Call lights are answered, patients are clean and dressed properly, and the environment is calm. The care team has been professional and responsive whenever we've needed assistance. Overall, this facility has shown consistency and care, and we are comfortable with the level of service provided.

The patients are in the hallway waiting for her while the front desk cannot locate Jasmine. She finally shows up says she's busy at the front desk could not locate her because supposedly she was on lunch. Later on I find her in the hallway chitchatting with other staff and they're cussing just very unprofessional this place I do not recommend bringing your family here. My uncle has complained about this Jasmine also but nobody does anything about her. Very unprofessional she doesn't even dress in proper uniform. If she does not like to work here why is she here? Patients matter here, but they seem to not matter to Jasmine. Who is running the show here, Jasmine? There needs to be an accountability. The patients have rights.

Meadow Creek (A Summary of RESIDENT RONALDO'S EXPERIENCE) I…read moream rating this Meadow Creek facility as a five-star nursing home based on my husband's experience as a resident and my experience as a family member. In doing this task, I can't help but reminisce about the experiences my husband, Rolando, and I had with a nursing home, as his wife. That is, Meadow Creek (MC) is the third one. My husband was discharged from this facility on Easter Wednesday, April 5th of this year after his six-month stay. I will limit myself to a description of Rolando's experience solely here at MC, but I would say it is the most satisfying of the three. What makes it so, FIRSTLY, is the sense of united purpose I observed at MC. I have seen this through the orchestration of the performance of duties by the various departments, under the able leadership_ in particular, of the Director of Social Services. The staff, in general, desire to care, and this desire was evident in their care for the residents. Iwas stunned by the management s respectrul approacn BS reminding the staff, residents, and their families of the facility's protocols and each person's duties and responsibilities. And likewise, by the cooperation of everyone, including myself as a resident's family, SECONDLY, I noticed that, because of the staff's desire to care/serve, a meaningful patient connection was evident. For me, this weighed more than the technical training the nursing staff had undertaken. Working one-on-one with residents, as with my husband, I could feel the nurse/nurse aide was learning to build trust while providing care. And building trust was not easy. But they were able to accomplish this with my husband while providing care. THIRDLY, I was astounded by the staff's ability to manage time, prioritize tasks, and understand the fast-paced workflow of the nursing home setting. They prioritized helping others in moments of urgency. This was easily seen when my husband was brought to a hospital for pneumonia. It was early morning. I was happy for such a quick move. The second time, the nursing home sent Rolando to a hospital because it was unsure about the reddish stool. He was sent back to the nursing home on the same day with a diagnosis of nothing to cause alarm. To me, those were gestures that they truly care! FOURTHLY, I can say that there are greater opportunities for growth towards excellence that are certain to take place in this facility. A high sense of professionalism, confidence rooted in spiritual strength and grace, and a strong work ethic, which characterized the administrators and staff, were all positive factors in the facility's performance. With a bit more clinical training and patient-based learning experience among the nursing staff and ancillary facility aides, the facility can achieve more than expected. To conclude, my husband's stay at Meadow Creek was a memorable part of his journey from illness to the restoration of his health. His experiences molded him into someone who aspired to a normal life. I stayed at the Country Villa Bay Vista Healthcare facility from December 14, 2022 to January 14,…read more2023 for physical and occupational therapy following a fall down a flight of steps at my apartment complex that initially rendered me unable to stand and walk due to severe injuries to my knees and quadriceps (thankfully nothing was broken). When I arrived at BV around 10:30pm that first night (my transport from the hospital was very late in picking me up), the staff was very welcoming and helpful, which helped to ease some of my overall uncertainty being in a situation where just a few days prior I was mobile and living my life, to now being immobile and unsure of what was going to happen next (I'm 52 years young). The two CNAs who tended to me that night were soooo kind, caring, helpful, generous, and understanding, I started feeling more comfortable in my new environment and situation. And such was the care that I received overall during my stay at Bay Vista--the nursing care including the medication nurses, the physical and occupational therapists, the patient advocate and social worker, the activities director, the kitchen director, the administrator--everyone offered such wonderful and friendly care and service! And the housekeeping staff were great at keeping the facility and my room clean. During my time there I went from being unable to stand and walk to being able to do both (with walker and wheelchair support) thanks to the amazing occupational and physical therapists! They were so supportive and helpful (and such sweet individuals) and were the perfect team to build the foundation for my recovery journey (and I continue getting stronger at home). When I was having challenges with my insurance company, the patient advocate kept me updated and went to battle on my behalf so I could focus on my healing. She even took the time to comfort me during a particularly stressful experience I had with them. And huge props to the CNAs--the sheroes and heroes who do the actual day-to-day, moment-to-moment work of intimately caring for the patients--who were excellent. Admittedly some were better than others (like with any job), but they were all good, and many went that extra mile to ensure your comfort. I speak not only from the care I received, but the care I observed with my roommates as each of us had different levels of needed care. I could share many instances where the staff exhibited such care, professionalism, support, and generosity with us (including with some of the more demanding [and impatient] patients :-). And although I was happy to finally go home after my stay, leaving was a tad bittersweet *only* because I was going to miss those staff with whom I had developed relationships, especially my therapists. But I look forward to going back--just to visit and say hi to everyone after I have fully recovered! I would definitely recommend Bay Vista to those needing nursing and PT/OT care.

Thinking about admitting my dad from the hospital to a nursing facility for a few days was a very…read morescary and hard decision. He needed the medical care I couldn't provide for him right away. I needed to weekend to prep my home and make calls for nursing care. In the meantime in the hospital the social worker connected me with Administration rep SAM who helped ease my worries and assured me my dad would be well cared for. The weekend my dad was there the nurses helped my dad with medication ,food and his bathing in bed. I was able to drop off clothing necessary and the nurses would take them back for me while visiting hours were over. The issues I ran into were: scheduling appointments were not communicated well amongst the staff. I came in a Saturday and the young receptionist as well as another young lady said I couldn't have had an appointment as I was not on the list. However I was just there Friday and they said I could come in the next day at a certain time. I won't spend much time on this as Sam helped fix this issue and I was very frustrated with these ladies, but given they only go by what their told I get it. Aside from that my dads needs were met.
