Finally a manager told me rudely I had been told from the beginning (it was in the notes so it must be true) that I needed to see the original doctor I saw and they would accept no other info including insurance co. authorization paperwork. How in the hell could I have so many emails telling me I needed different stuff each time when, silly me, I was told 3 months ago what was needed! Rudest most dumb supervisor on the planet.
