I strongly urge anyone considering this facility to look elsewhere before trusting them with a loved one. Unfortunately, this place was poor in the past and has not improved. My aunt's experience was deeply upsetting. The level of care we witnessed was unacceptable, and at times, disturbing. What made matters worse was the staff's consistently rude and dismissive attitude. It's incredibly disheartening to see people treated this way in a place that's supposed to provide care and compassion. From our experience, this doesn't feel like an isolated issue, but an ongoing pattern. Please do your research before making a decision. I would not recommend this facility to anyone. read more

My mother has been a resident there for 2-3 years now. The staff has always been very helpful and courteous with her needs. My dad was also a resident there years ago when this company was under a different management. The care was not like it is today. They've done a huge turn around with this facility. The nurse director, Susanne Martinez, has been exceptional with the dedication it takes to care for its' residents as well as guiding her nurses to go above and beyond to provide excellent care for their patients. My mother is well taken care of and our family is grateful for their attention. read more

I can literally say the whole experience was a perfect 10. Everyone exceeded expectations, but I'm gonna say that the office experience with Diana Ramirez and her assistant was like nothing I've ever experienced. You can tell who loves there job and go out of the way. I have to say you guys literally hit the lotto with this type of employees. Just the fact that they went beyond measures to help me in my dilemma. They solved my catastrophic issue and took away all concerns by taking my grandma home. My mom was transfered to this place from UCSD on hospice, and she was there for a little over 24 hrs before we took her home since we were aware she didnt have much time left. There are not a lot of options for facilities in the area unfortunately. Staff is unprofessional, front desk didnt even let go of her breakfast to let me know I had to wait a few minutes until 8am to be able to go in. The nurse in front of my mom said "oh yes, lots of people take their loved ones home to die" yes, she said that in front of my dying mom, not compassionate at all. Cna was licking a lollypop while giving us forms to sign. But the biggest issue was with management and the nurse in charge. We did take our mom home, we arranged transportation because we couldnt wait 2 days for hospice to do their evaluation. My brother brought her pain medication to the facility and they wouldnt give us her medication to take home with her, the one WE picked up from the pharmacy and handed to their nurse. Hospice was very helpful and called them and gave a verbal order to send home with medication, and the nurse and manager Andre still refused to give us her medication arguing it was their medication and will be disposing the medication. We try to make them understand for almost an hour, and they kept refusing. I knew my mom was going to need this medication for pain and respiratory distress so after mom was home I tried again talking to hospice to see if they could do something, besides waiting until the next day for the pharmacy to open. And they sent me the verbal order time stamped where it said to send home with meds. He got in touch with the facility and I had to go pick up the medication. Again, they just handed me the medication, didnt have to sign anything. Uncompassionate and unprofessional facility. I will be making a formal complaint to Long term ombudsman for improper discharge of a resident, and violation of resident rights. read more

If you want your special person to end up back in the hospital in worse condition put them in this facility. Want ALL their stuff to disappear? Put them in this so called facility... Horrible care and horrible rude unprofessional staff. Thief's and rude staff. Guess they forgot why they went into working in healthcare. They need a new career that doesn't involve caring for others bc they definitely don't know how. Filed a police report and still so called waiting on a call back from upper management for missing items after my mom was sent to the hospital and then got rid of all her stuff including her room. read more

Mi ser querido ingresó a El Centro Post-Agudo con fines de rehabilitación. Mientras estaba en cama, mi ser querido tenía la necesidad de ir al baño, pero debido a problemas de movilidad, no podía levantarse para ir al baño sin ayuda. Mi ser querido presionó el botón de llamada innumerables veces sin respuesta de nadie. En consecuencia, hubo un accidente de defecación. El personal incompetente tardó 4 horas en venir a limpiar a mi ser querido. Como resultado, mi ser querido adquirió una enorme llaga en etapa III. Andrés Área me aseguró que se haría una investigación sobre la negligencia. Debido a que nunca fui contactado por Andrés Área sobre el resultado de la "investigación" que supuestamente realizó, y la negligencia del personal, sacamos a nuestro ser querido de este deplorable lugar después de 3 días de haber sido ingresado. ¡Si realmente amas a alguien, te imploro que NO lo admitas en este INFIERNO! My loved one was admitted to El Centro Post-Acute for rehabilitation purposes. While in bed, my loved one had the need to use the restroom, but due to mobility issues, couldn't get up to use the restroom without assistances. My loved one pressed the call button countless times without response from anyone. Consequently, there was a defecation accident. It took the Incompetent staff 4 hours to come and clean my loved one up. As a result my loved one acquired a massive stage III bed sore. Andres Area assured me that there would be an investigation conducted into the negligence. Due to the fact that I was never contacted by Andres Area concerning the outcome of the "investigation" that he supposedly conducted, and the negligence from the staff, we removed our loved one from this deplorable place after 3 days of being admitted. If you truly love someone, I implore you NOT to admit them to this HELL HOLE!!! read more

My mom was transferred here in August 2024. We ended the stay after less than a week, although she was eligible and needed a longer period of time, because of medical emergency that occurred. She needed help when she was struggling to breathe and no once came to help her. She couldn't find the call button and struggled to call for help. Staff act very unprofessionally as though they are at home or at a party, calling out to each, talking loudly, laughing, visiting. How about checking on the patients!!!!! If that is not bad enough to make you think twice about placing a loved one here, my other complaint is the unhealthy food, high in carbs. My mom's doctor asked that my mom receive a diabetic-suitable diet; instead, every day she was served starchy foods, no fresh vegetables/fruit at all. I complained time and time again reminding staff of our doctor's request for a diabetic sensitve diet. Yet my requests were ignored. I don't recommend this facility. read more

First of all I had my doubts about this place but these past few weeks that my grandfather has been here I have a very different mindset. In my perspective every place is always going to have its upps and downs when it comes to some of the staff or the building it self, I feel like that's inevitable. In the other hand Post-Acute is a good place with good staff. Some workers that did stand out where Lizbeth Rocha and Diana Ramirez. They were extremely nice and helpful and would always give me an answer to my many questions! read more
