If I could give this place "0" stars, I would…read more This facility is shockingly understaffed and I would NOT under ANY circumstances, recommend ANYBODY using this facility. I have a family member there and when visiting them after check-in, could NOT, get an orderly, nurse or any staff member to check on him! I was told 'we are too busy with other patients!" I am still in disbelief at the treatment he and I received.

In this review I'd like to cover 3 things:…read more Where Dad was when he entered Lemay Where he is now Why you may not want believe me The Past Dad was admitted to McKee hospital in Loveland on the last day of June this year with a diagnosis of aspiration pneumonia and a very angry gallbladder that needed removing. Three weeks later he was ready for discharge but he could not stand without a lift, attend to his own toilet or other needs nor move without significant assistance. He was sleeping almost continuously, and as we were to soon find out, was deeply depressed and was considering taking his own life, just short of his 89th birthday. He just didn't see a way through. Given his condition, his inability to care for himself and the fact that Mom is also in her upper 80s, Kaiser authorized 3 weeks at a skilled nursing facility, which was later extended. Lemay was on the list of approved facilities and had a bed, but more on that process later. The Present This past weekend, here in mid October, Mom, Dad, my sister Sharon and myself headed up to my parent's cabin in the mountains to enjoy the last of the season and I'm happy to report Dad is doing great. He still needs a walker to get around and we have caregivers coming in a few hours a day but Dad is getting his independence back, and is bright and hopeful for the future. On Saturday we were sitting around talking and Dad volunteered that he "100% credited Lemay" for his recovery. The hospital saved his life, he said, but Lemay, and particularly the PT team, (looking at you Anne), brought him back from the brink. Why You May Not Want to Believe Me This is, by all accounts, a great success story, so why shouldn't you believe me? After all, I've done my best to give as honest an account as I can and Dad is proof of it. Because my sister, "Lorraine G" (see below), gives a very different account and to this day asserts that Lemay is a terrible place full of terrible people. So who do you believe? Maybe neither of us. Sites like this are very useful, but have their limits. Maybe the reviewer is having a bad day. Maybe the reviewer happened across an outlier day one way or the other. Maybe the reviewer has a preconceived notion that all facilities like this are places where families put old folks to die. This is why Mom made her decision to place Dad in Lemay not on a Yelp or Google review but on ratings at Medicare.gov which provides verified information about inspections, staffing, substantiated complaints and investigations from the agency that regulates them. Lemay enjoys 5 out of 5 stars there. So by all means, read the reviews here, including Lorraine's, but I'd recommend taking anything here for what it is: an anecdotal account from a source that you have no way of verifying. This ain't a taco you're buying, after all.

After a few days, he was discharged from the hospital and moved to the Suites Fort Collins for rehab. It is likely that he was discharged too earlier, but I don't think this was the right kind of facility for someone with heart disease like my Dad. Firstly, he needed to be monitored at all times and a lot of the time he was left unattended, like he was just on vacation. There is no permanent doctor at this facility, but my Dad was still in a very weakened state. The physical therapists and occupational therapist pushed him too hard and weren't listening to what he was saying. They were most concerned with the incision where the surgery took place, but didn't have much experience with a more complex elderly person with serious cardiological issues. On Friday, he was very frustrated that his concerns were being ignored and he complained that his condition had worsened since he left the hospital, where he had been making progress. He was already able to walk with a walker and assistance. The following day, we went to see him. When we arrived the staff was running around frantically. We approached my Dad's room and he was lying on the floor. Apparently, the staff had just discovered him. We heard them say, "Call 911," and they were trying to revive him, but it was too late. We don't even know how long he was there before they found him. Someone in my Dad's state needed to be monitored at all times. I blame the hospital partly for releasing him to soon and to the wrong type of facility, but the staff at The Suites, Fort Collins was simply too negligent and inexperienced.

Patients are left in their soiled garments for extended periods of time, depending on who is in charge of their care, and how motivated they feel to actually take care of patients and residents throughout the day. numerous patients that have severe dementia, and Alzheimer's issues are disregarded, ignored, yelled at, and overall treated like they are less than human, anyone that is a "inconvenience" is treated as such. Sexual harassment of swept under the rug at this business they do not care if you were being harassed, they would rather sweep it under the rug and pray you forget about it than actually do anything. I do not recommend sending family, yourself, or anyone here or working here. It's very close to being shut down and I hope it gets shut down with the amount of reports that it gets.

Residents were yelled at constantly and left untreated throughout the day and sometimes even the night. Residents would lay in their own bodily fluids for hours and hours on end. I worked there for about 3 months and throughout those 3 months I saw CNAs and housekeepers taking better care of the residents than the nurses do. The manager Todd does not care nor does he take responsibility for all of the problems in this place. I would not recommend sending your family here and I would also not recommend working here.

Therefore, I did not get out bed often, other than for therapy. I did not think the therapy would be helpful and was able to express that which changed their approach in problem solving for me when I would be back at home. Yes, there were times I had to wait for responses to my 'call' but all in all, I was satisfied with the services provided by this facility. In particular, one of the seasoned CNAs made the experience more than tolerable. He treated me as a person and not simply as a 'body' even though he had to do things that were related to bodily functions. I appreciated his care and respect for me and came to trust him quickly. Centre Ave. failed my dad in every possible way. My dad passed away in August 2024. He fell ill…read morejust two days before being transported to the ER. On the day my dad started feeling sick, my brothers, mom, and I visited him and asked staff to check on him because he was complaining of a sore throat and having trouble breathing. Centre Ave. lost his inhaler, which exacerbated his condition. Despite our requests, no one came to assess his situation. My mom had previously also expressed her concerns about my dad potentially falling out of bed and had asked multiple times for staff to put up barriers and padding on the floor for his safety. Unfortunately, they did not take action. The next day, my mom once again asked for assistance as my dad was still unwell. She waited and waited for help, finally, they informed her that she would need to take him to the doctor herself. At that point, my dad couldn't walk, hence the reason he was in rehabilitation. This was an unreasonable and inappropriate option. My mom was understandably upset and made several attempts to get someone to help her. She left later that day, and the next morning, she received a call from Centre Ave. stating that my dad had fallen out of bed during the night and was not feeling well. They waited until 9:30 AM to inform her of this!!!! My mom rushed to Centre to find my dad propped in a chair, clearly in distress. He was struggling to breathe, and she insisted he go to the ER. It took a couple of hours for that to happen. By the time the ambulance arrived at the ER, my dad was in respiratory failure, diagnosed with double pneumonia and dangerously low oxygen levels. The ER doctor informed us that the only option was to place him on a ventilator or he would die within minutes. This was because the pneumonia had progressed and my dads condition was dire. We had to make a rapid decision, opting for the ventilator. My dad remained on the vent for nine days with little progress. We were advised that if he survived, he would never be the same and would lack a good quality of life. Eventually, we made the heartbreaking decision to take him off the ventilator, and he passed away. The employees at Centre Avenue never showed any urgency or genuine concern for my dad, and they displayed no remorse for their shortcomings. I reported these issues to the Larimer County Health Department, which conducted an investigation and concluded that Centre Avenue was not at fault. They seemed to find it acceptable for a patient to fall from his bed without notifying the family until the 9:30am the next morning. I believe my dad had fallen sometime during the night and they found him in the morning. I believe he was on the floor most of the night. They called my mom late because they knew they were negligent. Moreover, the investigation found it appropriate when there was no medical attention given to a patient who, under their care, became severely ill due to negligence. The staff at Centre Ave. did everything wrong. EVERYTHING! They allowed my dad to suffer a fall, lost his inhaler, and failed to provide timely medical attention when it was requested. As a result of their negligence, my dad's condition deteriorated rapidly. By the time any action was taken, it was too late. My dad would be alive today had they done their jobs and provided the medical care my dad so desperately needed. If sharing my family's experience can save another family from enduring what we went through, I will feel satisfied. I trusted them to care for my dad. I will forever regret the decision to allow Centre to care for my dad. DO NOT allow Centre Avenue Rehab to care for your loved ones. Sincerely, A daughter who misses her daddy:( P.S. If you remove this review, I will post and repost on every single platform I can possibly share my experience.
