities, she often depended on staff to inform and help her participate. Without regular family advocacy, she was not consistently included. About a month into her stay, she was moved from a shared dining table with other residents to sitting alone--this occurred on her birthday. Given that she was adjusting to a major life transition and grieving the recent loss of her husband, this change was especially difficult. During her stay she was not consistently being taken to meals and lost a lot of weight in a short amount of time. We also observed concerns with staff presence and responsiveness. At times, staff appeared disengaged, sitting on phones or eating in common areas, making it unclear whether they were on break. Staffing levels seemed particularly limited during evenings and weekends. More concerning were issues related to personal care. During her stay, she lost 10 pounds and experienced multiple UTIs. There were several occasions when my husband arrived in the morning to find her still in the previous day's clothes, in wet incontinence garments, and with soiled bedding. While staff sometimes stated she had "refused" care, we were not confident that consistent or sufficient efforts were made. The call button system was also problematic. It required residents to hold the button down for an extended period before activation, and response times were often delayed. Some staff members were unfamiliar with how to properly reset the system so my husband had to show them how to use it. Although we requested the simple old style call button, we were told none were available, despite observing newer residents being given the old style buttons. We also encountered issues with missing personal belongings. A gold chain was replaced with a non-gold one, and a new TV remote went missing. Additionally, our mother-in-law reported that a staff member asked her to write them a personal check, which we immediately reported. Another resident nearby also reported missing cash. As her condition declined due to UTIs, she required the use of a Sara Stedy transfer device. We had to purchase this equipment ourselves and train staff on how to use it, as they had not received prior training. Eventually, we were advised to move her to memory care for more support, but instead we chose to relocate her to another facility. Since moving, her condition has improved significantly. She has regained strength, is walking short distances, maintaining her weight, bathing regularly, and is consistently dressed and cared for. While there were some compassionate individuals on staff at Brookdale, our overall experience did not meet expectations for the level of care being provided, especially since we were paying for the highest level of care. I was impressed with the intimacy of the facility because they only has 24 residents and they specialize in Dementia Care. Their staff is caring, attentive and they get to know each persons individual needs. They know my voice when I call and they are respectful to both his needs and mine as his caregiver - Are they perfect NO - Do they make every effort to take care of any situation and communicate - YES - If I had to do it all over again I would still pick Brookdale - Their Staff is the BEST

After a few weeks there, we got hospice involved. We chose Compassionate Care because they already had a nurse coming to the facility on a daily basis. We mistakenly thought that this would mean that it would be a smooth transition. We were wrong! While the nurse was pleasant and was able to get equipment moved in in a timely manner, we were VERY disappointed that they did not communicate and coordinate the necessary pain relief medications our mother needed. There were unnecessary delays in obtaining and starting the drugs, and getting them up to a level where they actually did any good in relieving her pain. I literally had to INSIST that they up her dosage! Having gone through similar situations with our other elderly parents, my siblings and our spouses are agreed that we absolutely cannot recommend Compassionate Care - Amedisys to care for a loved one. A not-for-profit like Hospice House in Pleasant Prairie would be a much better choice, and they did an outstanding job 12 years ago for our father.

BEWARE this facility has had over 7 Director's of Housing in the last 4.5 years. The bar is extremely low. If your loved one is in need of actual assistance, say Level 2 or 3 acuity they are not equipped nor willing to care in a dignified manner. My mother moved here in early 2020 with the same level of care needs as when she was forced out in late 2024. The current housing director, Candice, began a campaign to kick her out in June 2024 using emotional abuse, retaliation and bullying tactics. She repeatedly entered her room without permission and told her she was "feeble, and too much work." She insulted and dehumanized my mother and instructed caregivers to strip her of care needs she had been receiving for years. While some caregivers complied with these dehumanizing instructions, others did not (thank you to the few and shame on the rest!) In addition, the food is of low quality, with the same basic menu repeated every week. Bland picnic food such as hotdogs and potato chips are served multiple times a week with nary a vegetable in sight. There is NO DESIGNATED CLEANING STAFF. The caregivers are expected to clean the rooms on top of all of their other duties. This is simply not possible so please enjoy filthy rooms in unhygienic conditions. I am consciously choosing not to name caregivers in this post as I fear retaliation for them. But my hope is that if you have a vulnerable or disabled senior in your life you will read this review and look for another location for your loved one. Otherwise, plan on visiting MULTIPLE times a week/month until they make serious changes in management and find a competent Director of Housing. What a sad and disgraceful end for my family at this residence.

They have abandoned patients in the ER and refused to let them come home because they didn't want to provide simple wound care. Their staff are rude on the phone and uncaring. Think twice before sending someone here, it's very sad and sinister.
