The bar is extremely low. If your loved one is in need of actual assistance, say Level 2 or 3 acuity they are not equipped nor willing to care in a dignified manner. My mother moved here in early 2020 with the same level of care needs as when she was forced out in late 2024. The current housing director, Candice, began a campaign to kick her out in June 2024 using emotional abuse, retaliation and bullying tactics. She repeatedly entered her room without permission and told her she was "feeble, and too much work." She insulted and dehumanized my mother and instructed caregivers to strip her of care needs she had been receiving for years. While some caregivers complied with these dehumanizing instructions, others did not (thank you to the few and shame on the rest!) In addition, the food is of low quality, with the same basic menu repeated every week. Bland picnic food such as hotdogs and potato chips are served multiple times a week with nary a vegetable in sight. There is NO DESIGNATED CLEANING STAFF. The caregivers are expected to clean the rooms on top of all of their other duties. This is simply not possible so please enjoy filthy rooms in unhygienic conditions. I am consciously choosing not to name caregivers in this post as I fear retaliation for them. But my hope is that if you have a vulnerable or disabled senior in your life you will read this review and look for another location for your loved one. Otherwise, plan on visiting MULTIPLE times a week/month until they make serious changes in management and find a competent Director of Housing. What a sad and disgraceful end for my family at this residence.

Poppa's temporary home…read more.. Pretty standard for a nursing home type facility; splendid smell comes at no extra charge. There's a lot of coughing, a lot of "help me, help me" and a few strange patients including, but not limited to a) the whistle guy who, you guessed it, is ALWAYS whistling, b) the lady with purple headphones who just turned 100 who wants to know who drew the "no smoking" sign and c) the lady who repeatedly asks, "what if there's a fire?" Cool place, but let's hope Poppa (what I call my Grandpa) can return home soon. Until then, bring on the wheelchairs, hand sanitizer, oxygen tanks, and crazy cute old people.

Well now that I/we have had to utilize this property for rehabilitation/assisted living for my Mom, it is absolutely horrible. With in the first 4 days of being here, our Mom fell multiple times, she was placed in quarantine and no one could visit her to see what was actually taking place, because she came from a hospital setting. Once her 14 day quarantine was up, even with the Lasata Staffing knowing that they had a positive Covid testing of an employee, they moved our Mom to a different room, to start another 14 day quarantine. Luckily we were able to find and secure a new living facility for her, with in a weeks time, only to get her and see that her leg, ankle and foot were swollen. When Lasata was questioned no one would return call soft over a day. When they did call back they more or less said not our problem she's not enrolled with us any more. What a heartless caring staff you are Lasata - from the conference calls to now having a Mother in worse shape since her arrival to your facility proves / answers my unanswered questions - it's all about the money and the number of residents you keep- there is NO CARE for the resident or their families. My mother has been at this location for 6 week and the lack of basic care has been so subpar. My mother's safety has been at risk since day one (See Pictures). The staff sees nothing wrong with how she is on the wheelchairs in the pictures. Staff that doesn't know how to use the equipment properly, she goes the whole day without being checked. When pushing her button for help it goes hours with out anyone coming to see what she needs. She was there for 5 weeks and was never showered once. Simple things like brushing her teeth everyday can not even be done regularly. The final straw was when my mom was having a bad day and was nonresponsive when trying to wake her up. The young lady giving her pills decided since she could not wake her up she would take the pills and shove them in her mouth. Then after shoving them in her mouth tried to give her water and because she could not get any water in her mouth decided to leave the water and walk out of them room. Leaving the pills in her mouth for the potential to choke. This facility only has one care and that is the money that is being paid by the families. Your loved ones best interest is not their priority. The worst part is the facility puts the blame on the family and does not take accountability for their staff and their lack of care and training. I had several meetings with the directors and nothing got better.

BUT, after this year , I must say that your strengths are far beyond comparison. Here are several things that are an example of your stellar service. Mom enjoyed your pool for many months, telling visitors not to come at certain times as she could NOT miss her Aquatics class. (Not a single facility that I toured had a pool except you) Both PT, OT and the other support services have been so helpful in coming forward with suggestion of products and things that will help Mom, before I even knew she needed them. When Mom was beginning to show signs of decreased verbal skills, staff called, recommended and enrolled her in Speech Therapy Caregivers of all shifts are so willing to share things (both the good and the difficult) about Mom's habits, quirks and routines. It says a lot about their care and interaction with Mom. It is a comfort to myself and our family to know that they are aware of all this and share it as well. It speaks volumes of your caring staff. They know what movies she watches, what books she is reading, where and what she does on Wednesday nights (and ask about it), amazing! The Nurse has been spectacular. Prompt at addressing all of my many questions and concerns, she literally jumped out of her chair once and said " Let's check on that". She follows thru with so many little things and has many suggestions to help Mom and myself, as Mom continues to change and decline. The personal touch on a weekend I was out of town: She left me a phone message saying she was working the weekend, and was watching and assessing Mom. Just give a call if I needed anything or had questions. It is difficult for both the individual and the family to travel this path. All these examples of the personalized care that your staff provides Mom and in extension our family is priceless. Thank you to ALL of your staff. You should be proud of the service and care you provide. Our family couldn't have found a better home for Mom. Thank you

The initial intake involved a (literally) 15- minute in-person assessment of my loved one in their home by the site nurse before the move. The interview, combined with medical records provided directly from a geriatric physician (the primary care doctor), established the level of care. I was unhappy to find out via the auto withdrawal for rent (!) that the rate was increased by over $1,000/month in the week immediately after move in. I understand that it's hard to gauge a patient's exact needs in a short period of time and they increased the level of care but that it wasn't communicated is what really set me off. When I inquired, they apologized and admitted the lack of communication was an oversight. I observed, that despite having a small (under 15 total) population, some of the care staff seemed less than engaged. Others were great. And while some were nice and helpful there were one or two that were not friendly and seemed annoyed to have to answer my questions about my loved one and their transition into this environment. These same staff were seen on the floor scrolling their phones while the residents watched tv nearby. I guess it makes sense those 2 things happen at the same time, staff need a break too. My loved one has moved from this facility. I don't think it's bad, seemingly everyone is safe, but if you're able, tour it more than once and if possible at an unscheduled time, you may get a better sense of the care there. This review is based on a short residency at this facility.

Ultimately, it became clear to me that VMP does not like staff who speak up and their primary goal is to better cover up issues rather than actually fix them. They are currently under corrective action from the state and have a one star rating. The HR manager slipped up today when I pressed her and admitted she had conversations with the CEO, COO, and therapy director and that they were concerned that I would be one to report issues that I saw and that was the primary reason that they decided to rescind my job offer. Prior to this conversation, I was given a bunch of other conflicting reasons and lies were made up, including claiming that I refused to accept the pay they offered me even though I only learned in that conversation that they actually got exactly what I asked for approved. Holly (HR) and Lisa (HR manager) were the two people I've been dealing with for the last month and I would recommend that you get everything in writing or record your conversations with them in particular to protect yourself (WI does not require you to let them know you are recording them FYI). Holly said so many lies it made my head spin and the few times I had hard evidence that she was lying her story would change entirely and she would say that she "made a mistake" and "misspoke". Lisa said that Holly followed company policy, even when I pointed out that Holly was even caught in a lie in a conversation we all had on the phone together. Lisa also claimed that no one has ever said Holly is unprofessional, implying that I was the issue, but in the exact same sentence said that Holly admitted to being unprofessional with her lack of communication with me and follow through of actually calling me when she said she would (for reference, on several occasions she would tell me she'd call me on Friday, and then we didn't speak again until the following Thursday only because I would call her). Ultimately, the therapy department is understaffed right now, they told me how desperately they needed my help, and offered me the job on the spot, but they would rather operate understaffed than risk having an employee who advocates for the residents. I am passing this information along to the state too since they are still under corrective actions, but feel it's important that potential employees and residents are aware of the depth of problems here. Other past employee reviews on different websites also echo that anyone who speaks up about treatment of residents is fired, so be prepared to file a whistleblower claim if you take a job here. As someone who rarely reviews places, it really means something that this place made me concerned enough to warn people on several review platforms. I knew that VMP had high turnover in staff, but now I know why.

I do volunteer entertainment work so any assisted living-skilled nursing facility I review is going to be good at scheduling social activity and cultural stimulation for patients and residents, but St. Anne's seems extra special in this department. The outside isn't so attractive but the insides are clean and comfortable, the chapel is lovely and there are many friendly gathering rooms. Family and friends seem quite welcome but the most remarkable thing about this place is that the food is good, though they did foist some decaffeinated coffee on me.

My siblings and I visit often. She likes the meals and staff, and is very busy with the activities. She has made a number of friends there - and that was critical after my father passed. They have done a lot of remodeling in the past few years, and the common areas are beautiful and well maintained. There is a fitness center and pool, a pub, a library, a small store and a bank. She is Catholic and attends daily Mass. When my father was in home hospice, the staff was especially attentive and caring. I cannot say enough good things about her experience there - and about what we as family have seen when with her.

I would never recommend this place or want to live here myself. One positive thing I can say is that the front desk staff and food waitstaff are wonderful and helpful.
