This is a difficult review to write. Some aspects of care for OL were phenomenal and we were very…read morehappy, other considerations not so much. I'm going to base my star rating on CARE GIVEN and not the other aspects although they will be mentioned. I've never had to explore nursing home options before, and unfortunately like many, when it was needed it was at the time of need so we were ill prepared. I selected this nursing home based on a visit I had years ago and thought the residents were happy and well cared for. I enlisted the help of an elderly solutions "broker" I found online thru google, who was the foot in the door, although I don't know if that was necessary. Like many, I heard horror stories about nursing homes, and have been in several that were subpar visiting relatives over the years. It's believed that many will force you to sign over your house and assets. Surprisingly at OL that is not the case. In fact, they don't ask anything financial. They operate on a base rate and a $310/per month upcharge for every level of care needed. A level of care is based on every 30 minutes a day they tend to you, this is evaluated every month for accuracy so you could be paying a higher level one month and lower the next. Please note that the prices I'm providing here are based on a single room, single occupancy, non-dementia care. Memory care has a different set of pricing, and other style rooms, suites, efficiencies are available. The base rate was $4300/month + each tier of care. The nonrefundable entry fee is equal to one month. They operate on a month-to-month basis. Again, they ask no financial information. My dad lived out the last two months of his life at their Landisville facility and overall, we were very pleased with his care. The pros, aside from the pricing structure is the place is gorgeous, well kept, and the residents and staff seem happy. Residents are dressed and involved. There's a variety of activities every day, as well as outings. Transportation, move in assistance, medication management, all meals and activities, housekeeping, furnished room (if you choose), cable tv, basic supplies such as toilet paper, linens and laundry service are all included at the base level. Kara, who was our initial contact, was very friendly, and it felt like she had our dad's best interest at heart. She really knows how to "sell" their lifestyle, although some of her information was inaccurate. She promises the sun and the moon, however, either by miscommunication or staffing issues, not everything she promised was delivered. Some key furniture pieces that were promised upon check in were missing, but rectified. We were told 181 channels, however my dad complained that some required an additional payment, and channel offerings seemed to change every day. One day a channel would be free, the next it wasn't. We were told a landline phone with his own extension was included, which was a highlight for us given his difficulty in using a cell phone, only to discover we would need to have someone install the line, pay a monthly fee, and purchase a phone. A thorough nursing assessment was also promised when he arrived, but not done till later in the day when I mentioned it. Small things like that were all addressed both worth mentioning. Some things we can't overlook; dad was proud of his military service, and they have a veteran's wall complete with a picture/nameplate. I provided his picture a few days before he moved there, however by the time he passed - two months later it still wasn't hung. I had contacted the activity director on this several times prior. It was heartbreaking to see my dad look for it every day as he passed the wall on the way to meals, knowing he should be up there and wasn't. There was also an issue with obtaining a wheelchair thru insurance, which was a result of something as trivial as the provider needing an insurance number, which they had and never provided them, which once we got involved was addressed. As my brother put it there was a little more "hand holding" on some matters than we expected, however at the time of his passing I thought we worked out a lot of the kinks. What also was disappointing was that we & dad had grown to like the staff a lot, and since we were very hands on as a family communicated with them on the daily. However, when he passed no one reached out, sent a card, or acknowledged his death. The day he passed at the hospital I cleaned out his room, and a few staff came forward to offer their condolences but not key players, which was sad. The care he was given however was very good. They were attentive to his needs and treated him well. His room was spacious, newly renovated, and comfortable. PT & OT therapists work out of that building so getting his therapy needs met was easy. Communication was daily between us and key employees. He was clean, dressed, and tended to daily. Coming here probably gave us more time with him. He is dearly misssed.

My Great-Uncle has been living at Oak Leaf Manor (Landisville) for about a year…read more I have visited him at least twice a week since he has been there, so I am very familiar with Oak Leaf. The grounds are beautifully landscaped and well maintained. There is plenty of outside seating including two gazebo's. There is also an outside picnic area and nice walkways for the residents. The interior is wonderfully decorated for whatever season it happens to be. There is also a water fountain inside that is quite nice so sit around and relax. The dining room is nicely laid out and the food is good. The residents rooms are spacious and clean. The staff at Oak Leaf Manor is very friendly and caring. If any problems arise, the staff do their best to accommodate you as quickly as possible. There are a lot of activities for the residents, including BINGO, a theater room, scenic bus rides, crafts, and veterans activities. Overall Oak Leaf Manor has a very comfortable friendly atmosphere. If you are looking for a home for a loved one who can no longer live alone, I highly recommend Oak Leaf Manor.

The standards are to lie in medical records and then be proven wrong by a community doctor and then try to lie about something else! If you use choice it will take them several months to pay the bill if ever! Hopefully it will not cause your credit score to take a nosedive! Some medical providers within the community will not take choice because the Wilmington VAMC staff are delaying payment causing to community care to stop taking Vets who are paying with choice! Wilmington VAMC will and the VA will cause you to think "I would be better off dead". Following Vets around and even filling medications without just to get rid of the Vet is a norm! If your injury or illness does not fit with your VA quacks agenda it will disappear or never end up in your medical records. The place full of smiles and apathy! Never thank me for serving VA is 200 million dollar trash! Even when a Wilmington VAMC leaves a message and read what they wrote in your medical record it is also a lie!!! Bring a battle buddy and prepare to face a life long uphill battle against the Wilmington VAMC and the VA! Dr. Hsing-Chien James Cheng He is a terrible Dentist! Cheng prevents me from sending secure messages and will ask me questions but put the wrong answer down! I think Cheng either hates Vets or just me! I am not sure! Cheng should not be allowed to practice and should have be charged with medical crime! Cheng cannot hear anything should not be allowed to see any patients!

I would only recommend FVB if you protect your loved one and have an attorney look at your contract and beware of any promises. My mother was invited by Freedom Village to enroll in their "hardship" program in 2023. She went through their vigorous eligibility program and was approved. Both the facility and Corporate approved her request. She was approved again the following year and was granted additional funds. To our shock and dismay, after an audit last year my mother was told that she had to repay the funds and they increased her monthly payment substantially. They blamed the issue on us and took no responsibility for their change of heart. They changed the rules two years in and we had no standing due to a clause in the contract. They threatened to sue us if we didn't pay by the end of the year. Since we have so much money and time invested in this place, we have no choice but to keep my mother there.

We recently hired BAYADA's private duty aides to provide 24-hour care for our terminally ill…read moremother. My sister or I were almost always also present, but we needed an aide to assist with bathing, toileting, and so the family caregiver could sleep at night. We hired BAYADA aides for approximately 2 1/2 months during the COVID-19 pandemic, until my mother died. As a retired hospice employee, I have seen many home health care agencies, and had previously used two other Wilmington companies for Mom's care. BAYADA's private duty home health aides are far and away better trained and of higher caliber than any other such agency I've seen! PROs: 1) Services began very quickly, just 24 hours after I contacted the agency. 2) The office staff were organized and responsive. 3) Aides were consistently pleasant, respectful, on time, had good hygiene, reliable. The very few times we had problems, the office accepted our concerns and removed that aide from our schedule. 4) Aides were well-trained and enjoyed their job and their employment at BAYADA. 5) Generally positive attitudes! CONS: 1) In 2 1/2 months, we were unable to settle down to a schedule with less than five aides/week covering the 24/7 schedule, as we'd hoped. There were several factors that complicated scheduling, including: COVID-19, we had a large dog, our location (N. Wilmington.) The constant parade of new aides who each required orientation at the beginning of each shift, was tiring. 2) It is nearly impossible for an agency to guarantee 24/7 coverage and not have gaps. There were two times that employees did not show up, but the employee ending her shift needed to leave. If a family member had not been present, my mother might have been left alone. [although company policy states that the aide cannot leave until her replacement arrives, some of these aides were hurrying to another job or to care for their own child.] In sum, however, I was very, very pleased with BAYADA's private duty aide service.
