    This review is specifically for the skilled nursing and rehabilitation services. After a 1 week…read morestay at the local hospital my grandmother was recommended to Meadowood at O'Connor. She was septic with pneumonia. She was alert, moving, talkative and cheerful when she arrived here and within her 1 week stay she took a drastic turn for the worse. I feel this place is severely understaffed. Each day we noticed the lack of care she was receiving from the staff. We could also hear other patients in rooms nearby shouting for help and nothing was done for them either. The staff also mistook her for another patient so the fact that they could not even remember or keep track of her needs was alarming. They were giving her all the wrong foods, liquids, and hardly any medication to improve her health and get her strong enough to come home. Even after filling out their little cards for specific food and drink instructions the changes were not made. Within 1 week she went from cheerful and talkative to being so weak that she was unable to feed herself or formulate full sentences. The PT staff said she was progressing so well she was walking herself around to the dining area and would be released soon while the actual nurses were saying she was so weak they couldn't even assist her to the bathroom anymore. Talk about inconsistent and polar opposite feedback from the staff about the same person... since they couldn't help her to the restroom anymore she was going in the bed and constantly needed to be changed which they would take 1-2 hours to complete which is a bit dehumanizing and humiliating to have to sit in and after they changed her and the sheets they did not put her blankets back on her - mind you this was in the middle of winter. She was shivering and curled up in a ball with blankets on the floor when we saw her. She was severely dehydrated yet the entire time they did not have her on any IV fluids to get her body the nutrients it so desperately needed. After claiming she would be released for HOME (again I have no idea how they were able to say this to us when they we were clearly not talking about the same person) she was actually transferred back to the local hospital. From there the local hospital deemed her health was so irreversible that all there was left to do was to take her to hospice care. She passed away only 4 days later. I blame this facility for her rapid decline and I also blame myself for allowing her to stay here. We should've taken her out after all the mistakes and neglect here. When she arrived here she was her full self and when she left here she looked skeletal. She lost her figure and you could tell she was dying. Please think twice before taking your loved ones here and keep a close eye on the care they are receiving. If you feel like this is not the right fit, listen to your gut and find a different facility. It may look nice from the outside but the care is some of the worst I have seen.

Her transition from independent living to assisted was very smooth. After settling in, mom was invited to join in on the daily planned activities where she quickly made many new friends. With open arms our family was welcomed with invitations to Family Events throughout the years. We also made new friends with other family members! When our grandchildren visited mom they made wonderful memories joining her in activities, games and getting to know the other residents. Mom received loving care in her last days. The med techs and caretakers went above and beyond, showing us respect and mutual concern for mom's comfort. I will be forever grateful for the entire staff, administration, med techs caretakers, activity, house keeping, janitorial and dining room. they should all know what a difference they've made in our family's life. I will be forever grateful.

    The Oaks has recently went through a ownership change. Food and menu has greatly improved…read more Management and individual Directors have remained the same. Thank goodness for that as they are great at their jobs. Hopefully the new owners will approve some landscaping work and exterior painting to improve the appearance of the facility. I live here and truly enjoy the people here.

    Horrible! INHUMANE! Please stay away from this place . If you really love your family members…read moreplease don't bring them here. My grandfather had hip surgery and I fell for the "new name and management" of this place!!! This was my biggest mistake they had my grandfather on a wheelchair on strong pain killers literally sleeping with his bad leg that he just got surgery on "resting" on a Damm plastic trash can! They tried taking him to "PT" while he was on these strong pain meds that literally made him unconscious. That is a fall risk itself. He was at this facility for 3 days and that's only because I was afraid of not having everything set up at home for him. I took him out as soon as I could there was no way I was leaving him. He had a UTI because he had to hold his pee for hours because no one would go help him up. We picked him up to the restroom not the "nurses" this place is heartbreaking I literally cried those 3 days when I had to leave. They have roaches and they expect for him to use broken walker with tape all over it. Are you insane ? They need to shut this place down. So many people yelling for help and no one would help them. The van that transports is only doing there job of dropping off at the doctors appointments not one person from this facility went with my grandfather if you don't go they send them on their own and if they miss the van they have no clue. This happened to us but thankfully I drove behind this van and went with him to his appointment and that day I took him home. He recovered fantastic at home even after the "nurses" and the "director" told me I was making a mistake taking him because I didn't have a special bed ? They don't have anything that was helpful for him I'm so happy I followed my heart and took him out !
