The employees are well trained and clearly have had high expectations set for them. The campus is so serene and beautiful, it feels like a resort. Sort of like a vacation whenever we come to visit. This review is specifically for the skilled nursing and rehabilitation services. After a 1 week…read morestay at the local hospital my grandmother was recommended to Meadowood at O'Connor. She was septic with pneumonia. She was alert, moving, talkative and cheerful when she arrived here and within her 1 week stay she took a drastic turn for the worse. I feel this place is severely understaffed. Each day we noticed the lack of care she was receiving from the staff. We could also hear other patients in rooms nearby shouting for help and nothing was done for them either. The staff also mistook her for another patient so the fact that they could not even remember or keep track of her needs was alarming. They were giving her all the wrong foods, liquids, and hardly any medication to improve her health and get her strong enough to come home. Even after filling out their little cards for specific food and drink instructions the changes were not made. Within 1 week she went from cheerful and talkative to being so weak that she was unable to feed herself or formulate full sentences. The PT staff said she was progressing so well she was walking herself around to the dining area and would be released soon while the actual nurses were saying she was so weak they couldn't even assist her to the bathroom anymore. Talk about inconsistent and polar opposite feedback from the staff about the same person... since they couldn't help her to the restroom anymore she was going in the bed and constantly needed to be changed which they would take 1-2 hours to complete which is a bit dehumanizing and humiliating to have to sit in and after they changed her and the sheets they did not put her blankets back on her - mind you this was in the middle of winter. She was shivering and curled up in a ball with blankets on the floor when we saw her. She was severely dehydrated yet the entire time they did not have her on any IV fluids to get her body the nutrients it so desperately needed. After claiming she would be released for HOME (again I have no idea how they were able to say this to us when they we were clearly not talking about the same person) she was actually transferred back to the local hospital. From there the local hospital deemed her health was so irreversible that all there was left to do was to take her to hospice care. She passed away only 4 days later. I blame this facility for her rapid decline and I also blame myself for allowing her to stay here. We should've taken her out after all the mistakes and neglect here. When she arrived here she was her full self and when she left here she looked skeletal. She lost her figure and you could tell she was dying. Please think twice before taking your loved ones here and keep a close eye on the care they are receiving. If you feel like this is not the right fit, listen to your gut and find a different facility. It may look nice from the outside but the care is some of the worst I have seen.

My grandmother had a bladder infection that was not taken care of at one point and had to be admitted to the hospital. I would walk in the dinning hall and my grandmother would be faced towards the window with her food placed right in front of her (which she couldn't feed herself) she lost so much weight while being here and always looked over medicated. The staff was never friendly and was always watching me, which made me feel very uncomfortable. I would never send someone here. This place needs to be closed because not even a dog deserves this type of care.

She is well cared for and happy at Golden Haven. She has been there for a year now and is still very happy. Golden Haven is a large facility on 2.5 acres where residence can freely walk around because the grounds perimiters are secure and monitored. This gives residents the sense of independence and freedom they really need. The rooms all have sliders to enclosed outside courtyards where they are free to go again giving them the sense of independence. There is also a lovely main garden courtyard for anyone to use. The hallways are really wide and have skylights, that make it bright. The rooms are decorated with nice wooden furniture all with connecting bathrooms and private showers.There is daily activities for residence and something is always going on. They just had a Senior Prom last month. They even have a beauty parlor. The caregivers are very nice and seem to know residence by name always saying Hi and calling my Mom by her name. Many doctors come to the facility for checkups and they have a Medic staff at the facility that dispenses prescriptions. The place is run by Dr. Ramirez and she is great! She is really on top of everything. Lisa runs the office and is also great. Always there to answer questions and make sure everyone is happy. This is a great place they really care about there residents.

I met with them several times and was impressed with their care for their residents in memory care and their staff is also as caring and kind as they are. Typically you find highly processed and bland food at homes like these but when mom sat down for her first lunch I was so surprised at the deliciousness of their food which is cooked onsite (I have a good source there who advised me of this). It was so good I wanted to eat lunch too. Additionally, they serve fresh, delicious snacks, not anything pre-packaged. Mom tends to want to sleep much and so we put her on a schedule to keep her busy as she is very high functioning, and when I showed up the following day there she was interacting and happy. The staff are constantly checking in on her and are always available when needed without delay. I'm so happy to have made the move to The Commons for mom as we FINALLY found the right place for her.
